    Notes: A case of necrotizing sialometaplasia of the lip in an 68-year-old pipe smoker is described. Necrotizing sialometaplasia is a self-healing non-neoplastic disease probably of ischaemic nature. Thirty-nine cases of sialometaplasia are described in the literature up to early 1979. These cases appeared in the palate, nasal cavity, gingiva, lip, hypopharynx and maxillary sinus. Six cases have also been reported from major salivary glands. Histologically there is necrosis of mucous cells with partial replacement by squamous epithelium. This entity has often been mistaken for squamous or mucoepidermoid carcinoma. One has to be familiar with the existence of necrotizing sialometaplasia in order to avoid subjecting the patient unnecessary and potentially mutilating surgery.

    Notes: Pharmacokinetic data obtained after one dose of a 2-mg budesonide enema were compared with data obtained after the last dose of four weeks of daily treatment in 24 patients with active distal ulcerative colitis or proctitis. This open multicentre study involved 28 eligible patients. Sigmoidoscopy and biopsy scores improved significantly (P 〈 0.002) during the four-week treatment period. Maximal plasma concentration (Cmax) of budesonide was 2.1 nmol/L 1.3 h after the first dose and 2.5 nmol/L 1.2 h after the last dose; the difference was not significant. The area under the curve (AUC) of plasma concentration vs. time was after the first dose 9.7 nmol h/L and after the last dose 11.6 nmol h/L (P 〈 0.03). The small increase in AUC may be attributed to improved absorption. During the last dose interval, minimal plasma concentration was below the limit of quantitation in most subjects. The Cmax and AUC of budesonide increased slightly after four weeks of treatment, but budesonide did not accumulate. Mean morning plasma cortisol values did not change significantly during treatment (P= 0.083), although a small change in cortisol levels between the first visit (pre-treatment) and last visit was positively correlated to the C. of budesonide measured at the last visit (P= 0.012).

    Notes: Methods: Efficacy and safety of the topically acting glucocorticosteroid budesonide retention enema (2.3 mg/ 11 5 mL) were compared with prednisolone disodium phosphate enema (31.25 mg/125 mL) in patients with active distal ulcerative colitis. The study was a randomized, multicentre trial, with two parallel groups and single-blind to the investigator. One hundred patients with active ulcerative colitis, not reaching beyond the splenic flexure as determined by endoscopy, were treated for up to 8 weeks. Results: Forty-five patients were randomized to receive budesonide and 5 5 to prednisolone. Both treatment groups improved significantly in terms of endoscopic and histological scoring during the study, but there were no statistically significant differences between the two groups. Clinical remission, defined as no more than three daily bowel movements without blood and endoscopically non-inflamed mucosa, was achieved in 16% of the patients in the budesonide group after four weeks and in 24% in the prednisolone group (N.S.).After 8 weeks treatment the clinical remission rate in the groups had increased to 36 % for budesonide and 47% for prednisolone (N.S.).Mean morning plasma cortisol levels were unchanged in the budesonide group, whereas they were significantly suppressed in the prednisolone group after 2, 4 and 8 weeks (P 〈 0.0001). Side effects were mild and rare in both groups. Conclusions: Treatment with budesonide enema in active distal ulcerative colitis was comparable, regarding efficacy, to treatment with conventional prednisolone enema. A prolongation of the treatment time from 4 to 8 weeks doubled the clinical remission rate in both groups. However, budesonide may be preferable to prednisolone since it causes less systemic effects as reflected by a lack of plasma cortisol suppression.

