    Description / Table of Contents: Résumé La fréquence de complications des iléostomies conventionnelles de Brooke aboutissant à un non fonctionnement est élevée. Ceci compromet la rééducation postopératoire des patients. La moitié environ de ces complications (rétraction avec/sans sténose, inversion intermittente et prolapsus) sont en rapport avec un glissement des surfaces séreuses qui ne sont pas encore suffisamment solidarisées. Trente neuf iléostomies conventionnelles ont été stabilisées par une rangée longitudinale d'agrafes mécaniques paramésentériques. L'iléon avait été éversé de manière classique chez 19 patients ayant une iléostomie de première intention. Chez 11 autres patients, l'iléon avait été rendu proéminent en le tirant à travers l'orifice de la première intervention. Neuf cas d'iléostomies non stabilisées à l'origine l'ont été secondairement en raison de complications secondaires. Au cours d'une période d'observation variant entre 0.2 et 4.2 ans, on n'a pas encore observé de complications de ce type dans les 39 cas d'iléostomies stabilisées par cette technique, alors que nous avons observé un taux de 37% de complications jusqu'à deux ans après l'opération chez 38 patients ayant une iléostomie non stabilisée. Il a été possible de créer une exubérance stomiale plus facilement, avec plus de précision et moins de traumatisme en tirant sur l'intestin à travers l'orifice de la première intervention par rapport à la technique d'éversion classique. Grâce aux agrafes, un patient sur deux n'a pas nécessité de reconstruction d'iléostomie. Par conséquent, i'iléostomie selon Brooke s'effectue plus facilement avec la technique utilisant les agrafes, qui améliore la maintenance ou la restauration de la fonction.
    Abstract: Resumen La tasa de las complicaciones que alteran la buena función de las ileostomías convencionales (Brooke) es alta e interfiere con la rehabilitación postoperaria de los pacientes. Cerca de la mitad de tales complicaciones (retracción con/sin estenosis, recesión y prolapso intermitentes) del estoma prominente clásico es causada por el desplazamiento de las superficies serosas no suficientemente adheridas. Nuestro grupo ha realizado la estabilización de 39 ileostomías convencionales en los sentidos paramesentérico y longitudinal con hileras de grapas (“staples”) metálicas. El íleon fue evertido en la forma clásica en 19 ileostomías primarias. Por lo demás, en 11 casos se estableció la prominencia del estoma halando el íleon a través del estoma previamente suturado al plano de la pared abdominal. También se corrigieron 9 ileostomías que no habían sido previamente estabilizadas mediante la técnica de la sutura con grapas, sin complicaciones. En el curso de un período de observación de 0.2–4.2 años no se presentaron complicaciones de deslizamiento del estoma en las 39 ileostomías estabilizadas con la técnica de las grapas, en tanto que se presentaron complicaciones en 37% de 38 ileostomías no estabilizadas en observación efectuada hasta el segundo año postoperatorio. Fue factible crear la prominencia del estoma en una forma reproducible, más facil, más precisa y menos traumática halando el intestino de su posición a nivel del plano del estoma primario, que mediante la eversión clásica. Con el uso del suturado mecánico (grapas) en las correciones no fue necesaria la construcción local de la ileostomía en uno de cada dos pacientes. En consecuencia, el principio de la ileostomía de Brooke puede ser más facilmente efectuado con la técnica de estabilización con grapas, la cual hace posible el mantenimiento sostenido o la restauración de la función.
    Notes: Abstract The rate of functional complications of conventional (Brooke) ileostomy is high and endangers the complete postoperative rehabilitation of the patients. About half of these complications (retraction with/without stenosis, intermittent recession, and prolapse) occur within the classical prominent stoma and are caused by displacement of the serosa surfaces. We have stabilized conventional ileostomies in 39 patients with rows of staples placed paramesenterially and longitudinally. The ileum was everted in the classical way in the primary construction of ileostomies in 19 patients. On the other hand, in 11 patients the prominence of the stoma was established by pulling the ileum out through the previously plane-sutured stoma. In addition, 9 patients with ileostomies which were not primarily stabilized were corrected with the stapler technique to treat complications. With follow-up ranging from 0.2 years to 4.2 years, there have been no complications due to sliding of the nipple in these 39 patients. In contrast, there were complications in 37% of 38 patients with non-stabilized ileostomies with follow-up to the second postoperative year. It was more easily reproducible, more exact, and less traumatic to create the stoma prominence by pulling the intestine out from a primarily plane stoma than with classical eversion. With use of the stapler in repairing complications, local ileostomy constructions were no longer required in every second patient. Consequently, a Brooke ileostomy can be constructed more easily with the stapler technique, ensuring long-term function or restoration of function.

    Notes: Abstract The effect of rejection on myoelectric activity of an orthotopically transplanted small intestinal segment (group I,N=14) was studied. Electrodes were placed on grafts and recipient small bowel. Isografts (group II,N=5) and native bowel (group III,N=5) served as controls. The first morphological signs of rejection were seen on day 6 and steadily progressed until day 11, when the cellular infiltrate involved all layers of the bowel wall. Slow-wave frequencies remained unchanged throughout the observation period. No difference was detectable between grafts (group I: 31.9±1.65; group II: 31.36±0.7) and native bowel after transection (group I: 32.16±1.78; group II: 31.50±1.01), which was different (P=0.0001) from intact bowel of group III animals (38.4±0.81). Irregular MMCs were detectable in grafts from day 5 on and replaced after food intake by random spiking activities. At day 8, spiking activities disappeared in allografts, which showed a still preserved mucosal architecture, while slow-wave activities continued. These findings demonstrate that intestinal allografts during rejection develop paralysis before mucosal destruction is established, which might be of clinical relevance.

    Notes: Crawford TEM cells are often used to expose cell cultures or small animals in order to study the effects caused by high-frequency fields. They are self-contained, easy-to-use setups that provide a rather homogeneous field distribution in a large area around its center, corresponding approximately to far-field conditions. However, a number of conditions must be met if such TEM cells are intended to be used for in vitro experiments. For instance, poor interaction with the incident field must be maintained to avoid significant field disturbances in the TEM cell. This is best achieved with E-polarization, i.e., when the E-field vector is normal to the investigated cell layer lining the bottom of a synthetic Petri dish. In addition, E-polarization provides the most homogeneous field distribution of all polarizations within the entire layer of cells. In this paper, we present a detailed dosimetric assessment for 60 and 100 mm Petri dishes as well as for a 48-well titer plate at 835 MHz. The dosimetry was performed by using numerical computations. The modeling and the simplifications are validated by a second numerical technique and by experimental measurements. For thin liquid layers, an approximation formula is provided with which the induced field strength for many other experiments conducted in Petri dishes can be assessed reliably. © 1996 Wiley-Liss, Inc.
