    Description / Table of Contents: Zusammenfassung Hinsichtlich Indikation und Therapie hat sich auch die orthopädische Chirurgie an den Fortschritten und Neuerungen der Allgemeinchirurgie orientiert. Die Sofortversorgung beim Verlust einer Extremität, vor allem an den unteren Gliedmaßen, brachte einen entscheidenden therapeutischen Fortschritt. Die daraus resultierenden verkürzten Heilzeiten bedingen das sehr schnelle Wiedereintreten der Arbeitsfähigkeit der Patienten. Sehr viel weitergekommen ist man auch auf dem Gebiet der myoelektrischen Prothesen; hier sind jedoch noch weitere Entwicklungen im Gang. Darüber hinaus gewann die orthopädische Chirurgie auch entscheidende Aspekte durch die Osteosynthesetechnik, die nicht nur bei Frakturen, sondern auch bei wiederherstellenden und korrigierenden Eingriffen angewandt wird. Das große Gebiet der Arthroplastiken hat entscheidende Impulse erfahren und einen Entwicklungsstand erreicht, der es zuläßt, die Operationen an Hüft-, Knie- und Ellbogengelenk bei strenger Indikation als sicheren Eingriff zu bezeichnen. Wichtig für die Orthopädie war auch die Entwicklung der operativen Skoliosenbehandlung und hier besonders die innere Stabilisierung nach Harrington mit gewissen Modifikationen. Diese wenigen Beispiele zeigen, wie hervorragend sich auch die orthopädische Chirurgie entwickelt hat.
    Notes: Summary Concerning indication and therapy orthopaedic surgery also has been guided along by advances and innovations of general surgery. Immediate fitting after amputation, in particular of the lower extremities, represents a definite therapeutic improvement. As a result the shorter periods of reconvalescence have the patients regain their working capacity very rapidly. In addition much progress has been made in the field of myoelectric prosthetic devices, although further developments are in the process of taking place. Moreover, orthopaedic surgery has gained important features by the technique of modern osteosynthesis, which may be applied not only to fractures but also to reconstructive and corrective measures. The large field of arthroplasty has received decisive impulses and reached a level admitting to consider operations of the hip, knee and elbow as safe procedures, strict indications provided. Another important feature was the development of operative treatment of scoliosis, especially the technique of internal stabilization as initiated by Harrington with certain modifications. These few examples may demonstrate the outstanding evolution of orthopaedic surgery.

    Notes: Summary The most important therapeutic principles for the surgical treatment of malignant bone tumors are listed, and the technique and value of biopsy are described in detail. General surgical principles of excochleation, resection, amputation, and exarticulation are given. Reconstructive measures are touched on briefly, and delayed amputation is rejected. Treatment for metastatic disease is described. The most important malignant bone tumors are discussed, together with the most rational treatment for them, including cytostatic and radiation treatment.

Geeignet sind gestielte Sehnen aus der Pes anserinus-Gruppe oder ein freies Transplantat aus dem zentralen Drittel des Ligamentum patellae. Kombinierte, statisch-dynamische MaBnahmen am Kapselbandapparat sind bei rotatorischen Instabilitäten notwendig, so die Technik nach Nicholas bei antero-medialer, bzw. das Verfahren nach Maclntosh bei antero-lateraler Rotationsinstabilität, die empfohlen werden können.
    Notes: Summary The results of postoperative examinations of 152 late reconstructions of the anterior cruciate ligament using different methods are shown. The free ligamentum patellae transplant of the central third and the distally attached tendoplasty from the pes anserinus group show the best results. Rotational instabilities have to be dealt with according to the type of instability. The techniques of Nicholas for anteromedial rotational instability and of Maclntosh for anterolateral instability seem to be succeBful.
