    Notes: Summary The ECG-telephone-transmission (TTM) was used to record an ECG-strip during a typical symptomatic period in patients complaining of symptoms possibly caused by arrhythmias (palpitations, dizziness, paroxysm tachycardia, pulse irregularities; angina and dyspnea only if other reasons could be excluded). Patients complaining of syncope only were not admitted, because of the inability to make a telephon call successfully during such a symptomatic period. The ECG was transmitted to the CCU using a frequency modulation technique. In 60% of 196 patients an ECG-TTM could be achieved during a typical symptomatic period, whereas arrhythmias as cause for the symptoms could be excluded in 51 patients (26%). The remaining 66 patients (34%) demonstrated various arrhythmias ranging from simple SVPB and PVC to total AV-block and sustained VT. TTM, an easy-to-perform and cost-effective method allowed a successful ECG registration during a symptomatic period in almost two-thirds of symptomatic patients. In these patients arrhythmias could be verified or excluded as cause of the symptoms.

    Notes: Abstract  A new derivatization method for aromatic amines is presented based on the bromination of the aromatic ring system in an acetic acid medium to yield bromo derivatives. The derivatization method is very simple to perform and overcomes several problems common in the analysis of polar aromatic amines. The derivatives are easy to extract with pentane and can be separated with gas chromatography. Due to the introduction of the electron withdrawing bromo substituents sensitive detection using an electron capture detector is possible. The method was used to investigate the contents of amino- and aminonitrotoluenes in water samples from the site of a former ammunition plant.

    Notes: Abstract An analytical method for the determination of aromatic amines in water is introduced that uses iodination with a Sandmeyer-like reaction to replace the amino group by iodine in aqueous solution. The non-polar derivatives are extracted with pentane or toluene, separated with gas chromatography and sensitively detected with an ECD. Thirteen major metabolites of nitroaromatic explosives were investigated. The method was used to analyze these metabolites in water samples from the site of a former ammunition plant. The results are compared with the derivatization of aromatic amines via bromination of the aromatic ring.

    Notes: Summary The antihypertensive effect of ketanserin 40 mg b.d. in combination with a beta-adrenergic blocking agent or a diuretic was assessed in an open study in 35 patients with essential hypertension, who had not responded to treatment with beta-blockers, diuretics or their combination. The ketanserin/beta-blocker combination decreased mean sitting systolic/diastolic blood pressure (SBP/DBP) from 169/107 mm Hg to 156/91 mm Hg at the end of the 12-week active treatment period. The decrease in systolic blood pressure was significant only at Week 8, while the decrease in diastolic blood pressure was highly significant at all times. A significant reduction in heart rate by 10 beats·min−1 was observed with the ketanserin + β-blocker combination. The ketanserin/diuretic combination led to a significant reduction in mean SBP/DBP from 164/106 mm Hg to 146/92 mm Hg after 12 weeks, with no significant change in heart rate. Body weight slightly increased in both groups. There were significantly fewer adverse reactions with the ketanserin/diuretic combination than with the ketanserin/beta-blocker combination. The results indicate a potentially useful therapeutic role for ketanserin in combination with beta-blockers or diuretics.

    Description / Table of Contents: Zusammenfassung Bei einem 31jährigen, kreislaufgesunden Mann trat im direkten Zusammenhang mit einem stumpfen Thoraxtrauma ein kompletter AV-Block auf. Unter Schrittmacherbehandlung kam es zur Stabilisierung des Kreislaufs und nach 12 Tagen zur Wiederherstellung der AV-Überleitung. 2 Jahre nach dem Unfall läBt sich noch zeitweise ein 2:1 Block nachweisen. Es scheint dies der erste Fall in der Literatur zu sein, der einen kompletten AV-Block nach einem stumpfen Thoraxtrauma überlebte.
    Notes: Summary In a 31-year old patient with previously completely normal circulation, a complete A-V block occurred as a direct consequence of a blunt thoracic trauma. After use of a pacemaker, the circulation was stabilized and 12 days later the AN conduction was restored. 2 years after the accident there was still at times a 2:1 block present. This appears to be the first case in the medical literature of a patient with complete AN block, following blunt thoracic trauma, who survived.

    Description / Table of Contents: Zusammenfassung Schrittmacher von Patienten, bei denen der drohendeSchrittmacherausfall durch die oscillographische Messung erkannt wurde, wurden nach Explantation kontrolliert und insbesondere auch im Langzeitversuch beobachtet. Auf Grund der gewonnenen Ergebnisse läßt sick die oscillographische Impulskontrolle bei Schrittmacherpatienten als sicher bezeichnen, da sowohl eine frühzeitige Erkennung des Kapazitätsverlustes möglich ist, als auch zum Zeitpunkt der Explantation noch ein ausreichender Abstand der Ausgangsspannung der explantierten Schrittmacher und der Reizschwelle der Patienten besteht und schließlich zumindest in vitro ein ausreichender zeitlicher Sicherheitsabstand bis zum Unterschreiten der Reizschwelle der Patienten gefunden wurde.
    Notes: Summary Pacemakers of patients in whom the threatened pacemaker failure was recognized by oscillographic measurement, were controlled after explanation and observed in longterm trials. On the basis of the experience gained, oscillographic impulse control of the paced patient can be considered safe, since it enables early recognition of loss of capacity. At the time of explanation there is a sufficient difference between the potential of the explanted pacemaker and the stimulation threshold of the patient and finally, at least “in vitro”, a sufficiently long interval of time can be guaranteed before it falls below the stimulation threshold of the patient.

