    Description / Table of Contents: Zusammenfassung Zur Überprüfung der inkretorischen Hodenfunktion wird ein Leydig-Zell-Funktionstest vorgeschlagen, der auf der Bestimmung von Plasmatestosteron vor und nach Stimulation mit HCG beruht. Auf die Notwendigkeit der Ermittlung des repräsentativen Tagesmittels von Plasmatestosteron wird hingewiesen. Venenblut wird am 1. und 4. Tag des Testes um 4.00, 12.00 und 20.00 Uhr abgenommen, und 5000 IE HCG werden am 1., 2. und 3. Tag jeweils um 20.00 Uhr intramuskulär gegeben. Plasmatestosteron wird mit Hilfe der kompetitiven Proteinbindung bestimmt. Zur Beurteilung der Leydig-Zell-Funktion dienen das Plasmatestosteron vor Stimulation und der Stimulationskoeffizient, der sich aus dem Verhältnis des Plasmatestosterons nach Stimulation zum Plasmatestosteron vor Stimulation errechnet und graphisch dargestellt wird. Bei normalen Männern im Alter von 16–45 Jahren besteht eine linearc Korrelation zwischen dem Plasmatestosteron vor und nach Stimulation (Korrelationskoeffizient=0,94). Ihr Stimulationskoeffizient beträgt 2,5±0,2. Es findet sich keine lineare Korrelation zwischen Alter und Plasmatestosteron oder Stimulationskoeffizienten. Impotente Männer mit unauffälligem klinischen Befund haben bei normalen Plasmatestosteron-Ausgangswerten eine verminderte Stimulierbarkeit. Ähnliche Ergebnisse finden sich bei Diabetes mellitus und Aortenbifurkationssyndrom. Patienten mit chromophobem Adenom der Hypophyse dagegen haben bei niedrigen Ausgangswerten eine starke Stimulierbarkeit. Patienten mit echtem Klinefelter-Syndrom, der 2X-2Y-Form des Syndroms, einem Pseudohermaphroditismus masculinus oder einer testikulären Feminisierung und eine 46,XX-Mann zeigen unter der Norm liegende Ausgangswerte; sie lassen sich jedoch alle stimulieren. Ferner werden Ergebnisse der Anwendung des Testes bei 3 Patienten mit Leydig-Zell-Tumoren vor und nach Semikastration mitgeteilt.
    Notes: Summary A Leydig cell function test based on the determination of plasma testosterone before and after stimulation by HCG is discussed. The need of determining the representative daily mean of plasma testosterone is pointed out. Blood is drawn on the 1st and 4th day of the tests at 4 AM, noon and 8 PM and 5000 I.U. HCG are injected i.m. on the 1st, 2nd and 3rd day at 8 PM. Plasma testosterone levels are assessed by competitive protein binding. Testicular function is evaluated by means of the basic plasma testosterone values and by the “stimulation coefficient”, calculated from the ratio of plasma testosterone after stimulation vs. plasma testosterone before stimulation. — In normal men aged 16 to 45 years, a linear correlation exists between plasma testosterone before and after stimulation (correlation coefficient=0.94). In these cases, the stimulation coefficient corresponds to 2.5±0.2. No linear correlation between age and basic plasma testosterone values nor the stimulation coefficient can be demonstrated. — Impotent men with no clinical findings show normal plasma testosterone levels but decreased Leydig cell reserve. Similar results are observed in diabetes mellitus and severe aortic sclerosis. Patients with chromophobe pituitary adenoma, however, have low basic values but a high stimulation coefficient. Male patients with true Klinefelter's syndrome, the 2X–2Y form of the syndrome, a 46, XX chromosome constitution, male pseudohermaphroditism or testicular feminization show low basic plasma testosterone, but a clear increase after stimulation. Results from 3 patients with Leydig cell tumor are also presented. They were investigated before and during the 1st and 10th week after semicastration.
