    Description / Table of Contents: Summary In a 43 years old woman with an inoperable ovarian carcinoma, as shown by surgical exploration, chemotherapeutical treatment with Endoxan was performed (total dose 9,8 g). Hereafter a ‘second look operation’ became feasible (hysterectomy+adnexectomy). The histological examination showed remnants of the original tumor only in one ovary. Besides, however, a spread carcinoma in situ of the cervix uteri was found with a beginning invasion of the stroma. The cytological examination before Endoxan treatment was negative.
    Notes: Zusammenfassung Bei einer 43jährigen Frau mit inoperablem Ovarialcarcinom wurde eine chemotherapeutische Behandlung mit Endoxan durchgeführt (Gesamtdosis 9,8 g). Danach war eine Zweit-Operation möglich (Hysterectomie mit beiden Adnexen). Die histologische Untersuchung zeigte Reste des ursprünglichen Tumors nur in einem Ovar. Daneben wurde jedoch ein ausgedehntes Carcinoma in situ der Cervix uteri gefunden, mit beginnender Invasion in das Stroma. Die cytologische Untersuchung vor der Endoxantherapie war negativ gewesen.

    Description / Table of Contents: Summary A new syndrome with deficiency of glutathione reductase, hematological and neurological disturbances is described. 1. There will be found thrombocytopenia, nonspherocytic hemolytic anemia or pancytopenia. Liver and splen are always enlarged. 2. Pancytopenia may become dangerous after prolonged therapy with phenopyrine derivates, primaquine, or chloroquine; death may occur. 3. Neurologically all patients with enzyme defect from 4 families showed spastic signs, an increase of-waves in the EEG and in 1 family also oligophrenia (metabolic oligophrenia?). 4. Glutathione reductase deficiency could be shown in erythrocytes, leucocytes and thrombocytes. Also as far as the nervous system is concerned, the enzyme defect seems to be the cause of the disturbances. Thek M -values for GSSG was 2 to 3 times above normal, the pH optimum was shifted from pH 6.8 to 6.4. Thek M for NADPH2 was normal. 5. The GSH content of the erythrocytes was normal also in the patients with the enzyme defect. The GSH stability to acetylphenylhydrazine was normal or slightly pathologic. The Heinzbody test (Beutler test) was always markedly pathologic. 6. There is not much probability for an inhibitor or a deficient, stroma bound activator to be the cause of the decrease of the activity of glutathione reductase. 7. The enzyme defect and the neurologic and hematologic changes was autosomally dominant in the family Bo. (15 members). 8. There is no successful therapy for these patients as yet. Drugs, which can cause the manifestation of the enzyme defect, should be strictly avoided.
    Notes: Zusammenfassung In der vorliegenden Arbeit wird ein neues klinisches Syndrom mit Glutathionreduktasemangel, hämatologischen und neurologischen Störungen beschrieben. 1. Die klinische Manifestation des erblichen Enzymdefektes besteht hämatologisch in einer Thrombocytopenie, nichtsphärocytärer hämolytischen Anämie oder Pancytopenie. Leber und Milz sind stets vergrößert. 2. Der z.T. schweren Pancytopenie ging bei drei untersuchten Kranken ein langfristiger Medikamentengebrauch (Phenobutazonderivate, Primaquine, Resochin) voraus. In schwächerer Form scheint diese jedoch auch schon vorher bestanden zu haben. Nach Absetzen der Präparate kam es bei einem Kranken zu einer vorübergehenden Teilremission des Blutstatus. 3. Neurologisch fanden sich bei allen Enzymdefektträgern Pyramidenbahnzeichen (Babinski-Gruppe) und im Elektroencephalogramm eine leichte bis mäßige Allgemeinveränderung mit vermehrten Theta-Wellen. In einer Familie bestand gleichzeitig eine Oligophrenie. Da der Schwachsinn alle Enzymdefektträger betraf, muß an einen metabolischen Schwachsinn gedacht werden. 4. Biochemische Untersuchungen ergaben, daß es sich nicht um einen Mangel an normaler Glutathionreduktase, sondern um die Bildung einer pathologischen Glutathionreduktase handelt. Das abnormale Enzym zeichnet sich durch eine Verschiebung des pH-Optimums zum Sauren und eine Erhöhung der Halbsättigungskonstanten für GSSG aus. Die Michaeliskonstante für NADPH2 war normal. Von dem Enzymdefekt sind Erythrocyten, Leukocyten und Thrombocyten in gleicher Weise betroffen. Die Glutathionstabilität gegen Acetylphenylhydrazin ist noch normal oder leicht pathologisch, der Heinzkörper-Test stets stark pathologisch. 5. Eine Hemmung des Enzyms durch einen Hemmkörper oder das Fehlen eines stromagebundenen Aktivators konnte unwahrscheinlich gemacht werden. 6. Der Erbgang des Enzymdefektes in einer 15köpfigen Familie war — ebenso wie die Thrombocytopenie und die neurologischen Störungen — autosomal dominant. 7. Therapeutisch brachte die Behandlung mit anabolem Hormon und Nebennierenrindensteroiden keinen sicheren Effekt. Die einmal durchgeführte Milzexstirpation besserte lediglich die Thrombocytopenie, während die nichtsphärocytäre hämolytische Anämie unverändert blieb. Von den drei Kranken mit Pancytopenie starben alle in wenigen Monaten. Alle oben angegebenen Medikamente sollten strikt vermieden werden.

