    Notes: Several halogenated anaesthetic agents have been associated with hepatotoxicity. We report a case of fulminant, fatal hepatic necrosis after uneventful isoflurane anaesthesia in a patient without previous liver disease, who may have been sensitised by previous exposure to enflurane. Although no anti-trifluoroacetyl antibodies could be detected in the patient's serum, isoflurane hepatotoxicity seems very likely to be the reason for fulminant hepatic failure in this patient.

    Description / Table of Contents: Zusammenfassung. Von 63 Patienten, welche eine tiefe anteriore Rectumresektion erhielten, wurden 39 mit einer direkten coloanalen Anastomose (CAA), 24 unter Zwischenschaltung eine Colon-J-Pouches als colonpouchanale Anastomose (CPA) rekonstruiert. Lokalseptische Komplikationen traten in 20,5 % aller Patienten nach CAA gegenüber 12,5 % nach CPA auf, die Stuhlfrequenz war nach zusätzlicher Pouchanlage 3,3 pro 24 h verglichen mit 5,2 pro 24 Std nach direkter coloanaler Anastomose (p = 0,053). Die postoperative Kontinenz war in der Pouchgruppe tendentiell besser (n. s.), die Analmanometrie zeigte lediglich einen signifikanten postoperativen Abfall des Ruhedruckes nach direkter coloanaler Anastomose. Die Ergebnisse sprechen für die Anlage eines primären Colonpouches, da die lokalseptische Komplikationsrate nach colonpouchanaler Anastomose niedriger und das funktionelle Ergebnis tendentiell günstiger ist.
    Notes: Summary. Thirty-nine of 63 patients undergoing deep anterior rectal resection received a straight coloanal anastomosis (CAA); the remaining 24 patients additionally had a colon-j-pouch (CPA) constructed. After pouch-anal anastomosis, local septic complications occurred in 12.5 % of patients compared to 20.5 % after coloanal anastomosis. Stool frequency after pouch-anal anastomosis was 3.3 per 24 h compared to 5.2 per 24 h after straight anastomosis within the first year after ileostomy closure (P = 0.053). Continence was slightly better in the pouch group (n. s.), and anal manometry showed a significant postoperative decrease only in resting pressure after straight coloanal anastomosis (P 〈 0.001). This study supports the construction of a colon-j-pouch after deep rectal resection, as the pouch-anal anastomosis has fewer local septic complications and seems to improve functional outcome.

    Description / Table of Contents: Zusammenfassung. Wir berichten über den Fall einer subacromialen Schulterluxation, die durch die Kombination von einer cranialen Glenoidfraktur und einer Acromionfraktur möglich wurde. Der Patient hatte sich diese ungewöhnliche Kombinationsverletzung im Rahmen eines Handballspiels zugezogen. Erst die weiterführende Diagnostik mit einem CT der Schulter zeigte das ganze Ausmaß der Verletzung. Es erfolgte die osteosynthetische Versorgung der Glenoidfraktur nach Reposition der Schulter. Das postoperative Ergebnis zeigte eine gute anatomische Rekonstruktion der glenoidalen Gelenkfläche. Der Patient hat nach Abschluß unserer Behandlung eine gute funktionelle Wiederherstellung der Beweglichkeit im Schultergelenk mit Abduktion bis 180 ° und ist wieder voll berufstätig.
    Notes: Summary. We report on a case of subacromial shoulder dislocation, which resulted from a combination of a cranial glenoid fracture and an acromial fracture. The patient sustained this rare injury while playing handball. The whole implication of the injury was revealed only after extensive workup with CT scan of the shoulder. After reduction of the shoulder dislocation, the glenoid fracture was stabilized by osteosynthesis. The postoperative results showed anatomical reconstruction of the glenoid joint surface. After completion of therapy, the patient has achieved good functional results with a full range of motion (abduction 180 °) and has been able to work full-time again.

    Notes: Abstract The half life of the 206 kev state of Re187 was determined with delayed coincidence technic to be:T 1/2=(5·70±0·05)·10−7 s. The rotation of the spin of the 206 kev state in a magnetic field has been observed in a differential angular correlation measurement with the 480 kev−72 kevγ−γ cascade. From the observed Larmor frequency a magnetic moment ofμ=(5·02±0·06)μ K for the 206 kev level has been derived. The 480 kev−72 kevγ−γ angular correlation was found to be undisturbed in an aqueous solution of ammoniumtungstate.

    Notes: Zusammenfassung Mit Hilfe des Mößbauer-Effektes wurde Kernresonanzabsorption im 84 keV-Niveau von Yb170 nachgewiesen. Durch Messung der Resonanzabsorption als Funktion der Doppler-Verschiebung zwischen einer Tm170-Metallquelle und einem Yb2O3-Absorber bei 20° K wurde eine Hyperfeinstrukturaufspaltung des 84 keV-Niveaus beobachtet. Aus einer aufgelösten Resonanzlinie ergibt sich die Halbwertszeit des 84 keV-Niveaus zu (1,5±0,2)·10−9 sec. Die Aufspaltung kann durch eine Quadrupolwechselwirkung im Yb2O3 erklärt werden.

    Notes: Abstract The number of Moessbauer quanta from the 23.8 keV state of Sn119 in a tin metal source was measured after attenuation by a 16.1 mg/cm2 absorber of tin metal, as a function of temperature above and below the transition point of superconductivity. Source and absorber were kept at the same temperature. The data give evidence of a discontinuous change at the transition temperature: The transmitted intensity is lowered by (1,3±0.15)·10−3 in the superconducting state of source and absorber. This corresponds to an increase of the Debye-Waller-factor by about 0,4%.

    Notes: Abstract The Mößbauer absorption of the 14.4 keVγ-ray of Fe57 was investigated in Ferrocene, Fe(C5H5)2, and in a Ferriciniumcation [Fe(C H5)2]BF4. The resonance absorption spectra reveal a quadrupole splitting and an isomeric shift of the 14.4 keV level in both compounds. The data give some evidence of the occupation of the molecular orbits formed in these complexes.
