Zusammenfassung Die Koronarrevaskularisation ohne extrakorporale Zirkulation gewinnt zunehmend an Interesse, um die Probleme, die mit der extrakorporalen Zirkulation verbunden sind, zu vermeiden. Hierfür wurde die sogenannte Octopus-Methode entwickelt. Von August 1997 bis Oktober 1998 wurden bei uns 60 Patienten (47 Männer und 13 Frauen) nach dieser Methode operiert. 1 Patient hatte eine 1-Gefäßerkrankung, bei 39 Patienten lag eine 2-Gefäßerkrankung und bei 20 Patienten eine 3-Gefäßerkrankung vor. Alle Patienten hatten eine normale Ejektionsfraktion. Die Operation wurde über eine mediane Sternotomie durchgeführt. Insgesamt wurden 139 Mammaria-Anastomosen, davon 27 als sequentielle Anastomosen, 5 Anastomosen mit der Arteria radialis und 10 Anastomosen mit der Vena saphena magna durchgeführt. 2 Patienten erhielten eine, 26 Patienten zwei, 48 Patienten drei und 4 Patienten vier periphere Anastomosen. Dies entspricht einem Graft/Patient-Verhältnis von 2,5. Postoperativ verstarben 2 Patienten an einem Low-output-Syndrom, die übrigen überlebten die Operation und sind postoperativ beschwerdefrei. 22 der 60 Patienten wurden noch während des stationären Aufenthaltes nachangiographiert. Hierbei waren alle Anastomosen offen und ohne Beeinträchtigung. Nach unseren Erfahrungen ist das Octopus-Verfahren eine sichere Methode, eine ausreichende Stabilität der Herzoberfläche, auch im Seiten- und Hinterwandbereich des Herzens zu erreichen, so daß auch Mehrfach-Revaskularisationen möglich sind.

Summary There is increasing interest in coronary revascularization without using extracorporeal circulation to avoid the problems associated with extracorporeal bypass. The Octopus system has been designed for this approach. From August 1997 to October 1998 we operated on 60 patients (47 men and 13 women) using the Octopus system. 1 patient had single vessel, 39 patients double vessel and 20 patients triple vessel disease. All patients had a normal ejection fraction. The operation was performed through a median sternotomy. A total of 139 mammary anastomoses, of which 27 were sequential anastomoses, 5 anastomoses with the radial artery and 10 anastomoses with the segment of the vena saphena magna were performed. 2 patients received one, 26 patients two, 48 patients three and 4 patients four peripheral anastomoses. The graft/patient relationship was 2.5. Postoperatively 2 patients died of low cardiac output; the reminder survived the operation without postoperative symptoms. 22 of the 60 patients had reangiography within the first week and showed all anastomoses widely patent and not compromised. In our experience the Octopus method is a safe procedure which gives sufficient stability to the surface of the heart even in the lateral and posterior aspect, so that multiple bypass grafts are possible.
