    Notes: Summary Enlargement of a small aneurysm during an eleven year period was encountered in a 32-year-old man who had experienced intracranial haemorrhage from a distally located AVM. Clipping of the feeding artery together with the aneurysmal neck was performed. It was very rare to observe the aneurysmal growth associated with the AVM. This case was considered worth reporting as supporting evidence that hemodynamic stress plays an important role in the development of an aneurysm.

    Notes: A normal mode theory of standard capacitive rf discharges between parallel electrodes is achieved in terms of electron plasma wave resonances by solving electrostatic fluid equations under so-called linear-diffusion and nonlinear-recombination gas-discharge regimes for a wide gas-pressure range. The rf plasma is produced by the main resonance and also by another new and lowest resonance of the electron plasma wave. With a very thin plasma-thickness, the plasma is produced by this new lowest resonance. Even under an off-resonance condition, the plasma can be produced. Behavior of rf plasma under the recombination regime is quite different from that under the diffusion regime, for example, nonlinear low-density discharges and hysteresis phenomena of up-/down-jumping with gas pressure. Thresholds of applied rf voltage and gas pressure for maintaining the discharge are obtained within a first-order approximation. © 2000 American Institute of Physics.

    Notes: The microstructural factors limiting the critical current density Jc have been investigated for the silver-sheathed Bi2223 tapes prepared by the powder-in-tube technique. In a hierarchy among them, which factor is the most effective in determining Jc depends on how extensively each of them occurs. The Jc was found to increase when the volume fraction of 2223 phase increases, suggesting the importance of the connectivity among 2223 grains. The Jc also increased with decreasing amount of residual carbon. The temperature and magnetic field dependences of Jc indicated characteristics of Josephson currents through superconducting–normal–superconducting barriers. After a discussion based on the alternative current limiting mechanisms proposed up to now, it is suggested that the homophase grain boundary limits the Jc. © 1996 American Institute of Physics.

    Notes: Xeroderma pigmentosum (XP) is a rare autosomal recessive inherited disorder caused by a defect in the normal repair of DNA of various cutaneous cell types damaged by exposure to ultraviolet radiation. We present our 7-year experience with 36 XP patients who either visited the Department of Dermatology or were seen in the medical camps arranged in remote areas for patients' welfare, from 1995 to 2001. For ease of discussion we classified all cases into the following subgroups on clinical grounds only: mild, those with light brown freckles on the face alone; moderate, those with dark brown freckles with burning on the face, neck, ears, chest, hands and photophobia but without other associated obvious cutaneous and ocular changes; severe, those with extensive dark brown freckles with burning on the exposed parts as well as on the unexposed parts of the body, i.e. the chest, back, abdomen and arms including other associated cutaneous and ocular changes such as ulcers and malignancy. Of 36 patients, three (8·3%) were classified as mild, nine (25%) moderate and 24 (66·7%) severe; there were 18 males and 18 females, age range 2–30 years (mean 8·9 years). Seventeen patients had cutaneous changes: actinic keratosis, keratoacanthoma, fissures and ulcerative nodules on the exposed parts of the body. Four patients had wide ulcers, along with mass formation and severe pigmentation on the face, neck and head. Twenty-nine patients developed ocular symptoms: photophobia, conjunctivitis, corneal keratitis and lid ulcer. One patient had complete loss of vision. Histopathological findings revealed that six patients had squamous cell carcinoma (SCC) on the face, head, ear or lip. More than one sibling (two to four) was affected in four families. The majority of cases (20/36, 55·6%) were from the Brohi tribe (skin type III), while the remaining cases (16/36, 44·4%) were from the Sindhi population (skin type IV). The large number of XP patients seen in those with skin type III (Brohi tribe) compared with skin type IV (Sindhi population) indicates that the skin type and the race has a considerable value in the pathogenesis of XP. Furthermore, 24 of 36 patients were in the severe group and six of these had SCC. Moreover, no neurological abnormalities were observed in our patients. All patients were treated according to disease severity by prescribing oral antibiotics, local steroids, sunscreens and/or chemotherapy followed by irradiation in malignant cases. Two patients died because of extensive SCC.

    Notes: Summary Antiglobulin autoantibodies have already been demonstrated in the sera of patients with primary Sjögren's syndrome (primary SS). In our study, the specificity of primary SS antiglobulins for different regions IgG molecules was examined by employing both direct binding and competitive inhibition enzymelinked immunosorbent assay. We found that a considerable amount of total antiglobulins in primary SS was specific for the Fab portion, although the remainder was specific for the Fc portion, namely rheumatoid factor (RF). In contrast, most of the antiglobulins in RA were specific for the Fc portion of IgG. These results indicate that in primary SS, antiglobulins directed against epitopes different from those of RF are produced. These antiglobulins may prove to have a different role in primary SS than that ascribed to RF in RA.

    Notes: Summary We describe a 64-year-old female patient with ochronosis and rheumatoid arthritis. Magnetic resonance imaging of the spinal column disclosed the destruction of vertebral disks, and a bony bridging in Th12 to L2. In addition, we observed joint space narrowing in the wrists as well as among the carpal bones, positive rheumatoid factor and the presence of rheumatoid nodules, in which the histological findings were compatible with those of rheumatoid arthritis. The coexistence of these two diseases has not yet been previously reported. Pre-existing ochronotic arthropathy might have masked the manifestation of rheumatoid arthritis and made the diagnosis of rheumatoid arthritis rather difficult.
