    Notes: Summary This study was designed to examine the consistency of a number of easily identifiable predictive factors in assessing outcome within four days of a subarachnoid haemorrhage. Patients with a proven subarachnoid haemorrhage, aged between 15–65, of any neurological grade who had bled within 72 hours of admission, and who had undergone a CT scan within 96 hours of the ictus, were included. Three groups of patients were studied prospectively. The studies were separated in time and place. The series were similar overall but there were some variations between the three groups of patients because of alterations in referral patterns and management strategies between the series. There were significant differences in the patients' ages, grades on admission, timing of angiography, negative angiography rate and timing of operation. This did not affect overall outcome; 57%, 61% and 59% of the patients in series 1, 2 and 3 respectively making a good recovery. The proportion of patients with a poor outcome was also similar. To identify the level of risk of an individual patient within the first few days of haemorrhage, we considered a number of early predicitive factors. Two emerged as strong predictors of outcome; the early neurological grade and the distribution of blood on the CT scan. We developed a simple scoring system from the first series, based on these findings, designed to predict outcome at three months. The scoring system was calculated on the basis of the distribution of blood seen on the CT scan and the patients' neurological grade on admission. Two points each were scored for interhemispheric, intraventricular, basal or intracerebral blood (excluding blood in the sylvian fissures). Patients in grade 1–3 scored −1, grade 4 scored 0, grades 5&6 scored +5. The scan score and grade score were summated to give the overall score. Patients were placed in risk groups (low, score −1; medium, score 0–2; high, score 3+). The scoring system was then applied prospectively to the two subsequent groups of patients. In each of the three series there was a clear correlation between the patients' scores and their outcomes but more importantly the probability of each outcome for each risk group was considered. In both the second and third series the probability of a full recovery in the low risk group was very likely −P=0.000. In the second series, given a high risk category, there some indication that death was the likely outcome (P=0.085), while in the third series death was significantly the most likely outcome (P=0.001) in the risk category. This study demonstrates that outcome is determined at the time of initial haemorrhage and is reflected by the early neurological grade and the CT distribution of subarachnoid haemorrhage. Other variables, including age, systemic blood pressure, the use of nimodipine or β-blockers, and the timing of operation do not alter the risk grouping of patients, i.e., do not move a patient from high to medium or low risk groups.

    Notes: Summary Ischaemic complications both at the level of the cortex and the hypothalamus are well recognised after an aneurysmal subarachnoid haemorrhage. We have studied histological changes in the cortex (53 patients) and hypothalamus (48 patients) in patients who died after an aneurysmal subarachnoid haemorrhage. Cortical ischaemic lesions were demonstrated in 41 of the 53 patients studied. These changes were more common in patients who had impaired control of systemic blood pressure (p=0.0004) and in patients who died gradually (p=0.0003). Hypothalamic lesions were found in 24 of 48 patients studied; 23 of these patients had widespread associated changes in the cerebral cortex. Patients with moderate/severe cortical changes tended to have hypothalamic lesions and it was uncommon for patients with no cortical lesions to have changes in the hypothalamus (p=0.0007). We believe that these histological changes are due to a diffuse microangiopathy which develops slowly after a subarachnoid haemorrhage and affects the cortex and hypothalamus. Because the cortical lesions are widespread we postulate that they may be implicated in the aetiology of the well described psychosocial or cognitive problems in patients who survive a subarachnoid haemorrhage.

    Notes: Spray-dried encapsulated milk fat powders were prepared from stable emulsions containing 40-60% milk fat and carbohydrate matrices. Moisture content of the spray-dried powders varied from 1-4%. Lowest free fat content (〈10%) was found in powders with 40% fat, encapsulated in sucrose. Angles of repose ranged from 37 to 46°, and correlated with powder flow (p = 0.01). Bulk density was dependent on the encapsulant and declined with increasing fat content. Product density did not influence powder recovery through the cyclone of the dryer. Particle size distribution ranged from 20 to 120 μm with 80% of the particles 〈 100 μm. Powders with best physical properties were made with 40-50% butteroil encapsulated in sucrose.

    Notes: This article describes the modeling of transmission line transformers using methods which are analogous to the methods used to analyze both the ac frequency and transient response of conventional transformers. Transformers in which the lines used to construct them are wound inductively, in order to suppress parasitic short circuit paths within the transformers, are analyzed. It is shown that by using this technique the resulting inductive isolation of the secondary circuit from the primary substantially reduces pulse droop and pulse distortion. Despite the apparent complexity of these transformers, a method by which circuit models of these transformers can be deduced is given. From these models very simple equivalent circuits can be derived which can then be used to calculate accurately the performance characteristics of the transformers and, in particular, predict the pulse distortion characteristics of these devices. Different winding configurations are also considered and it is shown that, by the use of mutually coupled winding of the transmission lines in the transformer, it is possible to minimize pulse droop. Finally it is shown that the modeling technique can be used, in modified form, to analyze the ac frequency response of this type of transformer. © 1999 American Institute of Physics.

    Notes: The initial heteroepitaxial growth and structure of thin CdS layers on GaAs has been studied by conventional and high-resolution transmission electron microscopy. The work highlights the dependence of CdS crystal type on GaAs substrate orientation. Wurtzite-structure CdS is formed on (111)A GaAs and it is found to relieve misfit stresses by the introduction of interfacial defects, often associated with steps at the interface. Sphalerite-structure CdS is produced by initial growth on (001)GaAs and, in this case, misfit stresses are more slowly relieved, first with the formation of an asymmetrical array of interfacial dislocations and inclined stacking faults.
