    Notes: Glycopyrronium 5 or 10 μg/kg was administered either simultaneously with, or 1 minute before, edrophonium 1 mg/kg in order to antagonise competitive neuromuscular blockade in 80 children. Both doses of glycopyrronium given before the edrophonium resulted in an initial significant (p 〈 0.01) increase in heart rate. Heart rate decreased significantly (p 〈 0.01) in all groups after the edrophonium was given, and only glycopyrronium 10 μg/kg administered before edrophonium prevented a substantial decrease below baseline. Initial heart rate responses to glycopyrronium or edrophonium are rapid, and measurements at intervals of 30 seconds may be necessary to record these changes.

    Notes: A randomised prospective study was carried out in children undergoing surgery for squint correction, to determine the value of controlled ventilation as a prophylaxis against the occurrence of the oculocardiac reflex. One hundred patients anaesthetised with nitrous oxideloxygen and halothane were randomly assigned to either ventilated or spontaneously breathing groups of 50 each. Half the patients in each group received glycopyrronium 7.5 μg/kg intravenously at the time of induction of anaesthesia. Heart rate, rhythm, blood pressure and end tidal CO2 concentration were monitored throughout. A positive oculocardiac reflex. defined as a fall in heart rate of 20% or more and/or the occurrence of dysrhythmias, was observed in 72% of spontaneously breathing patients and in 100% of ventilated patients not receiving prophylactic intravenous glycopyrronium. The incidence of a positive reflex in patients receiving glycopyrronium was 10% (4 and 16% respectively in spontaneously breathing and ventilated patients). It is concluded that controlled ventilation is of no value as a preventive measure against the occurrence of the oculocardiac reflex in patients undergoing squint surgery and that prophylaxis is safely achieved with the use of intravenous glycopyrronium.

    Notes: Abstract The fatigue behaviour of carbon fibre/PEEK composite is compared with that of carbon/ epoxy material of similar construction, particularly in respect of the effect of hygrothermal conditioning treatments. Laminates of both materials were of 0/90 lay-up, and they were tested in repeated tension at 0° and at 45° to the major fibre axis. The superior toughness of the polyether ether ketone and its better adhesion to the carbon fibres results in composites of substantially greater toughness than that of the carbon/epoxy material, and this is reflected in the fatigue behaviour of the carbon fibre/PEEK. The tougher PEEK matrix inhibits the development of local fibre damage and fatigue crack growth, permitting a 0/90 composite with compliant XAS fibres to perform as well in fatigue as an epoxy laminate with stiffer HTS fibres. Hygrothermal treatments have no effect on the fatigue response of either material in the 0/90 orientation. The fatigue response of a cross-plied carbon/PEEK laminate in the ±45° orientation is much better than that of equivalent carbon/epoxy composites, again because the superior properties of the thermoplastic matrix.

    Notes: Summary The structure of the basement membrane of the high endothelium of reactive human lymph nodes was investigated by techniques selective for carbohydrates (periodic acid-Schiff; critical electrolyte concentration staining with Alcian Blue; lectin histochemistry), specific proteins (immunohistochemistry for laminin and fibronectin) and by conventional techniques of light and transmission electron microscopy. Adjacent small lymphocytes were assigned to B and T cell subsets by use of monoclonal antibodies and they were analysed for non-specific esterase,β-glucuronidase,β-N-acetylglucaminidase and proteolytic activities. The basement membranes were shown to be distinctive and to contain three layers, of differing laminin, glycosaminoglycan and glycoprotein oligosaccharide content. Certain lymphocytes (probably T) contained enzymes potentially able to degrade some components of these basement membranes.
