    Notes: Summary Studies have shown that a high plasma non-esterified fatty acid concentration may inhibit glucose induced insulin secretion in vitro and in vivo. The effect of lowering the fatty acid concentration on the acute insulin response was investigated in first degree relatives of people with Type II diabetes in a double-blind, randomised, placebo-controlled trial. Fifty first degree relatives of people with Type II diabetes volunteered for the study. Twenty five were given acipimox (250 mg/day, four times daily) and 25 placebo. The group treated with acipimox had a lower 2-h plasma glucose concentration (6.1 ± 0.2 vs 7.7 ± 0.3 vs mmol/l, p 〈 0.01); better insulin-mediated glucose uptake (35.4 ± 0.5 vs 28.3 ± 0.4 μmol/kg fat free mass per min, p 〈 0.01), acute insulin response (68 ± 4.4 vs 46 ± 7.3 mU/l, p 〈 0.01) and respiratory quotient (0.81 ± 0.02 vs 0.77 ± 0.03, p 〈 0.05); and a rise in the plasma glucagon (164 ± 63 vs 134 ± 72 ng/l, p 〈 0.05), growth hormone (1.31 ± 0.13 vs 0.97 ± 0.21 μg/l, p 〈 0.03) and cortisol (325 ± 41 vs 284 ± 139 nmol/l, p 〈 0.05) concentrations. The difference in the acute insulin response persisted, even after adjustment for the 2-h plasma glucose concentration, insulin-mediated glucose uptake, the fasting plasma glucagon concentration and the growth hormone concentration (p 〈 0.05). In a subgroup of eight patients acipimox was compared with acipimox plus intralipid. The acute insulin response (44 ± 5.1 vs 71 ± 5.3 mU/l, p 〈 0.01) and the insulin-mediated glucose uptake (27.4 ± 0.4 vs 36.7 ± 0.5 μmol/kg fat free mass per min, p 〈 0.003) were lower with acipimox plus intralipid treatment than with acipimox alone. It is concluded that long term acipimox treatment lowers the plasma fasting free fatty acid concentration and improves the acute insulin response and the insulin mediated glucose uptake.     Notes: Summary Within 5 hours of the onset of ischaemic stroke the filterability patterns and the counts of the granulocyte, monocyte and lymphocyte subfractions were monitored. The results were compared to those of a healthy matched control group and to those of a matched group with cardiovascular risk factors. The granulocyte filterability rate was significantly impaired in the group at risk and even more so in the stroke group suggesting alterations in it may be a sign of latent ischaemia.

    Notes: Abstract We evaluated whether hepatic abnormalities represent a specific feature in girls with Turner syndrome (TS) or whether they are related to an increased susceptibility to hormonal therapies and/or other factors. Alanine aminotransferase, aspartate aminotransferase and γ-glutamyl transferase were monitored in 70 patients with TS for a mean period of 7.6 ± 4.2 years. An increase in serum liver enzymes was observed in 14 out of 70 girls (20%) at a mean age of 12.7 years; it was present at entry before hormonal therapy in 3 girls and developed thereafter during the follow up in the other 11. The increase in serum liver enzymes was never observed before the age of 7 years. In the majority of cases (10/14) it was drug related: in 50% the liver abnormalities were transient and self-limiting, in the remaining cases they required interruption of hormonal therapy. Hepatotoxicity was more frequently observed in girls treated with oestrogens or oxandrolone than in those treated with growth hormone. In a small number of cases, liver disease was either auto-immunity-related (2/14), or cryptogenic (1/14) with a benign and self-limiting course. Obesity was a frequent finding, but it played a likely pivotal role only in one patient. Conclusion Hepatic abnormalities are relatively frequent in Turner syndrome and surveillance of liver function should be included in the management of these patients independent of initiation of hormonal treatment.

    Notes: The recent achievements in electronic control as well as the higher performance of advanced permanent magnets have brought about a change in the general strategy for design and development of new types of small electrical machines. A special application of permanent magnets (PM) is represented by hybrid stepping motors (HSM). The latter are a sort of combination of variable reluctance-motors and permanent magnet-motors. The influence of various PM inserts into the magnetic circuits of electromechanical converters on field and force distribution has been already investigated by the authors (1996). Promising results for HSM have been reported, especially when rare-earth PMs are used. The main aim of PM inserting, when they are put into slots of slotted pole shoes with radial N-S polarization, is to focus magnetic field lines in the corresponding teeth. As a result, the reluctance stepping torque can be increased depending on the material used. Now a further step is made by extending the analysis from the case of linear to that of non-linear demagnetization curves of permanent magnets.

