    Notizen: A nationwide search for patients with Alport syndrome (AS), a hereditary nephritis with sensorineural hearing loss and occasional ocular anomalies, was performed. As AS is usually transmitted in an X-linked fashion, its form is usually severe in male and mild in female patients. Semi-structured interviews were conducted with 24 patients with AS and their family members from 17 families and 11 pedigrees. The emphasis was on psychosocial stress experienced in everyday life, as well as the influence of AS on family relationships, especially mother–son relationships. Special attention was paid to children’s reactions and feelings as well as the coping strategies of the family.Denial was the most common psychological defence found in our study. We found several cases where AS had led to chronic mental suffering. One of the patterns was a combination of maternal depression and an overprotective attitude toward an AS-affected son. In other instances, depressive symptomatology and anxiety were seen in connection with complications and an adverse outcome of the disease.Families with any AS manifestations should be encouraged to discuss openly the past histories of family members, their fears, feelings of guilt, hopes and expectations. The role and empathic attitude of the clinician is crucial in this process. Mildly affected mothers who have sons with AS are especially in need of psychological support. Professional psychological help should be made available but not given routinely.

    Notizen: It is commonly believed that sensory deprivation can lead to cross-modal reorganization in an immature hut not in a mature brain. The results of the present study suggest, however, that plasticity between sensory modalities is possible even in adults: activity indicating involvement of parietal or occipital brain areas in pitch-change discrimination was found in individuals blinded after childhood. Event-related brain potentials of early blinded (before the age of 2 years). late-blinded (12–28 years of age), and sighted adults were recorded to stimulus sequences consisting of standard tones occasionally replaced by deviant tones. Even when participants were not attending to auditory stimuli, the deviant tones elicited the mismatch negativity (MMN) in each group. There were no significant MMN front-back scalp distribution differences among the groups. However, when participants were detecting deviant stimuli, these stimuli elicited N2 and P3 waves that were posterior in distribution in both groups of blind participants relative to those of the sighted participants. These results suggest that cross-modal reorganization may occur even in the mature human brain.

    Notizen: Abstract While data on the health-related quality of life (HRQOL) of adults are accumulating, very little is known about the HRQOL—and especially the perceived HRQOL—of children. In our study we introduced a 16-dimensional, generic self-assessment measure of HRQOL (16D) for early adolescents, and demonstrated its use with four populations of children aged 12–15: (1) 239 normal schoolchildren, (2) patients waiting for organ transplantation (n=5), (3) patients with genetic skeletal dysplasias (n=19), and (4) patients with epilepsy (n=32). The HRQOL profiles of the patients differed significantly according to the diagnosis, giving support to its construct validity. The reliability of the measure was high: its repeatability coefficient was 91%. The quality of life ratings of the healthy boys and their parents differed on the dimensions of distress, vitality, speech, mental function, and discomfort and symptoms (p〈0.05). In addition, there were significant differences in the health-related valuations between the girls, boys and their parents. We conclude that the assessment of quality of life of adolescents should be based on data collected from the adolescents themselves. Further, the 16D is so far the only generic HRQOL measure designed specifically for this purpose. It is capable of differentiating the HRQOL of healthy adolescents as well as patients with various diagnoses. Our experience also indicates that it is easy to use, yet it seems comprehensive, reliable, and valid.

    Notizen: Abstract Although interest in the health-related quality of life (HRQOL) of children has increased in the last years, validated methods for assessing the HRQOL-and especially the perceived HRQOL—of children have been missing. We introduced a 17-dimensional, illustrated, generic measure of perceived HRQOL (17D) for pre-adolescents, and demonstrated its application to three populations of children aged 8–11 years: (1) 244 normal schoolchildren; (2) 22 patients surviving organ transplantation and (3) 10 patients with genetic skeletal dysplasias. The HRQOL scores and profiles of the patients differed significantly according to the diagnosis, giving support to its construct validity. The reliability of the measure was high: its repeatability coefficient was 95%. As a structured interview of 20–30 minutes, the measurement burden is reasonable. We conclude that the assessment of quality of life of pre-adolescents can and should be based on data collected from the children themselves. Our initial experience indicates that 17D is comprehensive, reliable, and valid.

    Notizen: Abstract Onset of puberty is known to be a stressful period of life. The young adolescent must face several simultaneous developmental challenges, which are reflected as a gap in self-image. In this study, relationships between biological pubertal events and self-image were analyzed in a sample of 59 healthy adolescent boys from the Helsinki area. Detailed information on pubertal development was collected by a pediatrician at three-month intervals. The subjects filled in the Offer Self-Image Questionnaire at 13 and at 14 years and were interviewed by a child psychiatrist. During the year, self-image increased in four domains: Impulse Control, Emotional Tone, Sexual Attitudes, and Psychopathology. The changes in the first two domains were related to pubertal events. In contrast, changes in emotional health and in sexual attitudes did not correlate with bodily changes or testosterone. As regards body image, differences were not found in relation to age, but in relation to genital stages. These findings indicate that some aspects of self-image develop as a function of psychological and cognitive maturation, while other aspects are related to biological events.
