    Description / Table of Contents: Abstract Background and Objective. For rational therapeutic recommendations the spectrum and resistance of bacteria in skin diseases were investigated. Patients/Methods. Within 4 months 911 swabs of dematoses possibly caused by bacteria were taken prospectively (481 outpatients, 430 inpatients) and the material cultured on standard media. The positive cultures including resistance screening of 210 swabs of 168 outpatients and of 175 swabs of 85 inpatients could be evaluated, the remaining cultures were sterile. Results. Staphylococcus aureus was the most frequent pathogen (outpatient 67%, inpatient 61% of all positive cultures), followed by streptococci (groups A and B; 25%). In patients Pseudomonas aeruginosa was the most common pathogen in leg ulcers and between the toes (45% and 70% respectively). 13% of the Staphylococcus aureus isolates were resistant to tetracyclines and erythromycin; one strain proved to be methicillin resistant. Conclusions. Cephalosporins (I. generation), penicillins with b-lactamase-inhibitors, and to a lesser extent isoxazoyl-penicillin and clindamycin can be recommended for the treatment of skin infections. Oral quinolones are suited for infections with gram-negative bacteria (such as beween the toes). The indications for systemic antibiotic therapy of leg ulcers should be restricted.
    Notes: Zusammenfassung Hintergrund und Fragestellung. Zur Erarbeitung rationaler Behandlungsrichtlinien wurde die Erreger- und Resistenzsituation potenziell bakteriell verursachter Hauterkrankungen erfasst. Patienten/Methodik. Innerhalb von 4 Monaten wurden 911 Abstriche entnommen (481 ambulant, 430 stationär) und das Untersuchungsmaterial auf Standardmedien angezüchtet. Bei 210 Abstrichen von 168 ambulanten Patienten sowie 175 von 85 stationären Patienten wurden Erreger einschließlich Antibiogramm ausgewertet, die übrigen waren steril. Ergebnisse. Staphylococcus aureus war am häufigsten nachweisbar (ambulant 67%, stationär 61% aller positiven Kulturen), gefolgt von Streptokokken (Gruppe A,B: 25%). In Ulzera bzw. im Zehenzwischenraum stationärer Patienten dominierte Pseudomonas aeruginosa (45 bzw. 70%). Durchschnittlich waren 13% der Staphylococcus-aureus-Isolate gegen Tetrazykline und Erythromycin resistent, ein Stamm war Methicillin-resistent. Schlussfolgerungen. Zur Behandlung von Hautinfektionen können in erster Linie Cephalosporine (I. Generation) und inhibitorgeschützte Penizilline, eingeschränkt Isoxazoylpenicilline und Clindamycin empfohlen werden. Orale Chinolone sind für Infektionen mit gramnegativen Erregern geeignet (Zehenzwischenraum). Die Indikation zur systemischen antibiotischen Therapie von Ulcera crurum ist restriktiv zu stellen.

    Notes: Abstract The decay of the luminescence of CaWO4, CdWO4, MgWO4, and ZnWO4 after excitation with short pulses of light and electron beams has been investigated at different temperatures. Excitation with UV-light yields an exponential decay, and from the temperature dependence of the decay time it can be concluded that at high temperatures non-radiative transitions are prevalent. After excitation with electrons, there are additional effects by the heating up of the “excitation channel”. In this case the decay is not purely exponential and thermoluminescence is observed. This indicates the existence of traps, which are assumed to be located in the centers of the luminescence.

    Notes: Abstract Oral mucositis is a frequent complication of radiochemotherapy. The origin of radiation-induced mucosal lesions is iatrogenic in nature, although further development of mucositis is essentially influenced by infection. It can be assumed that disinfection measures should decrease the severity of mucositis induced by radiochemotherapy. Therefore, in a prospective randomised study the efficacy of prophylactic oral rinsing with a disinfection agent was investigated. A randomised, prospective comparative trial was conducted with 40 patients undergoing radiochemotherapy of the head and neck region because of malignant disease. The treatment scheme consisted of irradiation to the tumour region and adjacent lymph nodes, with a total dose of 71.3 Gy, and simultaneous chemotherapy with carboplatin (60 mg/m2) on days 1–5 and 29–34. In all patients mucositis prophylaxis with nystatin, rutosides, panthenol and immunoglobulin was undertaken. In addition, 20 patients rinsed the oral cavity 4 times daily with povidone-iodine solution, while the group for comparison rinsed with sterile water. Clinical examination of the oral mucosa was performed weekly. Onset, grading and duration of mucositis were used as the main variables. Clinically manifest oral mucositis was observed in 14 patients in the iodine group (mean grading: 1.0) and in all 20 patients in the control group (mean grading: 3.0). The total duration (mean) of clinically observed mucositis was 2.75 weeks in treatment patients and 9.25 weeks in control patients. Median AUC (area under curve for grade vs duration) was 2.5 in the iodine rinsing patients and 15.75 in control patients. All differences found between the two groups were statistically significant. Increased iodine incorporation was not observed. A pathologic rise in thyroid hormone levels was not found in the iodine group. The results obtained indicate that incidence, severity and duration of radiochemotherapy-induced mucositis can be significantly reduced by oral rinsing with povidone-iodine in addition to the standard prophylaxis scheme. It can be concluded that rinsing with povidone-iodine is an easy, cheap and safe prophylactic method and can be recommended as a supportive treatment during antineoplastic treatment of the head and neck region.

    Notes: Abstract Human monocytes/macrophages (MO/MAC) were isolated from peripheral blood and cultivated on hydrophobic Teflon membranes. This culture system is suitable for HIV infection of MO/MAC in vitro. After transfer into 24-well plates the mature macrophages (infected or uninfected) were used for measurements of phagocytosis. The uptake of different, radioactively labeled nanoparticles (NP) made of polyalkylcyanoacrylate, polymethylmethacrylate (PMMA), and human serum albumin (HSA) by the macrophages was determined. In addition, the influence on phagocytosis of size and composition, concentration, and surface of the NP was studied. Further, macrophages of different state of activation were tested. NP made of polyhexylcyanoacrylate (PHCA) or human serum albumin with a diameter of about 200 nm were found most useful for targeting antiviral substances such as azidotymidine to macrophages. Cells infected in vitro with HIV-1D117/III, a monocytotropic HIV isolate from a perinatally infected child, possessed an even higher phago-cytotic activity than noninfected cells. Macrophages isolated from HIV-infected patients also showed good incorporation of NP. Thus, the concept of a specific targeting of antiviral substances to macrophages in HIV-infected individuals appears quite promising.

Reaction of octacarbonyl dicobalt with 1b yields the dicarbonylcobalt complex 10b. Treatment of 1e with [(C5H5)Ni(CO)]2 results in the formation of the di-nuclear carbonyl-bridged complex 12e.

In 2c the chlorine atoms are substituted by N(CH3)2, N(C2H5)2, and CH3, respectively, with formation of 2d-f. The Lewis-acidic methyl derivative 2f reacts with Cr(CO)6, Mo(CO)6, and Ni(C3H5)2 to yield the complexes 7a, 7b, and 8, respectively. The tetracarbonylchromium complex 7a forms the zwitter-ionic addition product 9 with triethylphosphane. The X-ray structure analyses of 7b and 9 show that the heterocycle in 7b is bent more than in 9.

Der Tetracarbonylchromkomplex 7a bildet mit Triethylphosphan das zwitterionische Additionsprodukt 9. Röntgenstrukturanalysen von 7b und 9 zeigen, daß der Heterocyclus in 7b stärker als in 9 gefaltet ist.
