    Notes: Abstract The pulmonary toxicity of gold salts is an uncommon cause of life-threatening respiratory failure. Currently, patients who suffer from this do not need mechanical ventilation, and the toxicity can be difficult to diagnose when it occurs in patient with an illness producing pulmonary manifestations. We report a case of severe respiratory failure due to gold salt toxicity in a patient suffering from rheumatoid arthritis requiring mechanical ventilation. At such a time, the poor respiratory function makes some diagnostic procedures harmful. The diagnosis can be made after the exclusion of other causes of rheumatoid lung when the patient's poor respiratory status precludes invasive exploration. The clinical findings, radiological features, and results of pulmonary function tests may be enough to diagnose gold-related pneumopathy. This avoids the need for bronchoscopic examination or transfer of the patient for computed tomography. Attention must be paid to this complication because the outcome and functional prognosis are better when pulmonary involvement is gold related: in our case steroid therapy was life-saving and induced complete recovery of the lung damage.

    Notes: Abstract  This study was carried out to investigate the effect of very high cadmium concentrations (50 and 500 μg Cd g–1 soil) on some biochemical and microbiological measurements under laboratory conditions involving daily soil samplings. The data for both DTPA- and water-soluble Cd showed two distinctive patterns during soil incubation; from 0 to 4 days, values were about 50–500 and 1–100 μg g–1 dry weight soil, whereas they decreased markedly after 7 days. Both daily respiration and the ATP content but not the microbial biomass C determined by the fumigation–extraction method were lowered by high DTPA- and water-soluble Cd concentrations. Dehydrogenase and phosphatase activities as well as both enzyme activity : microbial biomass ratios were decreased by the high DTPA- and water-soluble Cd concentrations. In the first 2 days of incubation, the metabolic quotient (qCO2) was also decreased by the highest values of available Cd. The early (after 6 h) mineralization of l- but not d-glutamic acid to CO2 was inhibited during the 0–4 day incubation period by the highest Cd concentration. Possibly the l-enantiomer was used by a larger fraction of soil microorganisms than the d-enantiomer or, if they were used by the same fraction of soil microorganisms, the d-enantiomer was mineralized at a lower rate. The l- : d-glutamic acid respiration ratio was decreased by the high available Cd content because under polluted conditions soil microorganisms probably discriminated less between the two stereoisomers of glutamic acid.

    Notes: Abstract  An incubation experiment lasting 120 days was carried out to ascertain the effect on the soil microbial activity and organic matter mineralization of adding a sewage sludge compost contaminated with two different levels of Cd to an arid soil. Two composts, with a low (2 mg kg–1) and high (815 mg kg–1) Cd content, respectively, were used in this experiment. Both composts increased the total organic C, humic substance and water-soluble C contents, the beneficial effects still being noticeable after 120 days of incubation. The most labile C fraction (water-soluble C) was the most sensitive to the high Cd content. The high Cd concentration decreased soil microbial biomass C and stimulated the metabolic activity of the microbial biomass, the metabolic quotient (qCO2) revealing itself to be a very sensitive index of the stress that the incorporation of a Cd-contaminated sewage sludge compost causes in a soil. The effect of Cd contamination on enzyme activities (urease, protease that hydrolyse N-α-benzoil-l-arginamide, phosphatase, and β-glucosidase) depended on the enzyme studied.

    Notes: Sewage sludge is increasingly used as an organic amendment to soil, especially to soil containing little organic matter. However, little is known about the utility of this organic amendment in the reclamation of soil polluted with heavy metals. We studied the effects of adding sewage sludge on enzymatic activities of a semi-arid soil contaminated with Cd or Ni in the laboratory. The activities of urease, phosphatase, β-glucosidase and protease-BAA were measured in soil containing concentrations of Cd or Ni in the range 0–8000 mg kg−1 soil, and their inhibition was compared with those of the enzymatic activities in the same soil amended with sewage sludge and containing similar concentrations of the heavy metals. The inhibition was tested for three different incubation times to determine changes in the effect of the heavy metals on hydrolase activity with the time elapsed after contamination. Ecological dose (ED) values of Cd and Ni were calculated from three mathematical models which described the inhibition of the enzymatic activities with increasing concentrations of heavy metal in the soil. For urease and phosphatase activities, the ED values for Cd and Ni increased after application of sewage sludge to soil, indicating a decrease in Cd and Ni toxicity. The other two enzymes (β-glucosidase and protease-BAA) were less sensitive to Cd or Ni contamination, and it was more difficult to determine whether addition of sewage sludge had affected the inhibition of these enzymes by the heavy metals.

