    Notes: The participation of adhesion molecules in systemic vascular injuries of the generalized Shwartzman reaction was studied. The generalized Shwartzman reaction was induced in mice by two consecutive injections of lipopolysaccharide. Intercellular adhesion molecule-1 (ICAM-1) was expressed on vascular endothelial cells, renal tubular cells and alveolar wall in generalized Shwartzman reaction-induced mice. The preparative injection of lipopolysaccharides induced ICAM-1 expression in those cells, and the provocative injection of lipopolysaccharides for the generalized Shwartzman reaction augmented it further. The simultaneous administration of anti-gamma interferon antibody with the preparative injection of lipopolysaccharides completely inhibited ICAM-1 expression on vascular endothelial cells. The injection of recombinant gamma interferon in replacement of lipopolysaccharides resulted in ICAM-1 expression. The administration of anti-ICAM-1 antibody together with the provocative injection of lipopolysaccharides significantly blocked the apoptosis of vascular endothelial cells in generalized Shwartzman reaction-induced mice. It was suggested that ICAM-1 expression on vascular endothelial cells might be involved in systemic vascular injuries of the generalized Shwartzman reaction, and that it might be regulated by gamma interferon.

Nous rapportons l’action de l’injection épidurale caudale préopératoire d’un mélange de bupivacaine et de buprénorphine appliquée sous anesthésie générale pour diminuer ls douleurs après fusion lombaire inter-somatique ou laminotomie pour sténose. Les douleurs ont été comparées chez les groupes avec ou sans bloc épidural: 24 et 23 patients respectivement ayant subi une arthrodèse lombaire et 30 et 28 patients respectivement ayant subi une laminotomie. Dans les 12 premières heures postopératoires, le groupe avec blocage épidural a nécessité moins souvent des analgésies additionnelles (suppositoire de diclofénac ou pentazocine intra-musculaire) et le score de l’échelle analogique visuelle était meilleur que dans le groupe sans injection épidurale. Dans les secondes 24 h et plus tard, les patients qui avaient une chute tensionnelle de plus de 10 mm de mercure après l’injection épidurale ont eu moins besoin d’analgésie additionnelle que ceux sans bloc épidural et cela pour les deux types d’intervention; d’autre part, le score analogique visuel a été meilleur que dans le groupe sans bloc pour l’intervention de laminotomie mais pas pour l’intervention de fusion inter-somatique. Il n’y a jamais eu de blocage moteur post-opératoire. Cette étude prouve que l’injection épidurale est une méthode simple pour diminuer les douleurs. Chez les patients qui ont eu une baisse de tension artérielle par blocage sympathique, l’effet s’est poursuivi même après que l’action analgésique avait disparu.
    Notes: Summary. We report the effect of pre-incisional caudal epidural injection of a mixture of 20 ml 0.25% bupivacaine and 0.1 mg buprenorphine performed under general anaesthesia for relieving wound pain after posterior interbody fusion and laminotomy for spinal stenosis. Pain was compared in groups with and without blockade: 24 and 23 patients with fusion, and 30 and 28 with laminotomy respectively. In the first 12 h after each operation, the group with blockade required additional analgesics less frequently and had a lower visual analogue scale score than those without blockade. In, and after, the second 24 h patients with hypotension of 10 mm Hg, or more, after the blockade required additional analgesics less frequently than those without blockade for both operations and had a lower visual analogue score than those without blockade for laminotomy, but not for fusion. This is a simple procedure for relieving wound pain and in patients with hypotension caused by the block, the effect continued even after the analgesic action had disappeared.

    Notes: Abstract The cerebellum has anatomical connections to the cerebral cortex, through which it can affect language function. To study these connections, we investigated patients with chronic Broca's aphasia using MRI and single-photon emission computed tomography (SPECT). We selected 15 such patients (9 male, 6 female, aged 17–64 years, mean age 56 years) from 30 chronically aphasic patients. Using the results of SPECT, we divided them into patients with (group 1) and without (group 2) crossed cerebellar diaschisis (CCD). We compared the language function of the two groups. Patients in group 1 showed classical Broca's aphasia, while patients in group 2 showed mainly word-finding difficulty. Patients with CCD hat infarcts involving the lower part of the frontal gyrus but patients without CCD did not, which suggests that this region may have functional and anatomical connections with the cerebellum. Our findings support the notion that the cerebellum contributes to language.

    Notes: Abstract Primary progressive aphasia (PPA) presents with aphasia, with or without other minor cognitive dysfunction. We report five patients with PPA to show the correlation between their clinical signs and imaging findings. The patients can be divided into those with nonfluent (group 1) and those with fluent (group 2) aphasia. The characteristic speech impairment was bradylalia in group 1 and word amnesia in group 2. Impairment of comprehension was common but mild in both groups. On MRI, patients in group 1 showed predominantly left frontal and perisylvian atrophy with reduced uptake in the same region on single photon emission computed tomography (SPECT) using technetium-99m hexamethyl propyleneamine oxime (99mTc HMPAO). Patients in group 2 showed left temporal atrophy involving the superior, middle and inferior temporal gyri, hippocampus and parahippocampal gyrus on MRI and reduced uptake in the same region on SPECT. These findings correlated well with the functional anatomy of speech impairment.

    Notes: Abstract We parametrize the space of isometric immersions of the hyperbolic n -space into the hyperbolic ( n +1)-space by a family of properly chosen (at most) countable n -tuples of real-valued functions. This is an answer to the problem posed by Nomizu in 1973. We also construct a one-parameter family of deformable isometric immersions from the quotient spaces of the hyperbolic 2-plane modulo and some totally discontinuous subgroups of isometries.

    Notes: Poly(methacrylic acid) (PMA) and poly(acrylamide) (PAAm) with a small mole fraction of naphthol (NpOH) residues were prepared by polymerization of methacrylic acid or acrylamide with three position isomers, methacryloylaminonaphthol. The substituent effect on the apparent pK values (pKobsd), determined by spectroscopic pH titrations in aqueous solution, was observed for the NpOH-labeled PAAm. The electrostatic surface potential (ψ) for PMA was calculated from the difference in the pKobsd values for the labeled PMA and PAAm. The obtained ψ value decreased as the spacing between the OH moiety and the polymer main chain increased. This clearly indicates that the ψ value is a sensitive function of the distance from the polyion surface.
