    Notes: A monoclonal anti-idiotype antibody (IgG1k MoAb 3B11D4) raised against the amyloidogenic DEPλ chain dimer binds a conformational idiotope also present on the monoclonal DEP IgA immunoglobulin. MoAb 3B11D4 does not recognize the reduced and alkylated λ chain monomers, nor the 15-17-kDa fibrillar light chain fragments which have the same N-terminal sequence of the urinary light chains. The lack of about 70 ammo acid residues ofthe C terminal of the protein prevents the formation of the self-limiting dimer and may facilitate the deposition of the fragments into amyloid fibrils. MoAb 3B11D4 recognizes the plasma cell clone in bone marrow and 9% of circulating B lymphocytes. Panning experiments demonstrate that this antibody has the capability to selectively eliminate the idiotvpe positive cells from peripheral blood. Antibodies with these characteristics could become a useful tool for better understanding the pathogenesis of the disease and for new therapeutic options.

    Notes: Background : Eradication rates of Helicobacter pylori with standard triple therapy are disappointing, and studies from several countries confirm this poor performance.Aim : To assess the eradication rate of a new sequential treatment regimen compared with conventional triple therapy for the eradication of H. pylori infection.Methods : One thousand and forty-nine dyspeptic patients were studied prospectively. H. pylori-infected patients were randomized to receive 10-day sequential therapy [rabeprazole (40 mg daily) plus amoxicillin (1 g twice daily) for the first 5 days, followed by rabeprazole (20 mg), clarithromycin (500 mg) and tinidazole (500 mg) twice daily for the remaining 5 days] or standard 7-day treatment [rabeprazole (20 mg), clarithromycin (500 mg) and amoxicillin (1 g) twice daily]. H. pylori status was assessed by histology, rapid urease test and 13C-urea breath test at baseline and 6 weeks or more after completion of treatment.Results : Higher eradication rates were found with the sequential regimen compared to the standard regimen (intention-to-treat: 92% vs. 74%, P 〈 0.0001; per protocol: 95% vs. 77%, P 〈 0.0001). Higher eradication rates were also seen in patients with peptic ulcer disease and non-ulcer dyspepsia. In both treatments, compliance was similar (〉 90%), as was the rate of side-effects, which were mild.Conclusions : This 10-day sequential treatment regimen achieves high eradication rates in peptic ulcer disease and non-ulcer dyspepsia.

    Notes: Riassunto L'autore studia fotometricamente le modificazioni quantitative dell'acido desossiribonucleinico (a.d.r.n.) nel corso della mitosi degli eritroblasti nel midollo dell'uomo adulto normale. Egli osserva che la quantità di a.d.r.n. contenuta in queste cellule alla metafase è identica al tenore cariologico degli eritroblasti a nucleo tipico in intercinesi. Nell'anafase si compie una semplice ripartizione del contenuto in a.d.r.n. e di conseguenza la quantità di a.d.r.n.; posseduta da ogni singola stella cromosomica alla fase di diaster è pari alla metà del tenore in a.d.r.n. dell'aster. I nuclei degli eritroblasti figli, alla telofase precoce, hanno un tenore in a.d.r.n. pari press'a poco alla metà del tenore cariologico della cellula madre, e solo dopo la loro ricostituzione, terminati i fenomeni meccanici della cariocinesi, ha inizio in essi la sintesi dell'a.d.r.n. I dati sovraesposti sono in accordo con quelli raccolti daLison e collaboratori negli ucceli e nei piccoli mammiferi, e dimostrano che anche nelle cellule somatiche dell'uomo, la sintesi in a.d.r.n. non avviene durante l'evoluzione del processo cariocinetico, ma bensì dopo la ricostituzione dei nuclei, alla telofase.
