    Notes: Near infrared spectroscopy (NIRS) is a new technique that permits noninvasive monitoring of cerebral blood and tissue oxygenation. Recently, we and others have shown that NIRS measurements are sensitive enough to follow changes in cerebral hemoglobin oxygenation due to activation of brain function. Based on these findings we have assessed the influence of aging as well as the influence of neurodegeneration on cerebral hemoglobin oxygenation during mental work. The typical NIRS pattern in young healthy subjects while performing calculation tasks measured in the frontal cortex were increases in oxygenated hemoglobin [HbO2] and total hemoglobin [HbT] while reduced hemoglobin [HbR] decreased. Elderly healthy subjects showed a significant lower mean increase in [HbO2] and [HbT] levels. Regression analysis revealed an age-dependent decline in activation-induced local increase of [HbO2] as well as [HbT]. Furthermore, we monitored changes in cerebral hemoglobin oxygenation in the frontal cortex while patients with probable Alzheimer's disease (AD) performed cognitive tasks. Whereas elderly healthy subjects (as well as patients with major depression, age-associated memory impairments or vascular dementia) again showed clear increases in the local concentrations of [HbO2] and [HbT] during brain activation, AD patients showed significant decreases compared to the baseline levels in both variables that were most pronounced in the parietal cortex. To clarify whether the different patterns in cerebral hemoglobin oxygenation during cognitive activation were due to an altered functional brain organization in AD or to alterations in the cerebrovascular response to neuronal activation, we are currently performing simultaneous NIRS and (015-H20-)PET measurements during performance of a cognitive task (Stroop test). Our finding of a regional reduced oxygen supply during activation of brain function may be of relevance to the development and the time course of neurodegeneration.

    Description / Table of Contents: Zusammenfassung Mammasarkome sind äußerst selten; sie machen etwa 0,3 bis 3% aller Brustdrüsenkrebse aus. Gemeinsames Merkmal sind die gut abgrenzbaren, indolenten, mit der Umgebung nicht verwachsenen Einzelknoten. Regionale Metastasen sind selten, wenn sie auftreten, bedeutet dies eine schlechte Prognose. Die Ansichten über die Therapie sind nicht einheitlich, empfohlen werden Tumorexstirpationen, Mastektomie, Ablatio und Radikaloperation mit Ausräumung der Axilla. Im eigenen Krankengut kamen in den letzten 10 Jahren 9 Mammasarkome zur Behandlung (8 Frauen und ein Mann). Stets wurde die Radikaloperation mit Ausräumung der Axilla und Nachbestrahlung durchgeführt. Die guten Behandlungsergebnisse scheinen dieses Vorgehen zu rechtfertigen.
    Notes: Summary The pathogenesis, clinic, treatment and prognosis of sarcoma of the mamma in brief are discussed. This report consists of nine cases of radically operated patients, who subsequently got x-ray irradiation. Five of 9 said patients survived the 5 year limit. This good result could be accreditted to our method of treatment of the sarcoma of the mamma.

Wir fanden: Eine Narkose ist nicht unbedingt nötig. Kleine Tumoren lassen sich destruieren. Stenosen bleiben stationär. Die Passage bleibt erhalten. Lokale Beschwerden (Tenesmen, Schmerzen) klingen ab. Größere Komplikationen wie Blutungen und Infektionen traten nicht auf. Da aber die Kryochirurgie rein lokal wirksam ist und keine sichere Radikalentfernung erzielt, kann these Methode nur als palliative Maßnahme angesehen werden.
    Notes: Summary Kryosurgery was performed on 45 patients with a rectal carcinoma. No deaths occurred. We found that anesthesia was not really necessary. Small tumors were destroyed. Stenoses remain stationar, and passages stay the same. Local complaints such as tenesmus and pain were diminished. Serious complications such as bleeding and infection did not occur. However, since kryosurgery is only a local treatment, and not radical removal, it is only palliative.

    Notes: Summary Blood coagulation (fibrinogen, thrombinantithrombin III complexes, TAT, and prothrombin fragment F1+2) and fibrinolytic parameters [fibrin split-product D-dimer, tissue plasminogen activator (t-PA) activity, plasminogen activator inhibitor-1 activity (PAI-1), and plasmin-antiplasmin-complexes (PAP)] were evaluated in 16 women on low estrogen (EE) oral contraceptive (OC) therapy. Blood samples were taken before and between days 18 and 22 of the first, third, and sixth treatment cycle. Fibrinogen levels were found significantly elevated during OC treatment compared with pretreatment values, while TAT and also F1+2 levels remained unchanged. Treatment-induced activation of fibrinolysis was documented by elevated D-dimer [pretreatment (pt): 172 ng/ml (range: 65–640 ng/ml), cycle 6 (c.6): 351 ng/ml (range: 93–960 ng/ ml),p〈0.05)] and PAP [(pt: 46.6 ng/ml (13–220 ng/ml), c.6: 66.4 ng/ml (21–200 ng/ml),p〈0.05] plasma levels. Among the fibrinolytic components a decrease in PAI-1 [pt: 10.8 ng/ml (2–56 ng/ml), c.6: 5.3 ng/ml (2.2–14.4 ng/ml),p〈0.05] and an increase in t-PA activity [pt: 0.23 U/ml (0.17–0.45 U/ml), c.6: 0.33 U/ml (0.2–0.9 U/ml),p〈0.05] were detected. Experiments with cultured human endothelial cells (EC) showed that EE influenced neither EC hemostatic regulatory activities (tissue factor, thrombomodulin) nor the secretion of the fibrinolytic components t-PA and PAI-1.

    Notes: Abstract Experimental observations of solitons in stimulated Raman scattering are reported. Soliton formation resulted from the introduction of aπ phase shift in the incident Stokes beam as predicted by theory. Pulse sharpening and retardation on propagation in the Raman medium have been observed along with amplitude diminution. The first two features were predicted and the third was not. Spontaneous soliton formation has been observed in the absense of any amplitude modulation or apparent phase shift in the optical fields, indicating that additional sets of initial conditions may result in soliton formation.
