    Notes: We report a patient who had raised serum and amniotic fluid alpha-fetoprotein levels at 16 weeks gestation. Ultrasonography revealed an apparently normal fetus and amniography displayed a soft tissue tumor in front of the fetus. The tumor turned out to be a placental haemangioma, when an apparently healthy girl weighing 2·88 kg was born at term.

    Notes: Intact IgG and fragments F(ab')2γ, Fabγ and Fcγ from a rabbit anti-protein-A serum (RapA) and corresponding preparations from normal rabbit IgG (NRG) were tested for their inhibitory effect on the binding of protein-A-reactive 125I-IgE and 125I-Fcγ, respectively, to protein-A–Sepharose. Intact IgG, F(ab')2γ and Fabγ of RapA inhibited the binding of protein-A-reactive 125I-IgE, whereas only intact IgG and Fcγ fragments from both RapA and NRG inhibited the binding of 125Fcγ to protein A-Sepharose. Further, the functional relationship between RapA and human polyclonal IgG was studied in a nephelometric test system. Intat IgG or fragments of IgG from human polyclonal IgG and rabbit anti-protein-A were found to affect the precipitation between human IgG and protein A in a similar way. Thus F(ab')2γ fragments and intact IgG enhanced the precipitation, whereas Fabγ and Fcγ fragments inhibited the precipitation. Protein A and an Fc-binding fragment of protein A (fragment B) were tested for their abilities to link different radiolabelled immunoglobulin preparations expressing the alternative and the classical protein-A reactivity to immobilized Fc fragments. All proteins expressing the alternative reactivity were efficiently bound both by fragment B and by protein A, indicating that fragment B, in addition to its classical Fc-binding activity, also expresses the alternative protein-A reactivity.

    Notes: ELISA (enzyme-linked immunosorbent assay) inhibition with a monoclonal antibody (mAb) (42B6) to Lepidoglyphus destructor was used to detect and quantify the storage-mite allergens in 30 dust samples collected from barns. Regarding the mite fauna, microscopic inspection of the barn dust and mite counts showed that L. destructor infested all 30 barns investigated (range 430–195400 mites/g dust). In 29/30 barns, L. destructor constituted more than 70% of the Astigmata species. Acarus siro was found in 26 samples, the highest value being 16 155 nites/g. No Dermatophagoides species were found. As to mites of the suborder of Prostigmata, species belonging to seven different families were detected. Besides the predominant L. destructor, allergens derived from other storage mites such as Glycyphagus domesticus, A. siro, and Tyrophagus putrescentiue have previously been assessed by this ELISA method. The correlation between number of mites and concentrations of mite antigen as measured by ELISA was assessed by linear regression (r2= 0.83). Thus, inhibition of mAb 42B6 in ELISA would seem to offer a simple and reliable method to detect levels of dust-mite species belonging to the Acaridae and Glycyphagidae families.

    Notes: Ornamental plants have long been used for indoor decoration. An example is the india rubber tree (Ficus elastica). With the increased popularity of green plants, both in private homes and public premises, small-leaf species, such as weeping fig or Ficus benjamina (Fb), have become widely used. Exposure to dust from Fb may cause sensitization and allergic airway symptoms, which among occupationally exposed plant keepers occur both in atopics and non-atopics. The serum reactivity to sap extracts from Fb and seven other indoor plants of the genus Ficus were investigated with RAST and a RAST inhibition technique, using sera from 12 atopic subjects and 12 plant keepers, sensitized to Fb. The allergenic similarity between the different extracts was found to be extensive. The specific IgE antibodies with the highest concentrations in serum were those against Fb and its variegate form, “star light”, with decreasing values for the other species, especially those with larger leaves. The binding of IgE antibodies to the other Ficus RAST discs could be completely inhibited by extract of Fb. These reactions were probably due to cross-reactivity. Sensitization is believed to occur by inhalation of allergen-enriched dust, emanating from the leaves of the plants. The high allergenic potency of the species with many small leaves may be due to their large total leaf area.

    Notes: Comparison was made of IgE and IgA levels in aspirated and gravity-collected cord blood (CB) from the umbilical vein and in capillary blood samples collected on the 4-5th day of life among 21 infants with atopic heredity. The IgA levels, but not the IgE levels, were significantly (p〈 0.001) lower at days 4-5 of life than at delivery. Further, there were significantly (p〈0.05) more infants with decreasing IgA levels (20/21; 95%) than IgE levels (9/15; 60% of those with detectable IgE, i.e., ≥ 0.125 kU/1). These observations, together with the highly significant correlation observed between IgE in aspirated CB samples and at 4–5 days of age, suggest active IgE synthesis during the prenatal and postnatal periods. Contamination of CB with maternal blood, defined as an increased CBIgA level (≥ 14.1 μg/ml), was found in 3 (14%) CB samples, all of which were gravity-collected. Of 4 CB samples (gravity-collected) with elevated IgE (i.e., ≥ 0.9kU/l), 2 had suspected maternal contamination. Therefore, aspiration of CB or capillary collection at 4-5 days of age should be preferred for allergy prediction. If gravity collection is used, contamination should be investigated by determining IgA in all CB samples with IgE concentrations exceeding the cut-off point.

