Summary Purpose: The technical conditions, the optimized scanning protocols and the facilities of virtual endoscopy (VE) are presented in an overview. Methods: Phantom studies showed that, for a single-row-detector helical CT, collimation of 3.0 mm combined with pitch of 1.5 provided an optimal compromise between image quality and the scan duration per breath-hold. A multi-row-detector helical CT re- quires only a fraction of the scanning time. This is especially important for patients with dyspnea. Results: The threshold-dependent virtual endoscopic surface rendering is a reliable and rapidly practicable reconstruction algorithm for the imaging of the upper airway. Conclusions: The VE technique is suitable for the imaging of space-occupying tumors and restricted stenoses. With VE follow-up examinations can be performed non-invasively and interventional procedures can be pre-pared in an optimal way. Because of the computed tomographic data acquisition the structure of the mucosa is not assessable and there is no opportunity to obtain a biopsy specimen.

Zusammenfassung Ziel: Die technischen Voraussetzungen, die optimierten Untersuchungsprotokolle und die diagnostischen Möglichkeiten der Virtuellen Endoskopie (VE) der oberen Luftwege werden in einer Übersicht vorgestellt. Methode: Phantomstudien konnten nachweisen, dass für ein 1-Zeilen-Spiral-CT-Gerät eine Kollimation von 3,0 mm kombiniert mit einem Tischvorschubsfaktor von 1,5 ein optimierter Kompromiss zwischen der Bildqualität und der tolerablen Untersuchungsdauer pro Atemanhaltephase ist. Ein Mehrzeilen-Spiral-CT der neuesten Generation benötigt bei einer vergleichbaren räumlichen Auflösung nur noch einen Teil der Akquisitionszeit, was insbesondere bei Patienten mit Dyspnoe die Durchführung der Untersuchung vereinfacht. Ergebnisse: Die schwellenwertabhängige virtuell endoskopische Oberflächendarstellung (surface rendering) ist für die Abbildung der oberen Luftwege ein verlässlicher und schnell einsetzbarer Rekonstruktionsalgorithmus. Schlussfolgerungen: Die VE bietet sich für die Darstellung raumfordernder Prozesse und umschriebener Stenosen an. Verlaufs- und Therapiekontrollen können mit der VE nichtinvasiv durchgeführt und interventionelle Maßnahmen optimal geplant werden. Aufgrund der computertomographischen Datenakquisition kann die Struktur der Mukosa nicht beurteilt werden. Ferner fehlt die Möglichkeit zur bioptischen Probeentnahme.
