    Notizen: Abstract: Previously, we reported a modest but significant reduction in the concentration of neuropeptide Y in frontal cortices from victims of suicide relative to age-matched natural or accidental death control subjects. The reduction in neuropeptide Y appeared to be greatest in a subgroup of victims of suicide for which there was indirect evidence of histories of depression. We pursued these initial findings in the present study by measuring neuropeptide Y concentrations in frontal cortices from natural or accidental death control subjects and from suicide victims in whom a firm diagnosis of major depression was established by psychiatric autopsy. Because several subjects with major depression had a comorbid diagnosis of alcoholism, a group of victims of suicide that had an Axis I diagnosis of alcohol dependence was also studied. No significant differences in neuropeptide Y concentrations were observed between control subjects and victims of suicide with major depression or victims of suicide with alcohol dependence. These findings do not support a role for neuropeptide Y in major depression.

    Notizen: Abstract: Comparisons of the activity of the G protein-mediated phosphoinositide signal transduction system and of G protein levels were made in two regions of frontal cortex from eight schizophrenic, alcohol-dependent, and control subjects. G protein-mediated phosphoinositide hydrolysis was measured by stimulating cortical membranes incubated with [3H]phosphatidylinositol with 0.3–10 µM guanosine 5′-O-(3-thio)triphosphate (GTPγS). In frontal cortex areas 8/9, GTPγS-induced phosphoinositide hydrolysis was 50% greater in schizophrenic than control or alcohol-dependent subjects, whereas there were no differences among these groups of subjects in the response to GTPγS in frontal cortex area 10. Agonists for dopaminergic, cholinergic, purinergic, serotonergic, histaminergic, and glutamatergic receptors coupled to the phosphoinositide signaling system increased [3H]phosphatidylinositol hydrolysis in a GTPγS-dependent manner. Responses to most agonists were similar in all three subject groups in both cortical regions, with the largest difference being a 40% greater response to dopaminergic receptor stimulation in frontal cortex 8/9 from schizophrenic subjects. Measurements of the levels of phospholipase C-β, and of α-subunits of Gq, Go, Gi1, Gi2, and Gs, made by immunoblot analyses revealed no differences among the groups of subjects except for increased Gαo in schizophrenic subjects and increased Gαo and Gαi1 in alcohol-dependent subjects. These results demonstrate that schizophrenia is associated with increased activity of the phosphoinositide signal transduction system and increased levels of Gαo, whereas the phosphoinositide system was unaltered in alcohol dependence, but Gαo and Gαi1 were increased.

    Notizen: Abstract Passive and dependent qualities are commonly found in psychiatric patients suffering from a variety of disorders. While such tendencies are often secondary to an Axis I clinical syndrome, they are occasionally more persistent and refractory. The present article attempts to provide therapeutic guidelines useful when working with clients displaying stable personality traits of passivity and dependency. An attempt is made to adapt and combine several therapeutic techniques to be applicable to passive-dependent clients. More specifically, problem-solving approaches and self-control strategies can be used to facilitate autonomy if precautions are taken to reduce the dogmatic and didactic styles commonly employed with these techniques. The Socratic method of interviewing is posited as a useful means of developing problem-solving skills without creating unnecessary dependence on the therapist. Self-control strategies can help reduce the reliance on others as the primary source of satisfaction for dependent clients. In this way, passive-dependent clients may become more self- reliant and autonomous in their daily functioning.

    Notizen: Abstract The treatment of compulsive urges can be facilitated through behavioral approaches using exposure with response prevention. However, many clients benefit from preliminary cognitive therapy designed to improve their ability to cope with difficult situations. The present manuscript describes the cognitive-behavioral treatment of a client with compulsive repeating rituals. Coping self-statements were used to provide the client with cognitive tools to confront his urges and control his anxiety and behavior. Also, several therapeutic analogies were created to help shift the client's views of his urges and learn how to respond to them differently. A variety of detailed examples from his treatment are used to clarify the combined use of cognitive and behavioral therapy when working with clients who have obsessive-compulsive disorder.

    Notizen: Abstract Cognitive therapy has become a well established and widely used treatment for depression. However, most treatment guidelines describe a session-by-session focus that may not adequately conform with the individualized nature of individual psychotherapy used in most clinical settings. The present manuscript describes strategies for helping clients to reduce their cognitive biases by: (1) identifying their cognitive biases, (2) appreciating the relationship between cognitions, emotions, and behavior, (3) distancing themselves from their perspective, (4) developing realistic positive self-statements, (5) altering maladaptive thought processes, (6) shortening the temporal delay required to challenge their thoughts, and (7) learning to find positive elements in predominantly negative situations. These strategies for reducing cognitive biases can be incorporated into a comprehensive treatment plan that includes modules for the assessment of depression severity and suicide risk, enhancing social performance, and improving problem-solving skills. The modules are meant to be used in a flexible manner, allowing the therapist to address a variety of treatment goals while still responding to the unique needs of each client.

    Notizen: Abstract The adverse effects of maternal alcohol consumption have been noted throughout history. When not resulting in the death of the fetus, Fetal Alcohol Syndrome (FAS) is characterized by growth deficiencies, facial abnormalities, and central nervous system dysfunctions. Both direct and indirect teratogenic effects of alcohol are discussed. Also, the type and severity of alcohol's impact on the fetus are viewed in relation to critical periods during the gestational process. Finally, the severity of maternal alcoholism is related to the severity of damage suffered by the fetus. FAS is seen as the consequence of persistent toxic levels of alcohol throughout the gestational period. Several preventative measures are briefly discussed.

    Notizen: Abstract Cognitive-behavioral approaches have been found effective in the treatment of depression. In order to maximize the efficiency and flexibility of cognitive-behavioral treatment, the therapist can use of a series of interrelated therapy modules focused on different aspects of treatment. The present manuscript describes the module for improving social functioning in depressed clients. Suggestions are provided for guiding therapy to help clients increase their social activity, reduce the interpersonal conflict in their lives, and learn to adjust to loss and change resulting from stressful life events. Therapy of depression may be facilitated when the present module is used in combination with other modules focused on the assessment of depression severity and suicide risk (see Part I in this Journal), reducing cognitive biases in depressed clients, and enhancing problem-solving skills in clients.

    Notizen: Abstract Cognitive-behavioral treatment of depression has been found effective for helping a wide range of clients. However, current approaches to treatment emphasize the integration of a variety of treatment elements. The present manuscript describes the use of problem-solving therapy as incorporated into a broader cognitive-behavioral treatment model. Problem-solving approaches to therapy typically involve helping clients develop skill in five interrelated areas: (1) Develop an adaptive orientation to life's problems, (2) Define problems in terms of specific and realistic goals, (3) Generate a variety of response alternatives, (4) Help clients make rational decisions, and (5) Implement the plan and evaluate its effectiveness. These strategies can help many clients cope with the stressful life events that occur, and therefore can help reduce the intensity or prevent the onset of depressive symptoms. Furthermore, these strategies can be incorporated into a broader treatment plan that includes elements for monitoring depression severity, improving social functioning, and reducing cognitive biases.
