    Notes: Übersicht Die elektronegativen Gase der Methanreihe C Cl4, CHCL3, CH2Cl2, CH3Cl, CH4 sowie CCl2F2 wurden hinsichtlich ihrer Beeinflussung der Durchbruchfeldstärke im Gemisch mit Luft und mit Wasserstoff mittels einer kleinen Funkenstrecke experimentell eingehend untersucht. Dabei wurde das Verhältnis von Durchbruchfeldstärke in dem Gasgemisch zur Durchbruchfeldstärke im reinen Trägergas, — also der Steigerungsfaktor — in Abhängigkeit von einerseits dem Sättigungsgrad und andrerseits den prozentualen Volumenanteilen gewonnen.

    Notes: A system for the detection of a Borrelia burgdorferi (Bb)-specific gene segment in formalin-fixed, paraffin-embedded skin lesions is described. A nested polymerase chain reaction technique is used to selectively amplify in vitro a short segment of a Bb-specific gene recently described by Rosa et al. (J Infect Dis 1989: 160: 1018). The design of oligonucleotide primers for the amplification of a relatively short gene segment allows the successful analysis of DNA which has been altered by fixation in formalin. Using this technique, Bb-specific DNA was clearly identified in 8 of 12 specimens of erythema chronicum migrans and in 1 case of lymphadenosis benigua cutis. These skin lesions are known to represent cutaneous manifestations of Lyme disease. Negative control reactions, using DNA from borrelial strains not related to Lyme disease, were negative. The system enables the dermatopathologist to identify Bb in routinely fixed clinical specimens and allows the rapid analysis of various skin diseases for which an association with Bb so far has only been hypothesized.

    Notes: We report a case of lupus vulgaris with typical clinical and histological findings. Mycobacterium tuberculosis was not only identified by a conventional culture technique, but also by a recently established system which has been designed to detect mycobacterial DNA in formalin-fixed, paraffinembedded tissue by polymerase-chain reaction (PCR). Because results can be obtained within days, the PCR-based technique may markedly facilitate the diagnosis of skin tuberculosis.

    Description / Table of Contents: Zusammenfassung In einer prospektiven, randomisierten Multizenter-Studie zur Therapie des Erythema migrans wurden 40 Patienten mit täglich 1 g Ceftriaxon intramuskulär über fünf Tage und 33 Patienten mit täglich 3 × 1 Mill. I. E. Phenoxymethylpenicillin über 12 Tage behandelt. Die Nachbeobachtung betrug durchschnittlich 10±5 Monate. Acht mit Penicillin (24%) und sechs mit Ceftriaxon behandelte Patienten (15%) entwickelten leichtere spätere Manifestationen. Zwei mit Ceftriaxon und ein mit Penicillin Behandelter wiesen erhöhte IgG Antikörper-Titer 10 bis 20 Monate nach Therapie auf.Borrelia burgdorferi konnte bei 29 von 56 Patienten (52%) vor und bei einem Patienten der Penicillingruppe, aber bei keinem Patienten der Ceftriaxongruppe nach Behandlung aus dem Erythema migrans isoliert werden. Ceftriaxon war dem Oral-Penicillin bei einer Untergruppe von Patienten mit mehr als einem Symptom vor Therapie überlegen (p〈0,01), aber nicht bei der Gesamtauswertung klinischer, serologischer und bakteriologischer Erfolgsdaten. Ceftriaxon sollte dem oralen Penicillin bei Patienten mit schwererer früher Lyme-Borreliose vorgezogen werden.
    Notes: Summary In a prospective randomized multicenter trial for the therapy of erythema migrans, 40 patients received ceftriaxone 1 g daily for 5 days and 33 patients obtained phenoxymethylpenicillin, 1 million units 3 times daily, for 12 days. Follow-up was for a mean of 10±5 months. Eight oral penicillin recipients (24%) and six ceftriaxone recipients (15%) developed minor consecutive manifestations. Two ceftriaxone and one penicillin recipient(s) still had elevated IgG antibody titers 10 to 20 months after therapy.Borrelia burgdorferi could be isolated from the erythema migrans in 29 out of 56 patients (52%) before therapy and in one oral penicillin recipient but none of 24 other patients after therapy. Ceftriaxone was superior to oral penicillin in a subgroup of patients with more than one symptom prior to therapy (p〈0.01), but not in the overall evaluation of clinical, serological and bacteriological outcome data. Ceftriaxone ought to be preferred to oral penicillin in patients with more severe early Lyme borreliosis.

