Zusammenfassung In einer retrospektiven Studie wurden 149 Patienten mit anaplastischem kleinzelligem Bronchuscarcinom untersucht. 19 Patienten (13%) wiesen bereits initial eine obere Einflußstauung auf, 11 weitere Patienten (7%) entwickelten diese Komplikation im weiteren Verlauf ihrer Krankheit. 16 Patienten, die bereits zu Beginn ihrer Krankheit eine Obstruktion der oberen Hohlvene aufwiesen, wurden mit einer alleinigen Chemotherapie behandelt. 13 Patienten sprachen auf diese Behandlung mit einer teilweisen (7 Patienten) oder vollständigen (6 Patienten) Rückbildung der oberen Einflußstauung an. Wegen einer für heutige Begriffe suboptimalen Therapie entwickelten 7 der 13 Patienten (54%) im Mittel 4,5 Monate nach Therapiebeginn ein Rezidiv. 3 Patienten sprachen auf eine zweite Chemotherapie an, bei 4 Patienten war eine Radiotherapie erfolgreich. Die obere Einflußstauung ist eine häufige Komplikation des anaplastischen kleinzelligen Bronchuscarcinoms und kann mit alleiniger Chemotherapie erfolgreich behandelt werden.

Summary One hundred and forty nine consecutive patients with small-cell anaplastic carcinoma of the lung were studied retrospectively. 19 patients (13%) presented with superior vena caval obstruction, 11 further patients (7%) developed this complication during the course of their disease. 16 patients presenting with superior vena caval syndrome were treated with chemotherapy alone and 13 patients responded. Due to suboptimal chemotherapy for current standards 7 of the 13 successfully treated patients (54%) relapsed after a median of 4.5 months. 3 patients were effectively retreated with another chemotherapy combination, 4 patients with radiation therapy. Superior vena caval obstruction is a frequent complication of small-cell anaplastic carcinoma of the lung. It can be treated effectively with combination chemotherapy.
