Summary A retrospective case-control study (1990–1995), the Berlin Urothelial Cancer Study (BUS), examined analgesics and laxatives as risks for the induction of urothelial cancer in renal pelvis, ureter and bladder. Especially for renal pelvis cancer could observe substance and dose specific risk of compound analgesics. The analgesic substances Phenacetin, Paracetamol, Acetylsalicylic acid (ASA) and Pyrazolones were assessed. Besides a risk of contact laxatives (chemical or anthranoide ingredients) for urothelial cancer was found, not yet described. The highest risk shows the anthranoide plant Senna. Thus this study confirms the risk of specific analgesic ingredients and found an evidence for a new risk of contact laxatives. As both, analgesics and contact laxatives, are typical OTC- (“Over the counter”) products, a severe controlling is demanded and for laxatives further studies are needed.

Résumé Une étude rétrospective par cas-témoins (la BUS) examina d'analgésiques et laxatifs comme des risques pour les carcinomes des voies urinaires (1990–1995). Spécial pour les carcinomes du bassinets des reins on a constaté des risques spécifiques des substances et des doses d'analgésiques antipyrétiques des calmants combinés en particulier. On a analysé les substances analgésiques: phénacétine, paracétamol, acide acétylsalicile et les pyrazolones. L'existence d'un risque des laxatifs de type «laxatifs de contact» appelés aussi «stimulants» est un fait nouveau. Ces laxatifs qu'ils proviennent de substances chimiques ou végétales anthranoides peuvent favoriser l'évolution du cancer des voies urinaires. On compte avec les plus grands risques pour le végétal Senna. Les laxatifs ainsi que les calmants antipyrétiques font partie des préparations typiques de médicaments OTC («Over the counter»), ils représentent sur le marché des médicaments une grande partie de la vente. L'étude présente apporte ainsi une nouvelle contribution á l'évaluation des risques des médicaments OTC dans les cas de tumeurs des voies urinaires.

Zusammenfassung In eine retrospektiven Fall-Kontrol-Studie (1990–1995), der Berliner Urothelkarzinom-Studie (BUS), wurden Analgetika und Laxantien als Risiken für die Entwicklung von Urothelkarzinomen im Nierenbecken, Ureter und Harnblase untersucht. Speziell für Nierenbeckenkarzinome konnten substanz- und dosisspezifische Risiken antipyretischer Analgetika meist in Form von Kombinationsschmerzmitteln ermittelt werden. Bewertet wurden die schmerzstillenden Wirkstoffe Phenacetin, Paracetamol, Acetylsalicylsäure und Pyrazolone. Neu ist der Nachweis eines Krebsrisikos durch Laxantien für die ableitenden Harnwege vom Typ Kontaktlaxans (auch Stimulanz genannt) sowohl mit chemischen als auch mit pflanzlichen anthranoidhaltigen Wirkstoffen. Die anthranoidhaltige Plfanze Sennes zeigt die höchsten Risikowerte. Laxantien sowie die antipyretischen Analgetika gehören zu den typischen Selbstmedikationspräparaten, die mit hohen Absatzanteilen auf dem Arzneimittelmarkt vertreten sind. Die vorliegende Studie leistet somit einen neuen Beitrag zur Bewertung von Risiken durch die Selbstmedikation. Für die Bewertung des Krebsrisikos von Laxantien werden weitere Studien gefordert.
