    Notes: Summary [Ne-palmitoyl Lys (B29)] human insulin is a fatty acid-acylated derivative of insulin with extended action compared to unmodified insulin when infused intravenously (i. v.) secondary to its binding to circulating albumin. The duration and activity profile of the acylated (A) and NPH (B) insulins were assessed following subcutaneous (s. c.) doses of (A) 6 nmol/kg and (B) 1.2 nmol/kg (equivalent to 0.2 U/kg) in 9 subjects with IDDM. After overnight i.v infusion of regular human insulin, morning glucose was (A) 6.9 ± 0.1 and (B) 6.8 ± 0.1 mmol/l. After the s. c. injection, i. v. human insulin or glucose was infused to maintain near-basal glycaemia and tracer glucose to assess hepatic glucose production (HGP). An activity profile was deduced for each study by expressing the glucose infusion rate at each time point, as a fraction (%) of the basal (measured) HGP, and the i. v. insulin infusion rate as a fraction (%) of the basal requirement. The two fractions are combined by adding the fractional glucose infusion rate and subtracting the fractional insulin infusion rate. Infusion rates of i. v. insulin in the morning were (A) 0.96 ± 0.096 and (B) 1.22 ± 0.09 pmol · kg–1· min–1. After insulin injection, i.v insulin requirements decreased and were below 10 % of basal between 100 and 150 min. A constant activity profile of 0 % represents a perfect substitution of the basal i. v. insulin infusion by the s. c. dose. The actual profile is defined by deviations from this (above) and was –17 ± 11, 7 ± 10, –9 ± 6 and –18 ± 18 % for [Ne-palmitoyl Lys (B29)] human insulin and 17 ± 12, 5 ± 6, –9 ± 15, 22 ± 18 % for NPH insulin at 3, 6, 9 and 12 h after s. c. injection. HGP was similar for the two insulins, demonstrating similar metabolic actions and profiles both peripherally and at the liver. [Diabetologia (1998) 41: 116–120]

    Notes: Abstract  The Keno Hill mining district in central Yukon was the second largest silver producer in Canada with mines operating from 1913 to 1989 on more than 65 vein silver deposits. The seven and a half decades of mining activities have generated large volumes of mine waste disposed on the land surface, resulting in elevated metal contents in numerous small drainages. To assess the extent of metal mobilization, old mine workings and the associated mine waste were examined and the water courses draining to a major river valley sampled. The results of field observations and an array of water and sediment analyses led to three major conclusions. 1. Acid mine drainage is not widespread because of galvanic protection of pyrite from oxidative dissolution and neutralization by carbonates in the country rock. 2. Mechanisms operative to limit aqueous metal transport in small streams in the district include cryogenic precipitation, coprecipitation and sorption. 3. The near-surface concentration of metals limits the options of waste disposal in future mining developments due to potential metal-leaching problems.

    Notes: This paper is concerned with a phenomenon that has in previous descriptions been labelled ‘glottal reinforcement’, ‘pre-glottalization’ or ‘glottalization’ (for discussion of ‘glottal replacement’ and the distinction between ‘full’ and ‘pre-’ glottalization see below, 10.1). The essential characteristic of this glottalization is that the oral closure for /p/, /t/ or /k/ is preceded by a glottal closure, but there is much variation from one accent to another in terms both of the distribution of the glottal closure and of its articulatory characteristics (especially the duration of the closure). Of the literature on the subject, a speculative article by Christophersen (1952) followed by a reply from O'Connor (1952), a study by Andrésen eventually published as a book (1968) and a study by Higginbottom (1964) are the best known. I shall use the term ‘glottalization’ in this paper: being a more general label it does not commit one to the possibly misleading implications of the other names.

    Notes: Abstract Infantile renal dysplasias, including multicystic dysplastic kidneys (MCDK), are reported rarely to accumulate radiopharmaceuticals on renal scintigraphy.99mTc DMSA is a highly sensitive tracer for detecting functioning renal cortical tissue and may be more suited to studying renal dysplasia than99mTc DTPA. We reviewed the ultrasound studies and99mTc DMSA scintigrams of 42 infants (age range 1–12 months) with known or suspected MCDK. Overall, uptake on99mTc DMSA scintigraphy was evident in 6/41 (15%) dysplastic kidneys. Of the 18 patients who underwent nephrectomy, histopathological examination revealed that uptake correlated closely with the presence of mature renal cortical tissue in the affected kidney. Our study shows that a small, but significant number of MCDK will show low-grade uptake on DMSA scintigraphy. This finding may be relevant given the reliance placed on renal scintigraphy in planning treatment for infants with suspected MCDK, particularly with the increasing trend for the non-operative management of this condition.

    Notes: Abstract A characteristic scintigraphic pattern of focally intense skeletal tracer uptake surrounded by less prominently increased uptake has been described for osteoid osteomas. Demonstration of this finding is useful in the evaluation of patients with suspected osteomas. Pinhole magnification scintigraphy was significantly more effective than high-resolution planar scintigraphy at revealing this pattern in 19 osteoid osteomas of the appendicular skeleton. A similar advantage was not observed for vertebral osteoid osteomas. The absence of the characteristic pattern aided in differentiating other pathology from osteoid osteomas in seven patients. The authors recommend that skeletal scintigraphy of patients with suspected osteoid osteomas include pinhole magnification imaging.

    Notes: Abstract The development of an audiovisual pronunciation teaching and training method and software system is discussed in this article. The method is designed to help children with speech and hearing disorders gain better control over their speech production. The teaching method is drawn up for progression from individual sound preparation to practice of sounds in sentences for four languages: English, Swedish, Slovenian, and Hungarian. The system is a general language-independent measuring tool and database editor. This database editor makes it possible to construct modules for all participant languages and for different sound groups. Two modules are under development for the system in all languages: one for teaching and training vowels to hearing-impaired children and the other for correction of misarticulated fricative sounds. In the article we present the measuring methods, the used distance score calculations of the visualized speech spectra, and problems in the evaluation of the new multimedia tool.
