    Notes: Abstract Twenty-three children with severe asthma who had been resident at high altitude for at least 3 months, considerably reduced their requirement for regular anti-asthma therapy. On a standard exercise challenge in low humidity, ten had no response, seven had isolated immediate bronchospasm and six had biphasic responses with immediate bronchospasm followed 4–10 h later by a late reaction sustained for at least 2h. The existence of biphasic responses after exercise, similar to those observed after antigen challenge, supports the concept of mast cell involvement in exercise-induced asthma.

    Notes: Abstract Six infants were referred with symptoms and clinical signs suggesting airway obstruction during sleep. In each case, overnight recordings of arterial oxygen saturation, respiratory movements and end tidal expired carbon dioxide (ETCO2) showed the presence of abnormal episodes of hypoxaemia related to partial or complete airway obstruction and associated with a specific pattern of the inspiratory movement waveforms. These events and patterns were not found on recordings from 20 age-matched healthy infants and young children. ETCO2 levels were also abnormally elevated in all six patients when asleep. Fiber-optic upper airway endoscopy excluded structural abnormalities, including significant tonsillar or adenoidal enlargement, but showed an intermittent dysfunctional inspiratory obstruction in the pharynx. Continuous positive airways pressure and tracheostomy were effective in treating this obstruction.

    Notes: Abstract The vital capacity (VC) in patients with congential scoliosis was compared with the VC in idiopathic scoliosis. For any given Cobb angle (a measure of spinal angulation) the loss in VC was approximately 15% greater in congenital scoliosis than in idiopathic scoliosis. The greater impairment of lung function in congenital scoliosis may be due to the associated rib deformity or to an underlying lung anomaly.

    Notes: Background Raised peripheral blood mononuclear cells (PBMCs) proliferative responses to food allergens have been demonstrated in children with established atopic dermatitis.Objective In this report we investigate the PBMC proliferative responses to inhalant and food allergens from babies at birth, 6 months and 1 year of age, born to atopic and non-atopic parents.Methods PBMCs, separated by density gradient centrifugation. were cultured for 6 days with autologous plasma and a range of allergens (house dust mite [HDM], cat, grass pollen, tree pollen, betalactoglobulin and ovalbumin). Proliferative responses were measured by the uptake of [3H] thymidine added for the final 18 h of culture.Results At birth, infants born to atopic parents who developed allergic disease by 1 year of age had significantly more positive responses (stimulation index ± 2 with a value of ± 1000 cpm above background) to HDM (P = 0.0091), betalactoglobulin (P= 0.0166) and ovalbumin (P = 0.0035) than newborns who did not develop allergy. Tnfants who developed allergy also had significantly more positive responses to HDM (P - 0.03) and ovalbumin (P = 0.0057) than babies, born to non-atopic parents, who did not develop allergies. At 6 months of age a significant fall in response to HDM (P = 0.003) and cat fur extract (P = 0.006) was seen in infants who developed allergic disease by 1 year of age. A similar pattern was seen for proliferative responses to betalactoglobulin and ovalbumin (P = 0.0006. P= 0.004). Conversely, proliferations to grass and tree pollen extracts increased at 6 months (P = 0.04. P = NS) and 1 year (P= NS. P= 0.01) compared with birth which was significant for infants who did not develop allergic disease.Conclusion Proliferative responses to seasonal allergens increased over the first year of life whilst those to perennial allergens, both inhalant and food, fell. This suggests either the induction of a systemic immune tolerance by perennial exposure to antigens or movement of sensitized cells to target organs where allergen exposure occurs. This process may be independent of the development allergic disease.

