Summary This article takes account of the comprehensive study of the “Operation findings in the central nervous system in basilar impressions and related congenital abnormalities of the atlanto-occipital region” by Driesen (1960) and compares the end—results obtained from a through follow—up in 29 patients treated by suboccipital decompression and upper cervical laminectomy with the preoperative findings. First of all the radiological methods of recognising the dysplasia are referred to again. The clinical symptomatology receives special attention, and the commonest symptoms and signs together with the initial and early symptoms and the leading objective signs are set out. The operative findings are listed in tabular form. The events in the history show that operative management of basilar impression and associated abnormalities is justified. An unfavourable result is to be expected if irreversible damage of the nervous tissue has already developed with clinical deficits (especially those of the central control of circulation and respiration) arising from severe compression of the medulla oblongata.

Résumé L'étude actuelle se rattache au rapport présenté parDriesen, en 1960, intitulé: «Les constatations opératoires au niveau du système nerveux central, lors d'impressions basilaires et de malformations apparentées de la région occipito-atloÏdienne» et décrit les résultats de l'examen approfondi de 29 malades ayant subi une décompression sous-occipitale et une laminectomie cervicale haute, examen comparé à ceux qui avaient été pratiqués avant l'opération. En guise d'introduction, sont rappelées les méthodes radiologiques permettant de reconnaitre ces dysplasies. La Symptomatologie clinique fait l'objet d'une particulière attention; sont décrits en effet, les groupes symptomatiques objectifs et subjectifs les plus fréquents, de mÊme que les premiers signes, les signes précoces et enfin les signes objectifs pathognomoniques. Les constatations opératoires sont présentées sous forme de tableau. L'étude de l'évolution catamnestique montre donc que l'indication opératoire dans les impressions basilaires et les malformations du mÊme ordre est justifiée. Il faudra s'attendre à un résultat défavorable lorsqu'il existe des lésions irréversibles de la substance nerveuse avec des troubles cliniques manifestes, en particulier de la régulation centrale de la circulation et de la respiration, ceci en relation avec une compression importante du bulbe rachidien.

Zusammenfassung Die vorliegende Abhandlung nimmt Bezug auf die umfassende Studie der „Operationsbefunde am zentralen Nervensystem bei basilaren Impressionen und verwandten Mißbildungen der atlanto-occipitalen Region“ vonDriesen (1960) und stellt die Ergebnisse einer eingehenden Nachuntersuchung von 29 durch suboccipitale Dekompression und obere Laminektomie behandelten Patienten den praeoperativen Befunden gegenüber. Einleitend wird nochmals auf die röntgenologischen Methoden zur Erkennung der Dysplasien hingewiesen. Die klinische Symptomatologie findet besondere Beachtung, wobei die häufigste subjektive und objektive Symptomgruppe sowie die subjektiven Erst- und Frühsymptome und die objektiven Leitsymptome herausgestellt werden. Die bei der Operation erhobenen Befunde sind tabellarisch aufgezeichnet worden. Die katamnestischen Erhebungen zeigen, daß das operative Vorgehen bei basilaren Impressionen und verwandten Mißbildungen berechtigt ist. Ein ungünstiges Resultat wird erzielt bei irreversiblen Schädigungen der nervösen Substanz mit klinisch manifesten Störungen, besonders der zentralen Kreislauf- und Atemregulation durch hochgradige Kompression der Medulla oblongata.
