Notes: Abstract To assess the ability of gadolinium enhancement to suppress the overlapping vessel signals that diminish the image quality of magnetic resonance cholangiopancreatography (MRCP), we obtained MRCP before and after intravenous injection of gadolinium. Signal intensity ratio of the common bile duct to the main pancreatic duct was significantly improved after the injection of contrast media. The image quality of the postcontrast MRCP was better than that of the precontrast one.

Notes: Masticatory function was analysed before and after orthodontic treatment in two cases where tooth alignment remained unusual after treatment. The Sirognathograph Analyzing System® was used to analyse the masticatory function. In both cases, the right maxillary first premolar was located where the maxillary canine is normally positioned. The results of orthodontic treatment were satisfactory both morphologically and aesthetically, and masticatory function was greatly improved. However, some problems remained in the mandibular movement due to the abnormal contact between the mandibular teeth and the maxillary first premolar. These findings support the need for examination of masticatory function when treating patients with an unusual tooth alignment even if the results of orthodontic treatment are both morphologically and aesthetically successful.

Notes: Summary Background Netherton's syndrome (NS) is an autosomal recessive disorder characterized by trichorrhexis invaginata (‘bamboo hair’), congenital ichthyosiform erythroderma and an atopic diathesis. NS has recently been shown to be due to a defect in the SPINK5 gene, encoding LEKTI, a 15-domain serine protease inhibitor. SPINK5 maps to chromosome 5q31–q32, and has been suggested to be a locus predisposing to atopy in general. Recently, coding polymorphisms in SPINK5 exons 13 and 14 have been reported to be associated with atopy, asthma and atopic dermatitis (AD). Objectives To examine whether these polymorphisms are also associated with AD in Japan. Methods We characterized eight polymorphisms in SPINK5 exons 13 and 14 in 124 Japanese patients with AD and 110 healthy controls. The polymorphisms we examined were IVS12–26C→T, IVS12–10A→G, 1103A→G (Asn368Ser, in exon 13), 1156G→A (Asp386Asn, in exon 13), 1188T→C (His396His, in exon 13), IVS13–50G→A, 1258G→A (Glu420Lys, in exon 14) and IVS14+19G→A. Results We found significant associations between seven of these polymorphisms and AD in Japanese patients. Conclusions This study confirms the previous suggestion of an association between SPINK5 and AD.

Notes: A 50-year-old woman with von Recklinghausen’s disease, but not Carney’s complex, presented with a 1-year history of a hard subcutaneous mass on her right hip and right inguinal lymphadenopathy. Histological and immunohistochemical studies of the tumour revealed schwannian and melanocytic characteristics. Local recurrence without distant metastases was observed 5 years later. Although the diagnosis of malignant schwannoma with melanocytic differentiation, rather than neurotropic melanoma, was made for the primary tumour, based on the clinicohistopathological and ultrastructural findings, the overall clinical course in this case did not seem incompatible with malignant melanocytic schwannoma.

Notes: Effects of the resistant cultivars of melon on the powdery mildew fungus, Sphaerotheca fuliginea, were investigated in a melon-growing area of Japan. Powdery mildew fungi were isolated from four melon cultivars (‘HN21’, ‘Quincy’, ‘Earl's Knight Natsu 2’ and ‘Earl's Miyabi Natsu 2′) which are resistant to different races of S. fuliginea.‘HN21’is resistant to races 1 and N1,‘Quincy’ and ‘Earl's Knight Natsu 2’are resistant to races 1 and N2, and ‘Earl's Miyabi Natsu 2’is resistant to races 1, N1 and N2. All 155 strains isolated from ‘HN21’were identified as race N2. Seventy out of 81 strains isolated from ‘Quincy’ and ‘Earl's Knight Natsu 2’were identified as race N1, while other strains were identified as races 1, 2US, N2 and N3. Nineteen out of 29 strains isolated from ‘Earl's Miyabi Natsu 2’were identified as race N3, while other strains were identified as races 10, 2US, N1 and N4. These results suggest that, although the resistant cultivars are extremely effective for protecting the plants from their respective races, a race succession rapidly takes place, even in a small area, and other minor races become predominant.

Notes: Several evaluation methods for soybean chilling tolerance at the reproductive stage were examined under artificial climatic conditions. Comparisons were made on plants in control and treated plots using three cultivars differing in the level of chilling tolerance. In all methods, plants were grown at 22/17°C (day/night) until first flowering, and then transferred to growth chambers at 24/17°C for control and 15/15°C for chilling treatment, respectively. A method, in which plants were grown at 20/16°C after 4 weeks of the different temperature treatments, proved comparable to the conventional one, in which chilling tolerance at the flowering stage is evaluated using natural and artificial conditions. Another method, in which the plants in chilling treatment plots were grown at 15°C until maturity, also proved usable to evaluate genotypic differences in chilling tolerance independently of maturity time.

Notes: A 65-year-old Japanese woman presented with disseminated erythematous patches, plaques, and nodules on the trunk and limbs. Histological examination showed diffuse and dense infiltrates located in the dermis and subcutis, composed of large pleomorphic T lymphocytes. Immunohistochemically, neoplastic cells were positive for blastic T-cell markers, but negative for CD30 (Ki-1) antigen. Based on the clinicopathological findings, a diagnosis of primary cutaneous large T-cell lymphoma was made. Despite systemic chemotherapy, the patient died 7 months after diagnosis. Gene expression profiling using complementary DNA microarrays indicated significantly increased expression of an apoptosis-inhibitory protein and certain cyokines and cytokine receptors (e.g. MCP-1, MCP-2, IP-10, and IL-2Rγ) in the tumour-indurated skin. Comprehensive gene expression patterning in additional cases may provide useful information regarding the biological and clinical behaviour of aggressive cutaneous lymphomas such as CD30-negative large T-cell lymphoma.

Notes: A 31-year-old male presented with limpet-like, cone-shaped skin lesions on the scalp, the extremities, and the trunk of 4 months' duration, and had had severe joint pain in his right fingers, wrist, and knee for 1 month. Radiological examination revealed arthritis of the above mentioned multiple joints. Rheumatoid factor was serologically negative. Histopathological findings showed dense inflammatory cell infiltration and remarkable Munro's microabscesses in the horny layer in addition to psoriasiform epidermal hyperplasia, and predominant dermal oedema. Based on clinicopathological findings, a diagnosis of rupioid psoriasis with arthropathy was made. Following treatment for arthralgia using low doses of systemic steroid, the effects of cyclosporin combined with topical steroids was seen to alleviate dramatically the skin lesions and arthritis within 2 weeks. There was no recurrence of such skin and joint lesions during a follow-up period of 1 year.
