Summary 1. After observations on the psychology of severe pain and on anatomy of the higher segments of the pain-system, the treatment of chronic unbearable pain is reviewed. 2. Based on the literature, psychosurgical operations against pain are discussed, above all leucotomy with its variations and topectomy. 3. Report on 10 patients with pain, 9 of whom had been treated with leucotomy and one with topectomy. 4. According to our experience, the postoperative success in the first months is frequent, but only together with a simultaneous change of the person in general. 5. Relapses occur regularly after some months. Therefore the indications of successful operations are strongly limited.
Abstract:
Résumé 1. Après des observations de la psychologie des graves douleurs et de l'anatomie des plus hauts segments du système de la douleur, on jette un coup d'oeil sur le traitement des douleurs chroniques insupportables. 2. En se basant sur la littérature médicale, on fait des opérations psychochirurgicales pour les douleurs, surtout la leucotomie avec ses variations et la topectomie. 3. Rapport sur 10 malades souffrant de douleurs, dont 9 ont été traité avec la leucotomie et 1 avec la topectomie. 4. Selon nos expériences, les succès post-opératoires dans les premiers mois sont très fréquents, mais seulement avec un changement simultané de la personne en général. 5. Après quelques mois régulièrement on constate une rechute. Les indications d'opérations couronnées par le succès sont très limitées par conséquent.

Zusammenfassung 1. Nach Bemerkungen zur Psychologie schwerer Schmerzzustände und zur Anatomie der höheren Abschnitte des Schmerzsystems wird ein Überblick über die Behandlung unerträglicher chronischer Schmerzen gegeben. 2. An Hand der Literatur werden psychochirurgische Eingriffe bei Schmerzen, vor allem die Leukotomie mit ihren Varianten und die Topektomie besprochen. 3. Bericht über 10 eigene Schmerzkranke, von denen 9 leukotomiert und einer topektomiert wurden. 4. Nach unseren Erfahrungen sind Erfolge in den ersten Monaten post Operationem häufig, aber nur bei gleichzeitiger allgemeiner Wesensveränderung. 5. Nach einigen Monaten sind Rückfälle die Regel. Die Indikationen der erfolgreichen Eingriffe werden dadurch stark eingeschränkt.
