    Notes: Inhibitor development is a serious complication in patients with haemophilia A and B. Historically, a lack of optimal therapies and factor products for treating inhibitor patients resulted in many patients developing chronic haemophilic arthropathies and flexion contractures of the involved joints. The introduction of immune-tolerance protocols to eradicate high-titre inhibitors has greatly diminished the incidence of these types of complications but as in the case reported here, immune tolerance is not always successful. Various elective surgical procedures were often delayed or not even considered in patients with inhibitor because of the variability in achieving adequate haemostasis and the thrombotic risks involved with the use of activated prothrombin-complex concentrates (APCCs) over extended periods of time. The development of recombinant factor VIIa (rFVIIa; NovoSevenTM) and its demonstrated safety and efficacy in treating inhibitor patients has opened new possibilities for addressing severe arthropathy with flexion contracture. This case report demonstrates that the use of rFVIIa in such a situation must include dosing flexibility that is both patient-specific and related to the potential for bleeding; the ability to maintain clinical haemostasis with a prophylactic dose of rFVIIa given as little as once daily; and the capacity for higher doses of rFVIIa, particularly in children because their kinetic profiles differ from adults.

    Notes: Clinical implications of telomerase detection In 1994 a sensitive method for the detection of telomerase was described. This assay, which was based on the polymerase chain reaction, suggested that telomerase activity was associated with immortal and cancer cells. Since then more than a thousand studies have documented the expression and activity of the enzyme in diseased tissues, primarily tumours. This review gives an overview of the biological significance of telomerase expression and methods for detecting its activity. This is followed by an organ system-based discussion of expression in normal tissues and disease states. We finish with speculation as to the future role of telomerase detection in diagnostic histopathology.

    Notes: This paper provides a review and summary of strain comparison and selective breeding work carried out on redclaw crayfish Cherax quadricarinatus (von Martens) at the Queensland Department of Primary Industries, Freshwater Fisheries and Aquaculture Centre, Walkamin. Redclaw crayfish are a highly marketable, environmentally tolerant, freshwater crayfish with a moderately fast, but variable, growth rate. Five strains of the species were assessed morphologically and in terms of age at maturity. A non-replicated production trial was also carried out before two strains, from the Gilbert and Flinders rivers in North Queensland, were chosen to be part of an experimental selective breeding programme for improved growth. Initially, 14 families of each strain were randomly mated and grown out. At this point, a size-related selection took place, with the largest animals from each family forming a selected line, and individuals from around the mean forming a control line. Within-family selection and reciprocal mating between families was performed. Data were collected after each of two generations of growout and analysed by analysis of variance. Significant differences (P 〈 0.05) were measured between strains, sexes and between selected and control lines. Selected individuals grew 9.5% faster than the controls. Recommendations for selective breeding research involving freshwater crayfish are presented.

    Notes: Ta2O5 is a candidate for use in metal–oxide–metal (MOM) capacitors in several areas of silicon device technology. Understanding and controlling leakage current is critical for successful implementation of this material. We have studied thermal and photoconductive charge transport processes in Ta2O5 MOM capacitors fabricated by anodization, reactive sputtering, and chemical vapor deposition. We find that the results from each of these three methods are similar if one compares films that have the same thickness and electrodes. Two types of leakage current are identified: (a) a transient current that charges the bulk states of the films and (b) a steady state activated process involving electron transport via a defect band. The transient process involves either tunneling conductivity into states near the Fermi energy or ion motion. The steady state process, seen most commonly in films 〈300 Å thick, is dominated by a large number of defects, ∼1019–1020 cm−3, located near the metal–oxide interfaces. The interior of thick Ta2O5 films has a substantially reduced number of defects. Modest heating (300–400 °C) of Ta2O5 in contact with a reactive metal electrode such as Al, Ti, or Ta results in interfacial reactions and the diffusion of defects across the thickness of the film. These experiments show that successful integration of Ta2O5 into semiconductor processing requires a better understanding of the impact of defects on the electrical characteristics and a better control of the metal–Ta2O5 interface. © 2000 American Institute of Physics.

    Notes: This paper describes an empirical study, conducted in three acute hospitals, of the attitudes of central managers, medical managers and clinicians towards the adoption of benchmarking. Benchmarking was portrayed in The New NHS White Paper (1997) as an important means of improving efficiency over the next decade. The present paper examines the context of change and nature of benchmarking. Findings are presented in seven sections including: the understanding which respondents had of benchmarking; their willingness to be involved in benchmarking; the existence of strategies and policies for implementing benchmarking; the relevance of existing costing information; and the role of networks in facilitating benchmarking. The study concludes that the process of change adopted contradicted most of the factors associated with creating receptivity to change. Also, that the publication of the National Reference Costs seemed to have more relevance to resource planning at central National Health Service Management Executive level, than to effecting improvements at operational level in acute hospitals.

    Notes: During raspberry (Rubus ideaus L. cv Glen Clova) fruit ripening, endo-β-1,4-glucanase (EGase; EC 3.2.1.4) specific activity (per g fresh weight) increases approximately 15-fold. Highest activity was associated with the surfaces of the receptacles where the weakening abscission zones are located. Immunoblotting using antibodies raised against a bean abscission-EGase (ab.-EGase) identified a single protein of Mr 52 kDa that was present only in ripe fruit and was most prominent in the receptacle. Using reverse transcription-polymerase chain reaction (RT-PCR), two 497-bp partial putative EGase clones were obtained from ripe receptacle mRNA (termed RI-EGL1 and 2) which share 53% amino acid identity. The more abundant RI-EGL1 was used to obtain its full length clone from a ripe receptacle cDNA library. The deduced amino acid sequence of RI-EGL1 was similar to other ab.-EGase sequences (ca 67%) and contained the conserved motifs present in all E2-class EGases. Northern analysis revealed that RI-EGL1 expression was limited to ripe-fruit receptacles. Application of the ethylene antagonist 1-methylcyclopropene (1-MCP) to green fruits indicated that endogenous ethylene accelerates raspberry abscission and increases both EGase activity and RI-EGL1 expression.