    Notes: Bronchial provocation studies with L. destructor (L. des.) extract were made on 12 farmers with asthma who were exposed daily to stored hay and grain and had positive radio-allergo-sorbent test (RAST) to L. des. All 12 farmers developed more than a 20 % fall in FEV1 to the extract. None of the symptomatic farmers who were also RAST positive to the L. des. culture medium developed an asthmatic reaction to the medium extract. Neither four healthy farmers nor two patients with asthma caused by D. pteronyssinus (D. pt.) responded to L. des. or the L. des. culture medium. Challenges with D. pt. extract were negative in two symptomatic farmers who were RAST positive to L. des. and negative to D. pt. The present study further supports our previous hypothesis that there is an IgE-mediated immunological mechanism behind L. des. -induced asthma in farmers and that atopic farmers are liable to develop occupational asthma caused by allergy to L. des..

    Notes: Background:  Reactions after a blood transfusion could be allergic because of passive transfer of immunoglobulin E (IgE) antibodies from allergic donors.Aims of the study:  To compare spectrum and prevalence of IgE antibodies in blood donors from Sweden and Norway.Methods:  Using the ImmunoCAP method, serum samples from 1002 blood donors from Sweden and 500 from Norway were analysed for IgE antibodies to common inhalant and food allergens and allergens common in a hospital environment, such as penicilloyl G and latex.Results:  As many as 23.6–27.3% of the donors had IgE antibodies to at least one of the 14 allergens tested. Of these 6.8–16.7% had extremely high concentrations, i.e. 〉35 kUA/l corresponding to 100 times the cut-off for a positive allergy test. Most donors were sensitized to pollens, dander and mite but several had very high levels of IgE antibodies to penicilloyl G, latex and peanut. The pattern of sensitizing allergens differed between Sweden and Norway.Conclusions:  High serum levels of IgE antibodies to various allergens are common among blood donors and the degree of sensitization and spectrum of involved allergen varies between geographical regions. Present routines to identify IgE sensitized, potential risk, donors are not satisfactory; the sensitivity of selection procedures is about 25%.

Allergy to the ornamental indoor green plant Tradescantia ‘Albifloxia’.We report on a 32-year-old atopic female office employee with a moderate tree pollinosis who also suffered from indoor-related perennial rhinoconjunctivitis. Once when she repotted her two ornamental nonflowering green plants of the genus Tradescantia‘synonym: Albifloxia; family Commelinaceae’, she immediately experienced itching of the face, throat, and conjunctiva; swelling of the lips; and dyspnea and wheezing. Skin prick tests with the leaves of Tradescantia ‘T. albifloxia and T. fluminensis’‘Ta and Tf’ were strongly positive as was the specific IgE to Ta leaves extract. On RAST inhibition studies, no cross-reactivity was found between Ta and Ficus benjamina‘weeping fig’, a nonflowering green plant, which produces, in its milky sap, an important respiratory allergen. Green plants should be considered potential indoor allergens and tested in plant-keepers referred for allergologic investigation.

    Notes: Anaphylactic reactions after intravascular exposure to natural rubber latex (NRL) have been reported. Thus, there is an urgent need to produce medical devices with the lowest possible latex-allergen content. The latex-allergen concentration in extracts prepared from 92 lots of medical catheter (MC) balloons, manufactured by Nolato Polymer AB, Torekov, Sweden, from April 1993 to March 1996, was measured with an EAI (IgE antibody inhibition) assay. Inhibitory capacity was expressed in arbitrary units/ml (U/ml) in relation to reference NRL sap, given an arbitrary value of 1000 U. Extracts from randomly selected lots were measured for protein by the modified Lowry method. Water leaching, chlorination, and treatment with savinase were used experimentally to study reduction of the latex-allergen content. The latex-allergen content in extract from the regular MC balloons varied from 0.1 to 2.9 U/ml. All the methods used to reduce the allergen content were effective, and increased leaching stabilized the allergen content at a low level. The protein concentration of the extracts varied between 9 and 100 mg/1. No correlation was found between protein and allergen content. As a result of this study, the manufacturer has extended the stage of water leaching in the production process. This study shows that cooperation between immunologists and manufacturers may result in product development and improvement.

    Notes: Recombinant protein A(SpA) produced in Escherichia coli or Bacillus subtilis bacteria did not induce activation of human peripheral mononuclear cells, whereas SpA preparations obtained from naturally occurring Staphylococcus aureus bacteria as well as recombinant SpA from Staphylococcus xylosus were potent mitogens. Further purification of SpA from S. aureus showed that the mitogenic material was concentrated in the side fractions containing more basic molecules. Some staphylococcal enterotoxins are mitogenic for human cells and in order to test whether contaminating enterotoxins would be responsible for the mitogenic effect of SpA preparations, rabbit antibodies were produced against enterotoxin A and B. These antibodies inhibited activation of human cells induced by the enterotoxins used for immunization but did not affect the activation induced by SpA preparation. The addition of selected human sera to in vitro cultures resulted in an inhibition of the response induced by low doses of SpA. There was no clear relationship between these effects and the content of IgG antibodies against staphylococcal enterotoxins A, B, and C1 in the sera. Thus, we conclude that the mitogenicity of SpA preparations is caused by contaminating molecules, probably not enterotoxins A, B, or C1.
