    Notes: Summary The presence and distribution of different neural markers in 30 neuroblastic tumours (neuroblastomas, ganglioneuroblastomas) and 6 non-neuroblastic tumours were investigated by immunocytochemistry. Neuron-specific enolase (NSE), S-100 protein, tyrosine hydroxylase, neurofilaments and glial fibrillary acidic protein (GFAP) were localized in 3 undifferentiated neuroblastic tumours (group A), 12 poorly differentiated tumours (group B) and 15 well differentiated neuroblastic tumours (group C). Non-neuroblastic tumours (3 lymphomas and 3 Ewing sarcomas) showed no immunoreactivity. Tyrosine hydroxylase and, in particular, NSE were found in mature ganglion cells and developing neuroblasts of poorly and well differentiated tumours (groups B and C). S-100 was localised in neuroblasts with slender cytoplasmic processes in the same groups. Neurofilaments were detected in ganglion cells and differentiated neuroblasts (groups B and C) while GFAP was localised in immature neuroblasts of undifferentiated and poorly differentiated tumours (groups A and B). Thus, there are differences in the neural proteins found in neuroblastic tumours and a wide panel of antibodies against neural markers may be a useful tool in the histological assessment of nervous system neoplasms.

    Notes: Abstract In recent years, alternatives to surgery for difficult bile duct stones have been developed. Routine endoscopy fails in about 10% of patients. To verify the role of extracorporeal shock wave lithotripsy in residual CBD stones, we treated 32 patients by HM4 or MPL 9000 Dornier lithotripters. Ten (34.4%) patients needed two extracorporeal shock wave lithotripsy sessions, and 3 (10.3%) patients three. Complete clearance was achieved in 29 patients (90.6%) after one or more sessions either by endoscopic (20 pts) or percutaneous (9 pts) extraction of the debris; of the remaining 3 patients, in 2 a bilioduodenal stent was placed and in 1 electrohydraulic lithotripsy was performed. Eighteen and seven-tenths percent transient mild hemobilia, 12.5% microhematuria, and no mortality were observed. It is possible to state that in site- or size-related difficult biliary stones, extracorporeal shock wave lithotripsy is a rapid, safe, and highly effective treatment as an additional nonoperative option to resolve the failure of routine endoscopic measures.

    Notes: Abstract A large body of evidence points to a pivotal relationship between Th-1 cells and mucosal inflammation in Crohn's disease (CD). The aim of the present study was to assess whether CD is associated with specific functional activity of lamina propria T lymphocytes (LPT), particularly purified CD4, such as cytotoxic activity and specific cytokine-secreted profile. The results showed that CD4 LPT in patients displayed a chronically activated memory-like surface phenotype and, when compared to controls, had a significantly enhanced antibody-redirected cytotoxicity. Interestingly, the ratio of perforin expression in CD4 LPT was higher compared to controls, and a redirected lysis of human RBC mediated by a CD4 subset of intestinal lamina propria was evident, suggesting a cytolytic pore-forming mechanism. Moreover, a unique Th-1 cytokine profile pattern in the CD4 cells from CD was defined. These effector cells produced 12 times more IFN-γ, two times more TNF-α, and three times less IL-4 than controls. In contrast, no increase in IL-2 was detected, while IL-5 was undetectable. Our studies suggest that these preexisting in vivo activated CD4 LPT may play an important role in the inflammatory process in CD, thus directly contributing to the intestinal lesions.

    Notes: Abstract We have studied the response of thyrotropin to exogenous thyrotropin-releasing hormone in 13 patients who had previous intestinal resection for Crohn's disease, and in 42 healthy controls. An exaggerated and prolonged response curve was found in eight of the patients and one control (P〈0.01), while baseline hormone levels were normal in all. These results may be related to the state of iodine deficiency known to develop in inflammatory bowel disease, but the pathophysiology requires further elucidation.

    Notes: Summary The autonomic nervous system in rats has been assessed by means of indirect immunofluorescence using monospecific antibodies to neuron-specific enolase, neurofilaments, glial fibrillary acidic protein and S-100 protein (10 days after partial (70%) hepatectomy). Different groups of rats were studied: group A: 70% resection and normal dual blood supply (n=5); group B: 70% resection with only portal blood to the liver remnant (n=5); group C: 70% resection with only arterial blood to the liver remnant (n=5); group D: sham operated controls (n=5). All rats of groups A and D showed normal liver/body weight ratios after 10 days in contrast to groups B and C where liver weights were 50–60% of the preresection weight. In group A the regeneration process was histologically normal and associated with a remarkable increase of autonomic innervation patterns in the portal triad. In contrast, livers of animals in groups B and C showed under the light microscope features of hepatocyte degeneration associated with a decreased autonomic innervation compared to the controls. The changes are identical in groups B and C, and are therefore irrespective of the type of blood deprivation (arterial or portal). These results support the importance of dual blood supply for an optimal regenerative response in liver remnants after liver resection. We suggest that the autonomic nerve supply of the portal triad plays at least an important permissive role in liver regeneration.
