    Notes: Abstract  We report the case of a 68-year-old patient with a traumatic spinal subdural hematoma. MRI demonstrated an area of abnormal intensity and a black line in the inner part of the intradural space. We anti-cipate that MRI will help to make one more confident in the preoper-ative diagnosis of spinal subdural hematoma. The symptoms complet-ely disappeared immediately after the operation. Spinal subdural he-matoma requires immediate surgical evacuation. The prognosis for func-tional recovery is good if the condi-tion is appropriately diagnosed and treated before development of irre-versible paralysis. We recom-mend MRI to make an early diagnos-is and early evacuation of spinal subdural hematoma.

    Notes: Abstract  Intra-articular membranous interposition was detected by MRI in the hip joint with residual subluxation of a girl aged 5 years 10 months. This structure, which had low signal intensity on both T1- and T2-weighted images, separated the femoral head from the acetabulum. Histological examination revealed chondrometaplasia, which suggested that this interposition might be transformed to a surface cartilaginous tissue of the secondary acetabulum often observed in residual subluxation of the hip.

    Notes: Abstract The isthmic diameter of the vertebral pedicle is an important dimension affecting the stability of transpedicular screwing. The isthmic diameter of the pedicle was measured in the lumbar spines of 18 Japanese cadavers (10 male, 8 female) by axial computed tomographic (CT) scan and directly with calipers. In L1 and L2, there was no significant difference between the average value of the isthmic diameter as determined by the two methods (Student's t-test). However, in L3-5, the average value of the isthmic diameter as determined by axial CT scan was significantly greater than that given by caliper measurement. In view of the fact that caudally the longitudinal axis of the pedicle diverges to wards the mid-line of the vertebral body in the coronal plane, the plane of the CT scan cannot be parallel to the real isthmic diameter of the pedicle. Therefore, it is suggested that below L2 the diameter as determined by axial CT scanning is greater than the real isthmic diameter.

    Notes: Abstract  Objective. To clarify the clinicopathological features of periosteal ganglion. Design. Three patients with periosteal ganglion were studied clinicopathologically. Patients. One patient was selected from the files of our institute and two from a consultation file. Results and conclusions. All three lesions were located over the medial aspect of the tibia. Plain radiographs showed cortical erosions of varying degrees and mild periosteal reaction of the medial side of the tibia. MR images demonstrated well-circumscribed lesions overlying the cortical bone of the tibia, shown as low-intensity areas on T1-weighted images. On T2-weighted images, lesions were homogeneous, lobulated, and showed a characteristic markedly increased signal intensity. These findings are helpful in making a diagnosis of periosteal ganglion. Each patient had an uneventful clinical course after an excision involving the wall of the ganglion, the adjoining periosteum, and the underlying sclerotic cortical bone.

    Notes: Abstract  We describe a malignant granular cell tumor in the semimembranosus muscle of a 61-year-old woman. Magnetic resonance images disclosed a tumor with intermediate and low signal intensity on both T1- and T2-weighted images, measuring about 7×4×4 cm in size. The patient showed an uneventful clinical course during a 30-month follow-up period after the wide excision. The pertinent findings in making the diagnosis of malignant granular cell tumor are large size, intramuscular location, and mitosis including an atypical mitosis.

    Notes: Abstract In the 63 patients with spinal osteoporosis who had been treated with vitamin D3 and calcium supplement, back muscle strength was compared with the following parameters; bone mineral density (BMD) of the distal 1/6 and 1/3 of radius by single photon absorptiometry, BMD of lumbar spine and femoral neck by dual photon absorptiometry, body height, body weight and age. Back muscle strength correlated significantly with BMD of lumbar spine and BMD of radius (1/3), and less with BMD of femoral neck. The strength of back muscle also showed a significant negative correlation with age. Back muscle strength and the body weight were the significant predictors of the bone mineral density of the lumbar spine. These data suggest that back muscle strength has a possibility of affecting bone mineral density of the spine.

    Notes: Abstract In order to explain a risk factor for osteoporosis, effects of chronic malabsorption on bone mass were experimentally assessed. In beagles, the distal three quarters of the small intestine was resected, and effects of the deficit on bone mass was studied. After double tetracycline labelling each dog was sacrificed, and sections of undecalcified lumbar vertebra were prepared for bone morphometry. Effects of postoperative treatment with ursodesoxycholic acid(UDCA) and 1α(OH)D3 or with 1α(OH)D3 alone were also evaluated. The massive resection of the distal small intestine resulted in a decrease of serum lipid level and apparent malabsorption, leading to prominent changes in metabolism of vitamin D such as decreased production of 25(OH)D and 24,25(OH)2D. Six months after the resection, neither a decrease in bone mass nor impaired calcification could be documented in agreement with the lasting normal 1,25 (OH)2D level. Decreases in osteoid volume and thickness, as well as low bone formation rates, suggested insufficient bone matrix synthesis presumably associated with malabsorption. Consequently, bone mass would eventually decrease after massive intestinal resection. UDCA and vit.D3 supplements produced no clear effect on the bone.