    Notes: High pressure neutron diffraction measurements of the static structure factor of fluid SF6 are presented. The density of the liquid was varied in a range of ρ=0.85 g cm−3 up to ρ=1.85g cm−3 at the supercritical temperature 398 K. Aspects of the measurements and data evaluation, as far as they are specific for high pressure studies, are discussed. The intramolecular structure parameters are determined. The measurements were interpreted with statistical mechanical calculations (XRISM) as well as reverse Monte Carlo (RMC) calculations. The individual atom pair correlation functions are given and their dependence on density is discussed. An intermolecular potential of (12–6) LJ-type with added point charges is deduced. The density dependent measurements proved to be useful for appraising the intermolecular potential.

    Notes: Only a limited number of patients with advanced ovarian cancer obtain a long-term disease-free interval. The purpose of this study is to find new active drugs and/or regimens. Carboplatin 200 mg m−2 day 1, cisplatin 50 mg m−2 days 2 and 3, and etoposide 70 mg m−2 i.v. days 1–5 every 4 weeks for six cycles were given to 43 previously untreated ovarian cancer patients with residual disease after primary laparotomy.The median observation time was 75 months (range, 63–87 months). Forty-two patients were evaluable for response and the overall pathological response rate was 57% (pathologic complete response (PCR) plus pathological partial response (PPR)), with 26% being PCRs (11/42; 95% confidence interval, 0.14–0.42). The main hematologic toxicity was thrombocytopenia grade 3–4 which occured in 95% of the cycles. Dose-limiting leucocytopenia only occured in 21%. Non-hematologic toxicity caused dose-reduction in 33% of the patients, with nephrotoxicity being the main reason (24%).The purpose of the study was to give a high total dose of platinum in combination with etoposide. However, this was not feasible due to excessive cumulative hematologic toxicity and nephrotoxicity. As the activity of the combination was no better than that obtained with other, less toxic regimens, the combination cannot be recommended.

    Notes: A number of strength parameters were investigated in a group of subjects with chronic anterior cruciate ligament deficiency; The response of subjects' hamstring muscles to a perturbation on an isokinetic dynamometer was fractionated into the premotor and motor times. Rate of force development in the 75 ms following the perturbation was also measured. The functional ability of the subjects was examined using the Noyes knee rating system. The results showed that the motor time was significantly correlated with the rate of force development and functional ability of the subjects. The ratio of hamstring to quadriceps torque recorded at peak and 30° flexion during isokinetic muscle work at 180°/s was also investigated. No significant relationship was observed between these ratios and functional ability. These findings indicate that factors other than peak hamstring muscle torque may be important to the level of functional ability attained by people with chronic anterior cruciate ligament deficiency.