    Notes: Extracts of Aspergillus fumigatus are required for the measurement of specific antibodies that are important indices in the diagnosis of allergic bronchopulmonary aspergillosis (ABPA). This study investigated the effect of different culture conditions on the production and release of antigenic and allergenic proteins of A. fumigatus. Increasing the incubation temperature from 25°C to 37°C altered the production of proteins by the mycelium which resulted in the release of a greater number of proteins that reacted with precipitating antibodies. Static sporulating cultures produced a much wider antigenic spectrum than shake cultures although the number of precipitating proteins (5 and 3 respectively) and major IgE binding proteins (5 and 3 respectively) was not greatly altered. The widest range of proteins bound by precipitating antibody or IgE from ABPA serum were released into the culture filtrate during 28 day static incubation at 37°C. The resultant extract proved useful for screening patients for specific IgE and will provide a starting material for the identification of individual antigens or allergens.

    Notes: This study was set up to evaluate the food panel of a multiple specific IgE antibody assay in 67 atopic asthmatic children by comparing it to the conventional radioallergosorbent test (RAST) and skin-prick tests (SPT) and then comparing the results of these investigations with the parents perceptions of food related problems.Fifteen food specific IgE antibodies were measured using the multiple chemiluminescence assay (MAST-CLA). IgE antibodies to five of these food allergens were also measured by conventional RAST and SPTs were performed in 43 using 11 standardized food extracts matched to the multiple allergosorbent chemiluminescent assay (MASTCLA) profile. SPT and MAST-CLA results showed good agreement with one another, range 688-96.7% (average 87%), with significant correlation for most allergens tested. MAST-CLA was discrepant with RAST and/or SPTs in 58/210 (27 6%).A questionnaire was sent to the parents to determine their perception of food related symptoms. Sixty-two (92%) questionnaire replies were received, of which 56% reported symptoms with food. The most frequent symptom perceived to be due to food intolerance was behavioural disturbance. The commonest foods implicated were additives (39%), egg (27%), milk (26%), chocolate (23%) and orange (15%). History, SPT, MAST-CLA and RAST were compared for live allergens in 42 patients (210 values). In 14/210 (6·7%), all the tests were negative despite reported symptoms. Conversely in 49/210 (23·3%) at least one test was positive without symptoms.This study did not support a benefit of multiple antibody testing instead of individually selected RASTs or SPTs. The frequent perception of food related symptoms in these asthmatic children was often not supported by SPT, RAST and/or MAST-CLA. This may be a reflection of the current public concern about food, or of non-IgE mechanisms. These uncertainties can only be resolved by double blind placebo controlled food challenge. The inclusion of food specific IgE antibodies together with inhalant antibodies in a multiple test system for use in atopic asthmatics may be misleading.

    Notes: Children with allergic perennial rhinitis had a four-fold greater likelihood of having had an ear, nose and throat (ENT) operation than children with orthopaedic problems. Whilst the patients who had ENT operations had significantly lower serum IgA and IgE levels than the non-operated patients, there was no difference in clinical features of atopy or allergy skin-test responses between the two groups of rhinitic patients. Forty per cent of the perennial rhinitics had an improvement in symptoms following ENT operations, whereas 90% improved on medical therapy. Thus, patients with perennial rhinitis should have allergy investigations and the benefit of medical treatment prior to consideration for surgery.

    Notes: This study describes the effect of an orally administered grass-pollen vaccine, taken pre-seasonally, on the clinical symptoms of fifty-four children with hay fever, asthma or both, during the 1982 grass-pollen season.Oral mixed grass-pollen vaccine and a matched placebo were randomly allocated to form two matched groups. Although small differences were observed in favour of the active group for both hay fever and asthma, no significant advantage was demonstrated over placebo in terms of symptom scores or additional medication required.

    Notes: In children with perennial asthma, dual (immediate and late) reactions in the skin to D. pteronyssinus and Timothy grass pollen were more frequent with high doses of antigen and were associated with large immediate reactions. The frequency of dual bronchial or nasal reactions was not related to the size of the immediate reaction and dual reactions were commonly elicited to the lowest antigen dose which would elicit an immediate reaction. Serum IgG, IgA, IgM or IgE concentrations, IgE or IgG antibodies to the antigens and defective yeast opsonization did not differ in children with dual or immediate only reactions in skin, nose or lung. In five patients undergoing bronchial provocation tests with D. pteronyssinus there was no fever, no fine crepitations in the kings and no significant change in the levels of C3.