    Notes: Summary More and more β-lactamase-producing, penicillin-resistant strains of Neisseria gonorrhoeae are nowadays isolated in most parts of the world including the European continent. Therefore, there is a need for reliable alternatives in the treatment of gonorrhoea. Cefotaxime — first representative of a new generation of β-lactamase-resistant cephalosporins — was evaluated for this reason, first in vitro and then in vivo. All 191 strains tested proved susceptible, the highest MIC amounting to 0.03 μg/ml, a drug concentration well exceeded in plasma for more than 4h after i.m. application of 0.5 g cefotaxime. In contrast to other investigators we have therefore decided to test this relatively small dose clinically without the addition of probenecid. The clinical results were excellent: 146 of 151 patients (96.7%) were cured. In the female sub-group (34 patients) the cure rate amounted to 100%. Thus, the i.m. injection of 0.5 g cefotaxime as a single dose must be looked upon as a reliable regimen for the cure of uncomplicated gonorrhoea, both in males and females.

    Notes: Abstract Myositis is a rare manifestation of Lyme disease of unknown pathogenesis. This study describes the course of disease in eight patients with Lyme disease, aged 37–70 years, all of whom were suffering from histologically proven myositis. The Clnical, electrophysiological, and myopathological findings are reported. One patient showed signs and symptoms of myositis of all limbs. In six patients myositis was localized in the vicinity of skin lesions, arthritis or neuropathy caused byBorrelia burgdorferi. In another patient suffering from pronounced muscle weakness of the legs and cardiac arrest, inflammation of the myocardium, the conducting system and skeletal muscles was revealed at autopsy. Muscle biopsy revealed lymphoplasmocellular infiltrates combined with few fibre degenerations in three patients. The lymphoplasmocellular infiltrates were found predominantly in the vicinity of small vessels. Several spirochetes were stained in six of seven muscle biopsy samples by means of the immunogold-silver technique. Culturing ofB. Burgdorferi from the muscle biopsy samples was, however, unsuccessful. Antibiotic treatment succeeded in curing the myositis in four of six patients. In one patients signs and symptoms improved. One patient died from cardiac arrest caused by myocarditis and Guillain-Barré syndrome. The outcome is unknown in one patient. Clinical and myopathological findings indicate that Lyme myositis can be caused either by local spreading ofB. burgdorferi or an unknown antigen or toxin from adjacent tissues or haematogenously.

    Notes: Summary In Munich 169 isolates of Neisseria gonorrhoeae were investigated with respect to nutritional profiles (auxotypes) and antibiotic susceptibility. Of the 165 penicillin-susceptible strains 97 proved to be prototrophic (auxotype 1), 42 proline-dependent (auxotype 2). Only 3 arginine-, hypoxanthine- and uracil-dependent strains (auxotype 14) were found. A total of 10 different auxotypes were identified among the non-penicillinase-producing strains and one among the penicillinase producers. The minimal inhibitory concentrations of penicillin G ranged from 0.01 to 1 μg/ml, those of cefuroxime from 0.001 to 4 mg/ml; tetracycline and kanamycin had a narrow spectrum of effective concentrations, from 0.25 to 8 μg/ml and from 4 to 64 μg/ml respectively. The type 14 strains showed the well-known high susceptibility to penicillin G.