    Notes: A group of 20 mite allergic asthmatic children aged 6-12 years old. living in Sao Paulo, Brazil, was studied regarding their degree of sensitization to house dust mites and exposure to mite allergens in their homes. In 18 out of 20 houses at least one dust sample was obtained which contained 〉 10 μg Der p I/g of dust. The highest levels of Dermatophagoides pteronyssinm allergens, Der p I and Group II, were measured in bedding samples (geometric mean 38.4 and 36.6 μg/g, respectively), followed by bedroom floor, TV room and kitchen. Mite allergen levels in Brazilian houses were as high as those reported to be associated with sensitization and acute attacks of asthma in other parts of the world. In keeping with previous reports that D. farinae is rarely found in Brazil, Der fl was undetectable or found in very low levels (〈0.5 μg/g). Levels of cat allergen Pel d I of 〉 8 μg/g of dust were obtained only in 2 houses only. Cockroach allergen Bia g I was detected in five out of 20 houses. Levels of IgE antibodies to D. pteronyssinus were 〉 200 RAST U/ml in 19 out of 20 children (geometric mean 1588 RAST U/ml). IgE antibodies to cat, cockroach, A. fumigatus, ragweed and rye grass pollens were undetectable or 〈80 RAST U/ml. IgE antibodies to the mite Blomia tropicalix were also measured, and levels 〉200 RAST U/ml were observed in 13 out of 20 sera. Immunoabsorption studies demonstrated that the bulk of the IgE- antibody to B. tropicafis (64%) was to species-specific allergens and that 36% were cross-reactive with D. pteronyssinus. The implication of our results is that management of children with asthma in São Paulo should include skin testing for allergy to both Dermatophagoides and B. tropicalis as well as recommendations about environmental control of house dust mite exposure.

    Notes: Endotoxins (ET) are pro-innammatory substances present in hou.sc dust which may increase non-specific bronchial reactivity in asthmatic patients. Endotoxins (EU/g) and Der p I levels were compared in the homes often asthmatic and ten control children, aged 6-16 years, living in Sao Paulo, Brazil. The houses were visited once a month from February 1993 lo February 1994 and dust samples were collected from the bedding and floor of each subject's house. No significant differences were observed in ET and Derp 1 levels in the homes of asthmatics and controls. The highest ET levels were detected in January and November, whereas the lowest levels were detected in April and August (p 〈 0.05). demonstrating a distinct seasonal distribution. The highest Der p I levels in bedding were observed in July and the lowest in February (p 〈 0.05). while Derp I levels in floor did not show significant differences throughout the year. Symptom and medication scores were evaluated monthly in the group of asthmatic children. There was a significant correlation (p 〈 0.05. r = 0.63) between clinical symptom scores and ET exposure, however no significant correlation was found for mite exposure (p 〉 0.05. r = 0.19). The results suggest that ET exposure exacerbates asthmatic symptoms in mite allergic, asthmatic children.

The possibility of using eels from intensive rearing ponds to restock natural basins has been investigated by comparing growth, survival and rate of yellow-to-silver metamorphosis of ‘wild type’ eels and of eels from an intensive rearing pond. A total of 2243 marked eels were released into a closed natural environment of 22ha; 1254 were wild type (average length 45·6 ± 9·0cm), and 989 from an intensive rearing pond, split by a mechanical grader in two groups. The first group was made of 617 well-growing eels (44·9 ± 3·0cm), the second one of 372 smaller eels (36·9 ± 4·3cm) which had not adapted to the artificial conditions.The starting size being equal the animals grew to the same extent; in some cases the animals previously adapted to the artificial pond grew better than the wild type. The rate of metamorphosis from yellow to silver eel was 28·4% for the wild type eels and 22% for the others, although the latter became mostly silver males. Survival of the wild type eels (starting with the 35-cm size class and covering the period from April to December) was calculated by mark-recapture at approximately 98·6%. For the eels previously well adapted to the intensive rearing pond it was only 85%, indicating some difficulty in competing in the natural environment. The good growth shown by the smaller eels, with a survival of about 90%, would seem to indicate that these animals might be effectively used for restocking purposes, thus favouring an integration between the intensive ponds and the extensive environments.
