    Notes: Abstract Objectives To study the ability of different ventilatory approaches to keep the lung open. Design Different ventilatory patterns were applied in surfactant deficient lungs with PEEP set to achieve pre-lavage PaO2. Setting Experimental laboratory of a University Department of Anaesthesiology and Intensive Care. Animals 15 anaesthetised piglets. Interventions One volume-controlled mode (L-IPPV201:1.5) and two pressure-controlled modes at 20 breaths per minute (bpm) and I:E ratios of 2:1 and 1.5:1 (L-PRVC202:1 and L-PRVC201.5:1), and two pressure-controlled modes at 60 bpm and I:E of 1:1 and 1:1.5 (L-PRVC601:1 and L-PRVC601:1.5) were investigated. The pressure-controlled modes were applied using “Pressure-Regulated Volume-Controlled Ventilation” (PRVC). Measurements and results Gas exchange, airway pressures, hemodynamics, FRC and intrathoracic fluid volumes were measured. Gas exchange was the same for all modes. FRC was 30% higher with all post-lavage settings. By reducing inspiratory time MPAW decreased from 25 cmH2O by 3 cmH2O with L-PRVC201.5:1 and L-PRVC601:1.5. End-inspiratory airway pressure was 29 cmH2O with L-PRVC201.5:1 and 40 cmH2O with L-IPPV201:1.5, while the other modes displayed intermediate values. End-inspiratory lung volume was 65 ml/kg with L-IPPV201:1.5, but it was reduced to 50 and 49 ml/kg with L-PRVC601:1 and L-PRVC601:1.5. Compliance was 16 and 18 ml/cmH2O with L-PRVC202:1 and L-PRVC201.5:1, while it was lower with L-IPPV201:1.5, L-PRVC601:1 and L-PRVC601:1.5. Oxygen delivery was maintained at prelavage level with L-PRVC201.5:1 (657 ml/min·m2), the other modes displayed reduced oxygen delivery compared with pre-lavage. Conclusion Neither the rapid frequency modes nor the low frequency volume-controlled mode kept the surfactant deficient lungs open. Pressure-controlled inverse ratio ventilation (20 bpm) kept the lungs open at reduced end-inspiratory airway pressure and hence reduced risk of barotrauma. Reducing I:E ratio in this latter modality from 2:1 to 1.5:1 further improved oxygen delivery.

The aim of this study was to isolate the effects of inspiratory time on airway pressure, gas exchange, and hemodynamics, while ventilatory frequency, tidal volume, and mean airway pressure were kept constant. Design: Randomized experimental trial. Setting: Experimental laboratory of a University Department of Anesthesiology and Intensive Care. Animals: Twelve anesthetised piglets. Interventions: After lavage the reference setting was pressure-controlled ventilation with a decelerating flow; I:E was 1:1, and PEEP was set to 75% of the inflection point pressure level. The I:E ratios of 1.5:1, 2.3:1, and 4:1 were applied randomly. Under open lung conditions, mean airway pressure was kept constant by reduction of external PEEP. Measurements and results: Gas exchange, airway pressures, hemodynamics, functional residual capacity (SF6 tracer), and intrathoracic fluid volumes (double indicator dilution) were measured. Compared to the I:E of 1:1, PaCO2 was 8% lower, with I:E 2.3:1 and 4:1 (p≤0.01) while PaO2 remained unchanged. The decrease in inspiratory airway pressure with increased inspiratory time was due to the response of the pressure-regulated volume-controlled mode to an increased I:E ratio. Stroke index and right ventricular ejection fraction were depressed at higher I:E ratios (SI by 18% at 2.3:1, 20% at 4:1; RVEF by 10% at 2.3:1, 13% at 4:1; p≤0.05). Conclusion: Under open lung conditions with an increased I:E ratio, oxygenation remained unaffected while hemodynamics were impaired.

    Notes: Abstract Experiments using a soluble, chemically defined fermentation medium provided important knowledge about the kinetics of efrotomycin biosynthesis. Equivalent titers were obtained in a batch process in both shaker flasks and fermentors. By extended feeding of both monosodium glutamate and glycerol at elevated temperatures, in combination with sulphuric acid pH control, the specific production rate was increased 2.8 fold and overall production rate was improved 5-fold. If the monosodium glutamate was fed too fast, then ammonium accumulated with indications of strong repression of efrotomycin biosynthesis at concentrations above 6 mM. In contrast to the complex medium used for this process, the chemically define medium was completely insensitive to changes in sterilization conditions.

    Notes: Abstract Seven phenylketonuria (PKU) patients aged 15–24 years were allowed unrestricted diet for 3 weeks. Three of these patients performed well on unrestricted diet according to visual reaction time variability (RTv 50–100 ms) and did not show significant changes when returning to the phenylalanine-restricted diet (RTv 70–100 ms). Neither did the concentrations of homovanillic acid (HVA) and 5-hydroxyindoleacetic acid (5-HIAA) in cerebrospinal fluid (CSF) change significantly. Four of the patients, however, performed rather poorly (RTv 120–220 ms) on unrestricted diet and improved significantly (P〈0.03) when the diet was restored (RTv 70–150 ms). The improvements were accompanied by significant (P〈0.01 and P〈0.02) increases (mean 52% and 109%) in CSF levels of HVA and 5-HIAA. Five PKU patients aged 15–23 years were allowed unrestricted diet or unrestricted diet supplemented with various amounts of tyrosine (106–194 mg/kg per 24 h). Two of these patients performed very well on unrestricted diet (RTv 60 ms) and showed little change when the unrestricted diet was supplemented with tyrosine (RTv 70 ms and 80 ms). The three other patients, who performed rather poorly (RTv 120–220 ms), improved significantly (P〈0.03) when the unrestricted diet was supplemented with tyrosine (RTv 70–140 ms). HVA in CSF increased significantly (P〈0.01) with the tyrosine supplement when the amount exceeded a threshold of approximately 80 mg/kg per 24 h. The simultaneous increase in CSF level of 5-HIAA showed a positive correlation (r=0.90; P〈0.02) with the increase in HVA concentration suggesting a functional interrelation between the dopaminergic and the serotoninergic nervous systems.

    Notes: Abstract The chance coincidence of an X-linked disorder with an autosomal recessive disorder in one child is described. The child had the clinical phenotype of a mucopolysaccharidosis and the activity of iduronate sulphatase was almost absent. Furthermore, fibroblasts from a typical Hunter patient were unable to correct the patient's fibroblasts. However, three 24 h urine samples collected at 18–36 months of age showed a nearly normal excretion of acid mucopolysaccharides. The boy died in liver coma at 3 years of age. Autopsy showed cirrhosis of the liver and changes in liver tissue consistent with α1-antitrypsin deficiency.

    Notes: An experimental and theoretical study of the 19F relaxation in solid tetrakis (pentafluorosulfanyl) hydrazine due to the reorientation of the SF5 groups has been undertaken. T1 and T1ρ have been measured from 138 to 292 K. T1 is very long, reaching a maximum of 1140 s at the lowest temperature for one sample, and is determined primarily by paramagnetic impurities. T1ρ appears to be influenced by paramagnetic impurities below 227 K and determined by the reorientation of the SF5 groups above 227 K. Expressions for T1 and T1ρ have been derived for two different reorientational models. In the first model the SF5 group is assumed to undergo 90° jumps about the C4 axis; whereas, in the second model the SF5 group is assumed to undergo rotational diffusion about the C4 axis. The T1ρ measurements above 227 K have been fit to the derived expressions to obtain the correlation time for the reorientation.