    Notes: Aims: To study the effect of local or parenteral administration of the glucocorticoid budesonide in the acetic acid-induced colitis model in the rat.Methods: Colitis was induced in an exteriorized colonic segment by administration of 4% acetic acid for 15 s. Four days later, this colonic segment with colitis was examined using a morphological scoring system, and measurements of myeloperoxidase activity and of plasma exudation into the colonic segment. The experimental colitis showed morphological similarities to human ulcerative colitis, with 3-fold increase in myeloperoxidase activity and 6-fold increase in the plasma exudation. Budesonide in different doses administered for 3 days, starting one day after acetic acid instillation, prevented the development of colitis in a dose-dependent manner. The best effect of budesonide on the morphological score was achieved after local treatment at a dose of 10″5 M twice daily (76% reduction compared with a control colitis group) and parenteral treatment with 0.75 mg/kg (80% reduction). These doses also normalized myeloperoxidase activity and significantly reduced the plasma exudation. The systemic effects of the drug were most pronounced in the group treated with parenteral budesonide. This group showed the greatest reduction in body weight and a significant reduction of the weight of adrenal glands and spleen (as compared to controls). Thymus weight in animals treated systemically was significantly lower than in locally treated animals. In the group treated with local budesonide the weight of adrenals was reduced. However, the weights of spleen and thymus were not reduced and the reduction of the body weight was even less than in the control group.Conclusion: Local treatment with budesonide at a dose of 10−5 M (0.17 mg/kg if completely absorbed, but only 0.03 mg/kg with 15% bioavailability on colonic application) was as effective as parenteral treatment at a dose of 0.75 mg/kg in the attenuation of acetic acid-induced colitis in the rat, but resulted in minor systemic side-effects.

    Description / Table of Contents: Summary A 4-year-old girl with immunopathy and a hereditary neurological syndrome [previously described in Acta Paediat. Scand. 55, 264 (1966)] succumbed with varicella generalisata. At autopsy hemorrhagic lesions were found in the skin, liver, lungs, pancreas and spleen. With the light microscope intranuclear inclusion bodies were seen in the liver, lungs and spleen. Under the electron microscope viral particles were found in these three organs and in the pancreas. Virus isolation in human embryonic cells with characteristics of varicella proved positive for heart-blood, skin and lungs. In the cerebrum there was a dysplasia of nerve cells in the cortex and basal ganglia and heterotopic nerve cells were found in the white matter. In the cerebellum there was a patchy loss of Purkinje cells, probably a dysplasia. Non-specific degeneration was seen in dorsal root ganglia and in the lateral corticospinal tracts. A younger brother of the patient shows a similar neurological syndrome. It seems reasonable to assume that the neurological syndrome is genetically determined. Whether an immunopathy is present in the brother is not known at present.
    Notes: Zusammenfassung Ein 4 Jahre altes Mädchen mit Immundefekt und hereditärem neurologischem Syndrome [frühere Publikation in Acta Paediat. Scand. 55, 264 (1966)] verstarb an einer generalisierten Varicelleninfektion. Bei der Sektion fanden sich ausgedehnte hämorrhagische Nekrosen im Bereich der Haut, Leber, Lungen, Milz und des Pankreas. Lichtmikroskopisch konnten intranucleäre Einschlußkörper in der Leber, Milz und in den Lungen nachgewiesen werden. Außerdem wurden elektronenmikroskopisch Viruspartikeln in der Leber, Milz, den Lungen und im Pankreas beobachtet. Eine für Varicellen charakteristische Virusisolierung gelang durch Übertragung von Herzblut, Haut- und Lungengewebe auf menschliche embryonale Zellkulturen. Im Gehirn fanden sich die Zeichen einer Fehlbildung in Form von Nervenzelldysplasien in der Hirnrinde und den basalen Ganglien sowie einer Heterotopie von Nervenzellen in der weißen Substanz. Im Kleinhirn war ein unregelmäßiger Schwund von Purkinjezellen — wahrscheinlich ebenfalls als Ausdruck einer Dysplasie — nachweisbar. In den dorsalen Wurzelganglien und in den lateralen corticospinalen Bahnen bestand eine unspezifische Degeneration. Ein jüngerer Bruder zeigt ein sehr ähnliches neurologisches Syndrom wie die verstorbene Schwester. Es wird daher die Annahme vertreten, daß das neurologische Syndrome genetisch determiniert ist. Ob auch bei dem Bruder ein Immundefekt vorhanden ist, ist vorerst noch nicht bekannt.
