    Notes: Summary Rhythm conversion in patients with post-thyrotoxic atrial fibrillation (AF) has been performed with disopyramide in order to evaluate the conversion rate and to test its effect on the maintenance of sinus rhythm after cardioversion. The duration of AF ranged from 9 to 122 months (mean 31.8 months). Of 81 patients, 12 (15%) with relatively short duration AF were converted to sinus rhythm with disopyramide. The remaining 69 patients required DC cardioversion, which restored sinus rhythm in 58 patients. The 58 DC-converted patients were divided into two groups: a disopyramide group (D group) and a non-disopyramide group (non-D group). The D group received disopyramide 300 mg per day for 3 months after DC cardioversion and the non-D group did not receive anti-arrhythmic drugs. During the early observation period, only one patient relapsed in the D group into AF, but 5 successive patients in the non-D group reverted to AF, forcing discontinuation of the non-D protocol. A second DC cardioversion performed on 3 of those 5 patients was followed by maintenance therapy with disopyramide 300 mg per day, and they remained in sinus rhythm. With the inclusion of those three subjects, sinus rhythm was still present in 44 of the total of 58 patients converted by DC (76%) at the time of follow-up (64 months). Thus, disopyramide was effective in rhythm conversion and it was essential for the maintenance of sinus rhythm after cardioversion in patients with post-thyrotoxic AF.

    Notes: Lymph nodes in gastric B-cell lymphoma: pattern of involvement and early histological changes Aims: This study aims to analyse the histological pattern of nodal involvement in gastric B-cell lymphoma and to detect early involvement of the lymph nodes. Methods and results: Histological findings of 37 resected primary gastric lymphomas with 1313 regional lymph nodes were analysed. The primary tumour was classified into four groups: MALT lymphoma, MALT lymphoma with a minor large B-cell lymphoma (〈20%), large B-cell lymphoma with MALT lymphoma, and large B-cell lymphoma without MALT lymphoma. Histological patterns of nodal involvement were divided into sinusoidal, subsinusoidal/marginal, follicular, and diffuse patterns. Semi-nested polymerase chain reaction (PCR) analysis for IgH gene rearrangement was performed. Nodal involvement was found in 2/13 (15%) MALT lymphomas, 5/6 (83%) MALT lymphomas with a minor large B-cell lymphoma, 9/12 (75%) large B-cell lymphomas with MALT lymphoma, and 6/6 (100%) large B-cell lymphomas without MALT lymphoma. The MALT lymphoma and MALT lymphoma with a minor large B-cell lymphoma showed a predominantly sinusoidal and subsinusoidal pattern, whereas diffuse pattern predominated in large B-cell lymphomas without MALT lymphoma and large B-cell lymphomas with MALT lymphoma. The follicular pattern was least common, being observed in 10.2% of large B-cell lymphomas without MALT lymphoma and large B-cell lymphomas with MALT lymphoma. Sinusoidal obliteration with permeation of small monocytoid cells into subsinusoidal zone is a characteristic finding suggesting early nodal involvement of MALT lymphoma. Conclusions: Histological patterns of nodal involvement in gastric B-cell lymphoma vary according to the histological grade. Immunostaining for CD20 with or without PCR analysis for IgH gene rearrangement would be a useful ancillary method to confirm lymphomatous involvement.

    Notes: Aim : To study the correlation between lymph node metastatic disease and various pathological parameters. Lymph node metastasis is the most important factor in determining the prognosis of patients with early gastric cancer and is significantly associated with the submucosal invasion of neoplastic cells. However, the depth of submucosal layer as well as the depth of submucosal tumour invasion varies among different cases.Methods and results : We studied various pathological parameters including the macroscopic appearance, location, size, area, differentiation, invasion depth, vascularity and fibrosis of submucosal gastric carcinomas (SMGC), using 248 age- and sex-matched tissue samples. The presence of lymphatic emboli, a larger area, greater size, a non-flat gross type and an increased vascularity of the tumour were significantly associated with node-positive SMGC. Among the three depth-related parameters, the ocular scale measurement, the sm3 method and the sm2 method, only the ocular scale measurement showed a significant correlation with node-positive SMGC. Using multivariate analysis, the incidence of lymph node metastasis increased in the lymphatic tumour emboli and in the tumours that invaded more than half of the submucosal layer.Conclusions : These histological indicators seem to be a feasible and easy way to predict lymph node metastatic disease from limited surgery specimens. Pathologists should carefully investigate the lymphatic invasion of tumour cells and measure the invasion depth of the tumour.

    Notes: We report magnetotransport studies on La0.67Sr0.33MnO3/SrTiO3/La0.67Sr0.33MnO3 trilayer junctions, fabricated using 90° off-axis sputtering. Films were grown on both (001) (LaAlO3)0.3–(Sr2AlTaO6)0.7 and (110) NdGaO3 substrates. The sputtered trilayers show improved junction resistance uniformity over those made using pulsed laser deposition. Cross-sectional transmission electron microscopy and atomic force microscopy studies confirm smooth interfaces and a uniform barrier. Magnetoresistances up to ∼100% are observed for junctions on (001) (LaAlO3)0.3–(Sr2AlTaO6)0.7 with a 30 Å barrier at 13 K and around 100 Oe. Junction magnetoresistance versus magnetic field behavior is more stable, indicating improved transport and magnetic homogeneity across the junction. © 2001 American Institute of Physics.

    Notes: In the design of distillation processes, it is frequently necessary to obtain a specified purity of certain product from a given feed mixture by the use of an existing column. The only variable within the designer's control is the reflux ratio. This design problem cannot be solved easily by existing computational methods. In general a trial-and-error approach is used. To avoid this difficulty, this problem is solved as an estimation problem by the use of the quasilinerization approach. To show the effectiveness of this approach, several problems are solved. In general, a four-digit accuracy is obtained in four iterations with very approximate initial approximations. Furthermore, this approach can treat the overdetermined system in a straightforward manner by the use of least squares criterion and minimization. Only the cases with constant total flow rates of the vapor and liquid streams within each section of the column are considered. The relative volatilities are also taken to be constant throughoutthe column.
