    Notes: Summary Between 1981 and 1986 cytogenetic studies of bone marrow and/or blood cells in 139 patients with de novo acute myeloid leukemia (AML) were performed. The overall incidence of chromosomal aberrations was 53%, and this was not significantly influenced by sex, age nor the FAB classification. The aberrations most often found were: complex anomalies (n=13), t(8; 21) (n=10), trisomy 8 (n=9), monosomy 7 (n=6), monosomy 5, 5q-, trisomy 11, 12p- (n=4) and trisomy 6, 11q-, inv [16] (n=3). The prognostic significance of chromosomal findings was evaluated in 112 patients treated by combination chemotherapy. The chromosomal status NN, AN, AA did neither significantly influence complete remission rate (NN: 68%, AN: 71%, AA: 60%) nor mean survival (NN: 24, AN: 26.6, AA: 35.6 months). On the other hand, certain types of chromosomal anomalies were of prognostic value. CR was obtained in all 10 patients with t(8; 21) but only in 2 out of 9 patients with complex aberrations. Median duration of CR in patients with t(8; 21) was significantly longer than in patients with a normal karyotype (30 vs 7 months).

    Notes: Abstract A case of acute nonlymphocytic leukemia (ANLL) occurring 2 years after the diagnosis of multiple myeloma (MM) that had been treated by only one course of melphalan/prednisone chemotherapy is reported. Cytogenetic and fluorescence in situ hybridization analysis of peripheral blood cells revealed trisomy 8 as the sole cytogenetic defect at the time of diagnosis of ANLL. Two years earlier, when MM was diagnosed without any cytological evidence of co-existent myelodysplasia, chromosomal analysis of bone marrow cells showed the same pathological karyotype 47, XY, +8 in 14 of 20 mitoses studied. Our interpretation of this unusual cytogenetic finding is that at the time of diagnosis of MM, in spite of lacking cytological signs of myelodysplasia, an unrecognizable myelodysplastic syndrome must have been present which then evolved to ANLL.

    Notes: Abstract  We report the case of a 47-year-old patient who developed acute myelogenous leukemia (AML) 18 months after receiving high-dose chemotherapy with peripheral blood progenitor cell support (PBPCT) for relapsed low-grade follicular non-Hodgkin's lymphoma (NHL). Cytogenetic analysis of the leukemic cells showed the translocation (9;22)(q34;q11). In three mitoses, an addiditional Philadelphia chromosome (Ph1) was present. In the literature, Philadelphia chromosome-positive secondary AML has been described only once before in a patient with multiple myeloma.

    Description / Table of Contents: Contents To calculate magnetic fields with arbitrary material distributions and excitations the given vector potential differential equation is transformed into an algebraic system of equations (Finite Element Method). The application of the ampere turns rule leads (considerably more simply) to identical equations. Allowance for non-linearities is provided by the Newton-Raphson method. The transformation matrices are given for anisotropic material properties. Finally, a solver for sparsely populated matrices is deseribed.
    Notes: Übersicht Zur Berechnung von Magnetfeldern mit beliebigen Materialverteilungen und beliebigen Anregungen wird die Differentialgleichung für das Vektorpotential angegeben und mit Hilfe der finiten Elemente Methode in ein algebraisches Gleichungssystem umgewandelt. Die Anwendung des Durchflutungsgesetzes führt wesentlich einfacher zu identischen Bestimmungsgleichungen. Eine Berücksichtigung der Nichtlinearitäten geschieht mit dem Newton Raphson Verfahren. Für den Sonderfall von anisotropen Stoffeigenschaften werden die Transformationsmatrizen angegeben. Zum Schluß wird ein Lösungsverfahren für schwach besetzte Gleichungssysteme beschrieben.

    Description / Table of Contents: Contents For synchronous machines based on permanent magnet excitation and the transvers flux concept an analytical approach for field and force calculations is being described. One of the assumptions is a known trapezoidal wave form of the armature current, maintained by appropriate frequency inverter and terminal voltage. The analysis covers one and two-sided armature configurations as well as different configurations of the excitation (magnet) system of the rotor. Refinements of the analysis can be achieved by taking into account magnetic leakage components and saturation effects. Comparisons with the 3-dimensional FE-computation and with results from measurements are presented.
    Notes: Übersicht Ausgehend von der Transversalfluß-Anordnung des magnetischen Kreises wird für permanentmagneterregte Synchronmaschinen die Berechnungsmethode beschrieben. Hierbei ist vorausgesetzt, daß durch Wechselrichterspeisung die Stromform näherungsweise trapezförmig vorgegeben ist. Für ein- und zweiseitige Statoranordnungen sowie Erregersysteme in Flachmagnet- und Sammlerkonfiguration werden die mathematischen Beziehungen zwischen Feld- und Stromgrößen und den Abmessungsparametern angegeben. Es wird auf verschiedene Verfeinerungsstufen des Berechnungsverfahrens, etwa durch Berücksichtigung von Streuflußkomponenten und Sättigung des Eisenwegs, hingewiesen. Vergleiche mit der dreidimensionalen FE-Methode und mit Messungen an einem Modellmotor beschließen die Arbeit.

