Zusammenfassung Die im kleinen Netz verlaufende extragastrale vagale Antruminnervation als Reflexvermittler für Sekretion und Motilität ist in ihren Partialfunktionen bisher nicht ganz geklärt. Sicher ist lediglich, daß die zum Fundus-Corpus-Bereich ziehenden Fasern andere Reflexe vermitteln, als die Antruminnervation. Letztere wird bei unserer Methode der Vagotomie routine-mäßig geschont. Es stellt sich nun die Frage, ob neben der motorischen Funktion diese Antruminnervation auch eine Bedeutung für die Gastrinfreisetzung hat. Im Experiment kann das an Hunden mit einer Hauptmagenfistel und einem Heidenhain-Pouch nach einer „selektiven proximalen Vagotomie" durch den Insulintest überprüft werden. Nach dieser Vagotomieform bleiben die Sekretionswerte nach der Insulinstimulation fast im Niveau der Basalsekretion. Dies beweist, daß eine selektive proximale Vagotomie die insulinstimulierte Sekretion des Hauptmagens auf kaum meßbare Säurewerte reduziert. Diese Restsekretion entspricht dem vagal-antralen Anteil der Sekretion. Er kann keinesfalls ulcerogen werden, sofern die Säureintensität überhaupt der wesentliche Faktor der Ulcusätiologie ist.

Summary The extra-gastric innervation of the vagal antrum which can be traced in the small omentum has a partial function as a conductor of the reflexes controlling secretion and motility, which is still not entirely understood. It is only certain that fibres which lead to the area of the fundus and corpus transmit different reflexes from those which innervate the antrum. The latter was always spared when our vagotomy method was used. The question now arises whether the innervation of the antrum, in addition to its motor function, has any bearing on the release of gastrin. This can be tested by experiments carried out on dogs with a main-stomach fistula and Heidenhain-pouch after “selective proximal vagotomy” using the insulin test. After this type of vagotomy, the secretion levels after insulin stimulation remained nearly normal. This proves that a selective proximal vagotomy reduces the insulin-stimulated secretion of the main stomach to barely measurable acid values. The residual secretion corresponds to the vagal-antral portion of the secretion. It cannot be considered ulcerogenic in any way, as the level of acidity is the essential factor in the formation of ulcers.