    Description / Table of Contents: Résumé L'oedème de la synoviale pendant l'intervention apparaît comme une complication technique très fréquente au cours des arthroscopies réalisées en utilisant le sérum salé comme liquide de perfusion articulaire. Les auteurs ont étudié le comportement de 12 échantillons de membrane synoviale humaine en contact avec du sérum salé, Ringer lactate, Dextran 40.000 à 5% et Dextran 40.000 dilué à 10% avec du sérum salé. On a constaté au cours de ce travail que la solution de Dextran 40.000 à 10% dans le sérum salé diminue la quantité d'eau retenue par la synoviale sans la léser. En conclusion, on peut dire que cette solution représente le liquide idéal pour l'irrigation intraarticulaire de toutes les arthroscopies dont la durée peut être supérieure à 30 minutes.
    Notes: Summary Synovial oedema is a frequent complication in arthroscopic procedures performed with normal saline as the irrigating fluid. The authors have studied the effect of saline solution, Ringer lactate, 5% Dextran and 10% Dextran in normal saline on 12 specimens of human synovial membrane. They found that 10% Dextran in normal saline decreases the water content of the synovium without causing damage, and recommend this solution for procedures lasting longer than 30 minutes.

    Description / Table of Contents: Résumé Le but de cette étude est d'évaluer les effets directs des ondes de choc de haute énergie sur l'os cortical du mouton, afin de préciser leur utilisation éventuelle en chirurgie orthopédique. Les animaux ont été divisés en plusieurs groupes selon le type d'intervention pratiqué, fenêtres ou perforations, et selon le nombre et l'intensité des ondes de choc appliquées. Dans aucun des cas il n'y a eu de fragmentation ou d'érosion osseuse. Les ondes de choc ont entraîné une réaction intramédullaire au niveau de la corticale opposée aux orifices bombardés, sous forme de tissu mésenchymateux, siège de nombreuses cellules et d'une augmentation de la vascularisation. Les ondes de choc peuvent également perturber la reconstruction normale de l'os. Dans le groupe de contrôle les travées osseuses passent d'un bord à l'autre de la fenêtre, contrairement à ce qui se produit chez les animaux bombardés où les travées proviennent de la corticale opposée. Nous avons noté que les ondes de choc, aussi bien à haute qu'à basse énergie, provoquent d'importants hématomes musculaires et sous-cutanés.
    Notes: Summary It has been proposed that high energy shockwaves could be used to create microfractures in cortical bone. This quality might be exploited clinically to perform closed osteotomies and promote healing in nonunion (15). However, no study has previously documented the effect of shockwaves on cortical bone “in vivo”. We report an investigation designed to demonstrate the effect of shockwaves on mature cortical and healing bone. An osteotomy was performed on the tibiae of 37 lambs; two weeks later the operation site was exposed to shockwaves. Three weeks later the lambs were killed and specimens of the bone examined histologically and radiographically. Shockwaves had no effect on the periosteal surface of mature cortical bone, but on the endosteal surface some new trabecular bone was seen. Healing of bone was delayed by the shockwave therapy. We conclude that there is currently little place for shockwave treatment in clinical orthopaedics.

    Notes: Summary Venous digital subtraction angiography (vDSA) and computed tomography (CT) are two non-invasive techniques that allow direct imaging of coronary grafts. Neither of them is 100% accurate when compared to selective angiography. We studied 52 patients with 107 coronary grafts (101 saphenous and 6 mammary artery grafts) by CT and vDSA. Fifty patients had control selective angiography and 2 had control digital arterial aortography. CT correctly diagnosed 88 of the 95 patent grafts and 8 out of the 12 occluded grafts (in 1 patient 3 patent grafts could not be well analyzed owing to artifacts from pacemaker wires). vDSA correctly identified 93 patent grafts and the 12 occluded grafts, allowing the correct diagnosis of the 11 non/or misdiagnosed CT grafts. CT allowed the correct identification of ihe 2 misdiagnosed vDSA grafts. These results show that when there was agreement between CT and vDSA there were no diagnostic errors and that the combination of these two non-invasive techniques may avoid selective angiography for studying coronary bypass graft patency.