    Notes: Abstract After administration of hexachlorobenzene rats excrete sulphur-containing conjugates from which pentachlorothiophenol can be split off. In the present study we describe the identification of pentachlorothiophenol and pentachlorothioanisol in the livers of animals treated with hexachlorobenzene. In order to clarify the further fate of these two substances, we administered them to rats, and isolated the conversion products excreted in the urine and feces. The metabolites of pentachlorothiophenol and pentachlorothioanisol are excreted in both conjugated and free form. From extracts of the excreta, we isolated tetra-and trichlorobenzene with two or three sulphur-containing substituents on the ring, analogous compounds in which thiol groups were converted into sulphoxide and sulphone groups, as well as analogous compounds with a phenolic oxygen in addition to sulphur, and sulphur-containing compounds in which chlorine was replaced by hydrogen. Following administration of the sulphoxide and of the sulphone of pentachlorothioanisol under analogous conditions, pentachlorothiophenol and pentachlorothioanisol and their metabolites were detected in the excreta of the animals. No evidence was obtained that the parent compounds are excreted in the unchanged form.

    Description / Table of Contents: Zusammenfassung Weiblichen Ratten wurde innerhalb von 5 oder 10 Tagen 260 bzw. 390 mg/kg 14C-Hexachlorbenzol intraperitoneal verabfolgt. Nach Ablauf von insgesamt 4 Wochen wurden Kot, Urin und Gewebe der Tiere auf ihren Gehalt an Radioaktivität sowie auf Hexachlorbenzol und Metabolite untersucht. Mit Hilfe der Gaschromatographie, der Isotopen-Verdünnungs-Analyse und der kombinierten Gaschromatographie — Massenspektrometrie wurden im Urin Pentachlorphenol, Tetrachlorhydrochinon und Pentachlorthiophenol als Hauptmetabolite identifiziert. Daneben wurde eines der Isomeren von Tetrachlorthiophenol nachgewiesen. Im Kot wurden nur Pentachlorphenol und Penta-chlorthiophenol als Metabolite identifiziert. Bis zum Ende des Experiments waren mit Urin und Kot 7 bzw. 27% der insgesamt verabfolgten Radioaktivität ausgeschieden. Mehr als 90% der im Urin ausgeschiedenen Radioaktivität waren in den Hauptmetaboliten enthalten. Rund 30% der im Kot ausgeschiedenen Aktivität entfielen auf Abbauprodukte von Hexachlorbenzol. Der verbleibende Anteil der im Kot ausgeschiedenen Radioaktivität war in der unveränderten Substanz enthalten. In den Geweben der Tiere fand sich neben Hexachlorbenzol nur Pentachlorphenol in meßbarer Menge. Auf diesen Metaboliten entfielen 10% der im Blut und weniger als 0,1% der im Fettgewebe festgestellten Radioaktivität. Bei Abschluß des Experimentes entfielen insgesamt 16% der verabfolgten Radioaktivität auf Metabolite von Hexachlorbenzol.
    Notes: Abstract Female rats were dosed intraperitoneally with 14C-hexachlorobenzene. The drug was administered on 2 or 3 occasions. The total doses amounted to 260 and 390 mg/kg 14C-hexachlorobenzene, respectively. Urine and feces from the animals were collected over a period of 4 weeks after the first injection. Both excreta and some tissues of the animals were examined for their content of radioactivity and for hexachlorobenzene and its metabolites. Gas chromatography, isotope dilution analysis, and combined gas chromatography-mass spectrometry were used to identify the metabolites of hexachlorobenzene. In urine pentachlorophenol, tetrachlorohydroquinone, and pentachlorothiophenol were present as major metabolites. One of the isomers of tetrachlorothiophenol was present as a minor metabolite. In the feces pentachlorophenol and penta-chlorothiophenol only were identified. At the end of the experiment, carbon-14 excreted with urine and feces amounted to 7% and 27%, respectively, of the radioactivity administered. More than 90% of carbon-14 excreted in urine was contained in the major metabolites. In the feces about 30% of the excreted radioactivity was bound to metabolites and about 70% was contained in the unchanged drug, while in the tissues of the animals only pentachlorophenol was detected in measurable amounts, accounting for 10% of label in blood and less than 0.1% of carbon-14 determined in body fat. Total radioactivity contained in the metabolites detected in the animal body and in the excreta at the end of the experiment accounted for about 16% of the administered radioactivity.
