    Notes: Summary Gastrointestinal motility disorders induced by diabetes mellitus are most commonly manifested in the colon, and are considered to be an expression of diabetic autonomic polyneuropathy. In this study, for the first time, the intrinsic nervous system of the bowel has been subjected to an ultrastructural and morphometric examination in rectal biopsy material obtained from diabetics with and without diabetic enteropathy. In patients with diabetic enteropathy, the nerves of the submucous plexus (Meissner's plexus) revealed significant swelling of the axons, as an expression of neural degeneration. Furthermore, in about a half of these patients, thickening of the basal membrane of Schwann's cells was also detected. Additional, although unspecific, changes found in the diabetics were a quantitative increase in the lysosomes, lipofuscin and glycogen in the Schwann cells. The ultrastructural findings represent evidence for the presence of an intrinsic autonomic polyneuropathy in diabetics with diabetic enteropathy. At the same time, they indicate that the diagnosis of diabetic enteropathy is no longer merely an “exclusion” diagnosis.

    Notes: Abstract Between April 1987 and May 1990 a total of 212 consecutive patients with tumours in the head and neck region were admitted to a prospective study comparing planned prospective enteral nutrition via percutaneous endoscopically guided gastrostomy (PEG; n=47) and oral nutrition (n=134). The nutritional status (anthropometric and laboratory chemical parameters) and the quality-of-life index according to Padilla et al. [Res Nurs Health 6: 117–126 (1983)] were determined prior to radiotherapy, 2, 4, 6 weeks later during radiotherapy and 6, 12 and 18 weeks after completion of radiotherapy. The quality-of-life score of the orally nourished patients decreased quickly during radiotherapy and improved only slowly afterwards. Although PEG patients had a worse starting score, their quality-of-life index did not deteriorate during therapy (statistically significant difference between the two groups). The same applies to the nutritional status. These results show that an early and constant enteral nutrition by PEG can stabilize the nutritional state and the quality of life of patients with tumours of the head and neck area during radiotherapy.

    Description / Table of Contents: Summary A twenty-four percent solution of carbohydrates 0.2 g/kg/h consisting of fructose, xylit and glucose in a 2/3n1/3n1 ratio and an aminoacid solution 0.03 g/kg/h were given to 13 patients of the internal medicine intensive care unit. There was an unimportant loss of carbohydrates, only, with but few exceptions, i.e. a loss of 11% in 3 patients. The results demonstrate that the carbohydrates of the combined solution become utilized independent of each other. The balance of water, sodium, potassium and magnesium were not only negative but also positive. The elimination of calcium was between 1–19 mval/24 h, of phosphate between 0.6–8.7 mval/24 h.
    Notes: Zusammenfassung Nach intravenöser Dauerinfusion einer Kohlenhydratkombinationslösung, bestehend aus Fruktose, Glukose und Xylit (2/3n1/3n1) mit einer Zufuhrrate von 0,2 g/kg/h und gleichzeitiger Zufuhr von Aminosäuren mit einer Dosierung von 0,03 g/kg/h, fanden sich nur geringe Verluste von Fruktose, Glukose und Xylit. Nur bei 3 Patienten lag der Endverlust über 11%. Die Ergebnisse zeigen, daß unter Anwendung einer Kohlenhydratkombinationslösung die einzelnen Substrate unabhängig voneinander verwertet werden. Die Bilanzen für Wasser, Natrium, Kalium und Magnesium waren teils negativ, teils positiv. Die Ausscheidung von Kalzium bewegte sich zwischen 1 und 19 mval, die von Phosphat zwischen 0,6 und 8,7 mval/24 h.

    Description / Table of Contents: Summary In 30 obese subjects three different methods for weight reduction were applicated over a period of 14 days. One group (n=10) was treated with total starvation, the other group (n=10) with total starvation and 80 mval potassium in addition and the third group (n=10) with a 700 cal. diet. In total starvation the balance of sodium, potassium and phosphate amounted to -9 mval/d, -34,9/d and -8,8 mval/d respectively. Whereas calcium showed a positive balance of 4,4 mval/d. During addition of 80 mval potassium sodium excretion increased whereas potassium excretion was diminished resulting in a potassium balance less negative. During treatment with the 700 calorie diet the highest negative sodium and a high positive potassium balance were observed.
    Notes: Zusammenfassung Bei 30 übergewichtigen Patienten wurden drei verschiedene Methoden der Gewichtsabnahme über 14 Tage durchgeführt. Bei der ersten Gruppe (n = 10) wurde eine Null-Kalorien-Diät mit Mineralwassergabe verordnet, die zweite Gruppe erhielt zusätzlich 80 mval Kalium pro Tag, die dritte Gruppe wurde mit einer definierten 700-Kalorien-Diät behandelt. Bei der Nulldiät betrugen die Bilanzen für Natrium, Kalium und Phosphat-9 mval/d, -34,9 mval/d und -8,8 mval/d. Kalzium zeigte eine positive Bilanz mit 4,4 mval/d. Während der Zugabe von 80 mval Kalium pro Tag stieg die Natriumausscheidung auf 26 mval/d an, während die Kaliumausscheidung absank. Bei der 700-Kalorien-Diät zeigte sich die größte negative Natriumbilanz und eine hochpositive Kaliumbilanz.

    Description / Table of Contents: Summary Within 3 hours, 10 healthy male volunteers were infused 500 ml of a 5 percent emulsion in which 25 percent of the fat proportion had been replaced by MCT-fats. As expected, the ketone body concentration in the blood rose, while pyruvate remained constant and lactate dropped. The results show that, basically, a MCT-containing fat infusion is suited for parenteral nutrition and, because of their specific properties, medium chain triglycerides may be used as rapid energy donators.
    Notes: Zusammenfassung An 10 freiwilligen gesunden männlichen Probanden wurden 500 ml einer 5 %igen Fettinfusion, bei welcher 25 % des Fettanteiles durch MCT-Fette ausgetauscht wurden, innerhalb von 3 Stunden infundiert. Erwartungsgemäß kam es zu einem Anstieg der Ketonkörperkonzentration im Blut, während Pyruvat konstant blieb und Laktat geringfügig abfiel. Die infusionsbedingte Hyperlipidämie war nur geringfügig, die Serumklärung bereits 1 Stunde post infusionem abgeschlossen. Die gewonnenen Ergebnisse zeigen, daß eine MCT-haltige Fettinfusion grundsätzlich für die parenterale Ernährung geeignet ist und aufgrund der spezifischen Eigenschaften mittelkettiger Triglyzeride als rasche Energiedonatoren einsetzbar sind.

    Notes: Zusammenfassung Mittelkettige Triglyceride unterscheiden sich auf Grund ihrer biochemischen und physikalischen Eigenschaften von den üblichen Nahrungsfetten. Durch die weitgehende Unabhängigkeit von der Pankreaslipase und den Gallensäuren, durch ihre rasche Resorption und durch die schnelle Energiebereitstellung stehen die mittelkettigen Triglyceride in der modernen Ernährung als wertvolles Diätetikum bei allen denjenigen Erkrankungen zur Verfügung, die mit Fettmalabsorption und Lymphabflußstörungen einhergehen. Auch der Einsatz bei der fettinduzierten Hyperlipidämie und bei der Hypercholesterinämie ist erfolgreich. In der vorliegenden Arbeit wird die klinische Wertigkeit dieser Fette sowie die verschiedensten Stoffwechselwirkungen diskutiert.