    Description / Table of Contents: Übersicht Eine Methode zur Simulation von Permanentmagneten für eine analytische Behandlung mit der Transfer-Matrix-Methode wird dargestellt. Die Methode ist dafür geeignet, analytische Ausdrücke von magnetischen Feldern und elektromagnetischen Kräften in elektrischen Maschinen mit Permanentmagneterregung abzuleiten. Die Methode wurde auf die Analyse eines permanentmagneterregten synchronen Langstator-Linearmotors angewandt. Messungen der Kräfte an einem synchronen Linearmotor mit 150 Seltenerden-Kobalt-Erregermagneten wurden herangezogen, um das in dieser Arbeit dargestellte analytische Berechnungsverfahren zu überprüfen. Beim Vergleich mit den experimentell ermittelten Werten zeigt sich eine gute Übereinstimmung.
    Notes: Contents A method of simulating permanent magnets in analytical approach for the transfer-matrix-method is given. The method is suited for deriving analytical expressions of the magnetic fields and electromagnetic forces in electrical machines containing permanent magnets. The method was applied to the analysis of a long-stator linear synchronous motor with permanent magnet excitation. Measurements of the forces in the 150-pole Samarium-Cobalt type linear synchronous motor for electric propulsion were used to verify this permanent magnet modeling analysis. Comparison with experimentally obtained results shows a very good agreement.

    Notes: Abstract Since 1988, treatment strategies for our sarcoma patients have been determined by the same team and operations performed by one surgeon. The aim of this study was to analyse prognostic data on local recurrence and survival of 101 consecutive patients who presented in our institution with the primary tumour manifestation. After a median follow-up of 35 months, the local recurrence rate was 13.5%, the mean survival time was 68 months and the 5-year survival rate was 83%. Besides positive lymph nodes (only 3 patients) the quality of resection significantly influenced local recurrences (P〈0.05). Univariate predictors of mortality were tumour grade (P〈0.01), tumour size (P〈0.05), distant metastases (P〈0.01), and resection quality (P〈0.01). Multivariate predictors of mortality consisted of grade (P〈0.0001), positive lymph nodes (P〈0.001) and resection quality (P〈0.01). In this homogeneous group of patients, excellent recurrence and survival rates could be achieved. An optimized surgical treatment not only reduces the rate of local recurrences but also augments survival time.

    Notes: Abstract Between early 1992 and December 1994, laparoscopic splenectomy was performed in 27 patients with idiopathic thrombocytopenia (ITP), hairy-cell leucemia, HIV, or Hodgkin's disease. In all cases medical treatment, especially cortisone therapy, failed. In Hodgkin's disease the splenectomy was combined with liver biopsies and dissection of parailiacal, paraaortic, and mesenteric lymph nodes for abdominal staging. The operation was performed using four trocars; the splenic vessels were divided by a linear stapler. In general the spleen was removed in a bag through a slightly enlarged trocar incision or after morcellation. Three patients needed a small laparotomy for the removal (laparoscopic assisted). In a recent case of Hodgkin's disease the intact spleen was removed via posterior colpotomy. In 22 of 27 cases (81%) the operation was finished laparoscopically. Five times a conversion to conventional laparotomy was necessary because of bleeding of enlarged lymph nodes at the hilum. Wound infections occurred in two cases. In one patient with ITP the platelet count did not improve and continuous blood loss led to relaparotomy at the 1st postoperative day. No surgical bleeding was found. All patients tolerated a fluid diet at the 1st postoperative day and hospitalization time was 4.4 days (range 3–14). Regarding the low complication rate and the advantages of a smaller abdominal trauma in the postoperative period, the laparoscopic approach for elective splenectomy and laparoscopic abdominal staging has a substantial benefit for the patients.
