    Notes: Summary Tissue factor (TF) is the primary cell-bound initiator of the coagulation protease cascade. The cytological distribution of TF in various tissues may be described on the basis of immunohistochemistry with epitope-defined monoclonal antibodies and the extravascular distribution of TF apparently represents a haemostatic envelope ready to activate coagulation when vascular integrity is disrupted. The present study localized TF in human breast cancer tissues when compared with normal breast gland tissues and benign disorders of the mammary gland. By use of a cocktail of three epitopedefined monoclonal antibodies, TF was detected only in the myoepithelia of the resting breast gland. In proliferating disorders like fibrocystic disease or in fibroadenomas, both myoepithelia and luminal epithelia showed TF expression. Of 115 breast cancers 93 reacted with anti-TF, in an inhomogeneous manner in terms of intensity and number of positive cells. There was a tendency for more positive and intensely stained cells to be found in well-differentiated structures such as tubules. Invasive ductal carcinomas exhibiting more positive and more strongly stained cells were less commonly metastatic to lymph nodes when compared with the tumours with no detectable or very low TF immunostaining. A semiquantitatively recorded score of TF immunostaining correlated with the procoagulatory activity measured (7 fibroadenomas and 24 carcinomas). The results of this study suggest that proliferation and differentiation of the mammary gland is associated with enhanced TF expression in the epithelia which are negative for TF staining in the resting gland. Malignant growth is characterized by randomly expressed epithelial TF, which expression is enhanced and more frequent in well-differentiated tumour cells.

    Notes: Als Endophthalmitis wird eine Entzündung der intraokularen Gewebe und Flüssigkeiten bezeichnet. Diese kann als gefürchtete Komplikation nach ophthalmochirurgischen Eingriffen, aber auch endogen und posttraumatisch auftreten. Oft droht eine rasche, manchmal schmerzhafte Erblindung, selten auch der Verlust des Auges. Deshalb ist die prompte Diagnose und schnelle und effektive Behandlung für die Prognose entscheidend. Die akute postoperative Endophthalmitis ist aufgrund der hohen Anzahl intraokularer Eingriffe (Kataraktchirurgie) die häufigste Form dieser Erkrankung. Besteht der klinische Verdacht mit Schmerzen, Visusminderung, Vorderkammerreizzustand mit Hypopyon, Glaskörperinfiltration und erschwertem oder fehlendem Funduseinblick, müssen Glaskörperproben zur Erregerbestimmung gewonnen werden. Dies gelingt am sichersten durch eine Vitrektomie. Dabei werden nicht nur schädigende Entzündungsmediatoren ausgespült, sondern auch Anibiotika – gegebenenfalls in Kombination mit einem Antimykotikum und/oder Steroiden – intravitreal injiziert. Zur Dämpfung der Immunreaktion ist der Einsatz von systemisch applizierten Steroiden vorteilhaft. Im Verlauf muß die antibiotische Behandlung gelegentlich nach Resistenzbestimmung angepaßt werden. Die Auswahl der Präparate zur systemischen Behandlung hängt wesentlich von ihrer Penetrationsfähigkeit durch die Blut-Augenschranke und bei intravitrealer Applikation von ihrer Neurotoxizität ab. Entscheidend zur Eingrenzung der Diagnose Endophthalmitis ist die gezielte Anamnese. Wegen des zu erwartenden Erregerspektrums ist die Einordnung in postoperativ, posttraumatisch oder endogen entscheidend. Hieraus ergeben sich konkrete Behandlungsempfehlungen.

    Notes: Abstract  Background:Trophic factors [e.g. basic fibroblast growth factor (bFGF)] released by transplanted retinal pigment epithelial (RPE) cells are able to slow down the hereditary degeneration of the retina in the Royal College of Surgeons rat in sites distant from the site of transplantation where rod outer segment (ROS) phagocytic activity is not reconstituted by the transplants. Methods:To investigate whether iris pigmented epithelial (IPE) cells are also able to generate this rescue by trophic factors, we transplanted IPE cells from Long-Evans rats into the choroid and subretinal space of 17 young RCS rats. The eyes were enucleated after 6 months and prepared for light microscopy. Six age-matched RCS rats served as controls. Light microscope sections from the whole choroid, healthy choriocapillaris, transplanted cells and the maximum thickness of the choroid, and outer nuclear layer parameters were analyzed by computer-assisted morphometry. Results:In transplanted animals photoreceptor cells were rescued from degeneration although the majority of the transplanted IPE cells were located in the choroid. In the non-transplanted group photoreceptors were absent. Conclusions:Transplantation of IPE cells slows down degeneration of the photoreceptors in the RCS rat. This photoreceptor-sparing effect by the IPE cells was observed even when the transplants were predominantly located within the choroid. The beneficial effect observed may be related to trophic factors possibly secreted by the transplanted IPE cells.
