    Notes: A single-well, “push-pull” test method is proposed for the in situ determination of microbial metabolic activities in ground-water aquifers. The method consists of the pulse-type injection (“push”) of a test solution into the saturated zone of an aquifer through the screen of an existing monitoring well followed by the extraction (“pull”) of the test solution/ground-water mixture from the same well. The test solution contains a tracer and one or more reactive solutes selected to investigate specific microbial activities. During the injection phase, the test solution flows radially away from the monitoring well into the aquifer. Within the aquifer, biologically reactive components of the test solution are converted to various products by the indigenous microbial community. During the extraction phase, flow is reversed and solute concentrations are measured to obtain breakthrough curves, which are used to compute the quantities of reactant(s) consumed and/or product(s) formed during the test and reaction rates. Tests were performed to determine rates of aerobic respiration, denitrification, sulfate reduction, and methanogenesis in a petroleum contaminated aquifer in western Oregon. High rates of oxygen, nitrate, nitrite, and hydrogen utilization and nitrite, and carbon dioxide production support the hypothesis that petroleum contamination has resulted in an increase in microbial activity in the anaerobic portion of the site. The results suggest that the push-pull test method should be useful for obtaining quantitative information on a wide range of in situ microbial processes.

    Description / Table of Contents: Summary Introduction. Nephroblastoma (Wilms-Tumor) is one of the most frequent solid tumors of childhood and only 2% of the affected patients are adults. Case report. We report on a 33 year old female patient with flank pain and hematuria who underwent nephrectomy because of suspected renal cell carcinoma. Histological exam of the renal mass surprisingly revealed adult nephorblastoma and adequate chemotherapy and radiation was initiated. Discussion. Therapy of adult patients with nephroblastoma should be carried out according to the guidelines developed for pediatric patients. Because of the unspecific findings, these patients are typically diagnosed at an advanced stage. Therefore long term outcome in adults is worse compared to children with the same stage of the disease.
    Notes: Zusammenfassung Hintergrund. Das Nephroblastom (Wilms-Tumor) gehört zu den häufigsten soliden Tumoren im Kindesalter und tritt nur in 2% der Fälle bei Erwachsenen auf. Kasuistik. Es wird über eine 33 Jahre alte Patientin mit Flankenschmerz und Hämaturie, die bei Verdacht auf Nierenzellkarzinom nephrektomiert wurde, berichtet. Postoperativ wurden ein adultes Nephroblastom diagnostiziert und eine Polychemotherapie und Bestrahlung eingeleitet. Schlussfolgerung. Die Therapie des Wilms-Tumors beim Erwachsenen richtet sich nach den für kindliche Tumoren erarbeiteten Richtlinien. Aufgrund der uncharakteristischen Befunde wird die Diagnose eines Wilms-Tumors im Erwachsenenalter jedoch häufig verspätet gestellt, sodass die Prognose im Vergleich zu gleichen Tumorstadien im Kindesalter deutlich schlechter ist.

    Description / Table of Contents: Summary Today borate is a rarely used agent. Only few reports about borate intoxication have been published within the last few years. Acute renal failure and skin-lesions are well-known symptoms of an acute borate intoxication. We report on a 14 year old girl with a suicidal ingestion of about 4 g borate. Twelve hours later the girl developed an acute renal failure, which was compensated by hemodialysis. Twenty five days later an allergic-toxic erythema appeared (”boiled lobster syndrome”) in parallel with an eosinophilia of 50% (5184 eosinophiles/µl) in the peripheral blood smear. Under symptomatic therapy eosinophilia and other symptoms resolved completely. Discussion: The very late development of the typical erythema is as remarkable as the unusual, enormous eosinophilia, which has not been reported before in this context. Mostly, mild courses are seen. Our patient recovered completely.
    Notes: Zusammenfassung Borsäure wird heute sehr selten benutzt. Entsprechend sind die Erfahrungen mit Borsäureintoxikationen in den letzten Jahren gering. Akutes Nierenversagen und desquamative Hauterosionen sind bekannte Intoxikationserscheinungen. Ein 14jähriges Mädchen entwickelte 12 h nach der Ingestion von etwa 4 g Borsäure in suizidaler Absicht ein akutes Hämodialyse-pflichtiges Nierenversagen mit Anurie über 14 Tage. 25 Tage nach der Ingestion kam es zu einem desquamativen Exanthem (Boiled-lobster-Syndrom) sowie zu einer ausgeprägten Eosinophilie von 5184/µl bei einer Gesamtleukozytenzahl von 10800/µl. Unter symptomatischer Therapie kam es zu einer schnellen und vollständigen Remission der klinischen Symptomatik sowie der Eosinophilie. Diskussion: Das späte Auftreten des typischen Exanthems sowie die ausgeprägte Eosinophilie sind bemerkenswert und in diesem Ausmaß bisher noch nicht beschrieben.
