    Notes: Background: Ranitidine bismuth citrate (GR122311X) is a new drug which offers potential benefits in healing duodenal ulcers and prevention of relapse. Methods: This randomized, multi-centre double-blind study of 1620 patients compared the effect of 4 weeks of treatment with GR122311X 200 mg b.d. (n= 401), 400 mg b.d. (n= 404) or 800 mg b.d. (n= 404) or ranitidine hydrochloride 150 mg b.d. (n= 411) on the rates of duodenal ulcer healing and of overall success (ulcers healed and remaining ulcer free in the 24-week follow-up phase). Results: All four treatments were equally effective at ulcer healing (79%, 85%, 84% and 81% of patients, respectively). GR122311X 400 mg b.d. (38%) and 800 mg b.d. (37%) were significantly more effective than ranitidine hydrochloride 150 mg b.d. (32%) with respect to overall success (P = 0.050 and P = 0.030, respectively) but there was no difference with GR122311X 200 mg b.d. (31%). GR122311X caused effective, dose-related suppression of H. pylori (47%, 61% and 74%); H. pylori eradication rates were 18%, 21% and 22%. GR122311X was safe and well tolerated, with an adverse event profile similar to that of ranitidine hydrochloride 150 mg b.d. Median week 4 trough plasma bismuth levels were 1.3 ng/mL, 2.3 ng/mL and 3.3 ng/mL with GR122311X 200 mg b.d., 400 mg b.d. and 800 mg b.d. respectively. No individual plasma bismuth concentrations were of clinical concern. Conclusions: GR122311X is a safe and effective ulcer healing drug, and provides a platform on which anti-H. pylori therapy can be based.

    Notes: : Both triple therapy with ranitidine bismuth citrate (RBC) plus two antibiotics for 7 days and dual therapy of RBC with clarithromycin for 14 days have been extensively studied; both regimens effectively eradicate Helicobacter pylori. However, few studies have assessed the efficacy of dual therapy given for 7 days.〈section xml:id="abs1-2"〉〈title type="main"〉Aim: To compare the efficacy and safety of RBC 400 mg with clarithromycin 500 mg, alone or with metronidazole 400 mg, given twice daily for 7 days for the eradication of H. pylori.〈section xml:id="abs1-3"〉〈title type="main"〉Methods: This single centre, randomized, double-blind study involved 118 patients with dyspepsia or a history of peptic ulcer disease. H. pylori infection was detected initially by CLO test, and confirmed in 109 patients by urea breath test and/or microbiology culture. H. pylori eradication was assessed 4 and 12 weeks after the end of treatment by urea breath test. H. pylori antibiotic susceptibility was assessed pre-study in all patients, and post-treatment in patients with a positive post-treatment urea breath test. Adverse events were recorded throughout the study.〈section xml:id="abs1-4"〉〈title type="main"〉Results: H. pylori was eradicated in 93% of patients who received RBC with clarithromycin and metronidazole and in 84% of patients who received RBC with clarithromycin (intention-to-treat rates). Per protocol eradication rates were 98% and 90% for triple therapy and dual therapy, respectively. The eradication of metronidazole-resistant H. pylori was achieved in 100% and 88% of patients following dual therapy and triple therapy, respectively, and acquired resistance to clarithromycin occurred in only one patient following treatment failure. Both treatments were well-tolerated; only one patient (2%) was withdrawn from each treatment group due to adverse events.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusions: RBC with clarithromycin and metronidazole is a highly effective and well-tolerated triple therapy regimen for the eradication of H. pylori. RBC with clarithromycin dual therapy has a similar efficacy, and offers an alternative to triple therapy when there are concerns about treatment with metronidazole or the use of multiple antibiotics. Both regimens are effective against antibiotic-resistant strains of H. pylori.

    Notes: Abstract The modelling of liquid-solid phase change phenomena is extremely important in many areas of science and engineering. In particular, the solidification of molten metals during various casting methods in the foundry, provides a source of important practical problems which may be resolved economically with the aid of computational models of the heat transfer processes involved. Experimental design analysis is often prohibitively expensive, and the geometries and complex boundary conditions encountered preclude any analytical solutions to the problems posed. Thus the motivation for numerical simulation and computer aided design (CAD) systems is clear, and several mathematical/computational modelling techniques have been brought to bear in this area during recent years. This paper reports on the application of the finite element method to solidification problems, principally concerning industrial casting processes. Although convective heat transfer has been modelled, the work herein considers only heat conduction, for clarity. The heat transfer model has also been coupled with thermal stress analysis packages to predict mechanical behaviour including cracking and eventual failure, but this is reported elsewhere. Following the introduction, the mathematical and computational modelling tools are described in detail, for completeness. A discussion on the handling of the phase change interface and latent heat effects is then presented. Some aspects of the solution procedures are examined next, together with special techniques for dealing with the mold-metal interface. Finally, some numerical examples are presented which substantiate the capabilities of the finite element model, in both two and three dimensions.

    Notes: Abstract The secretion and metabolism of endogeneous neurotensin-like immunoreactivities after a test meal were studied in five healthy human subjects. Intact neurotensin and the N-terminal metabolic fragment, neurotensin 1–8, were quantified by radioimmunoassay with C- and N-terminally directed antisera in conjunction with gel filtration of plasma samples obtained at timed intervals. Both C- and N-terminal neurotensin-like immunoreactivities rose after the meal, reaching a plateau level after 20 and 30 min, respectively. During the plateau phase, which lasted for the rest of the experimental period of 180 min, the molar ratio of intact neurotensin to neurotensin 1–8 remained approximately constant at 1∶4.6. Meal-stimulated immunoreactive neurotensin appeared to be metabolized in a manner comparable to that of exogenously infused neurotensin in man. The results suggest that intact neurotensin is secreted at an approximately constant rate during the plateau phase. The relatively low plateau level of neurotensin 1–8, which has a much longer half-life than intact neurotensin in the circulation, implies that only a fraction of the secreted intact neurotensin is metabolized to neurotensin 1–8, indicating the existence of alternative pathways of neurotensin metabolism.
