    Notes: Abstract: In the last decade it has become well established that in the skin, nitric oxide (NO), a diffusable gas, mediates various physiologic functions ranging from the regulation of cutaneous blood flow to melanogenesis. If produced in excess, NO combines with superoxide anion to form peroxynitrite (ONOO–), a cytotoxic oxidant that has been made responsible for tissue injury during shock, inflammation and ischemia-reperfusion. The opposite effects of NO and ONOO– on various cellular processes may explain the ‘double-edged sword’ nature of NO depending on whether or not cellular conditions favour peroxynitrite formation. Peroxynitrite has been shown to activate the nuclear nick sensor enzyme, poly(ADP-ribose) polymerase (PARP). Overactivation of PARP depletes the cellular stores of NAD+, the substrate of PARP, and the ensuing ‘cellular energetic catastrophy’ results in necrotic cell death. Whereas the role of NO in numerous skin diseases including wound healing, burn injury, psoriasis, irritant and allergic contact dermatitis, ultraviolet (UV) light-induced sunburn erythema and the control of skin infections has been extensively documented, the intracutaneous role of peroxynitrite and PARP has not been fully explored. We have recently demonstrated peroxynitrite production, DNA breakage and PARP activation in a murine model of contact hypersensitivity, and propose that the peroxynitrite-PARP route represents a common pathway in the pathomechanism of inflammatory skin diseases. Here we briefly review the role of NO in skin pathology and focus on the possible roles played by peroxynitrite and PARP in various skin diseases.

    Notes: A vesiculobullous eruption with clinical and histological features of bullous pemphigoid developed in a 38-year-old woman with proved systemic lupus erythematosus. The patient had a sulphone responsive blistering disease that was characterized by pruritic subepidermal bullae and linear, predominantly IgA basement membrane zone deposition and IgA pemphigoid antibodies in her sera. Because both diseases are associated with immune complexes of special immunoglobulin classes, this association may not be entirely fortuitous.

    Notes: Abstract. Double parathyroid adenoma (DA) has been suggested to be a clinically distinct entity of primary hyperparathyroidism (HPT). Altogether 659 patients with sporadic primary HPT were analyzed retrospectively by evaluating consecutive primary operations for more than three decades. Patients with postoperatively persistent HPT and those with incomplete medical, operative, or histologic records were disregarded. The mean age (± SD) at surgery was 60.9 ± 12.7 years; 78% of the patients were female; and the duration of postoperative follow-up averaged 7.8 ± 7.2 years. DA, defined as two enlarged parathyroid glands, was found in 77 patients (12%). Enlargement of a single gland (SA) or three or more glands (MA) occurred in 80% and 8%, respectively. DAs were bilateral in 72% of patients, and the weight of the two glands differed by 317 ± 407 mg. The three histologic subgroups of patients exhibited no significant discrepancy with respect to age, classic symptoms of HPT, proportion of overtly asymptomatic individuals, or parathyroid tissue weight. A lower female predominance and extent of hypercalcemia and higher preoperative serum creatinine level occurred with DA and MA compared to SA. SA and DA differed from MA with respect to variably strong trends to lower postoperative incidences of recurrent HPT and hypoparathyroidism. DA seems to comprise an underrated histologic cause of HPT, which is eligible for a conservative operative approach. Clinical characteristics consistent with a distinct entity of sporadic primary HPT were not seen.

Ihre immunfluorescenzmikroskopischen Untersuchungen ließen bei allergischer Kontaktdermatitis- und Arzneimittelallergie-Patienten in den Gefäßwänden Präalbuminablagerung und in der Dermis Präalbumin-positive Entzündungszellen (Granulocyten, Lymphocyten, Makrophagen) feststellen. Bei arzneimittelallergischen Patienten konnte von der Membran der Lymphocyten ein mitogen wirksames Agens, offenbar ein Antigen, eluiert werden, welches in einigen Fällen auch Präalbumin enthielt. Diese neueren Befunde unterstützen ebenfalls die frühere Hypothese der Autoren von der Hapten-tragenden Rolle des Präalbumins.
    Notes: Summary A significant decrease of serum prealbumin levels and an increased proportion of prealbumin bearing lymphocytes in peripheral blood have been found in patients of hapten-induced diseases. Prealbumin deposition in the vessel walls, and/or prealbumin positive cells (granulocytes, lymphocytes, macrophages) in the dermis have been observed in patients with proved drug allergy as well as allergic contact dermatitis using immunofluorescence technique. The eluates from membrane of lymphocytes of patients with drug allergy contained a mitogenic substance, probably antigen and sometimes prealbumin as well. These results also support the earlier suggested hypothesis that the prealbumin may act as a hapten carrier protein.
