Summary As a result of the existing bone defect in the alveolar process caused by the unilateral and bilateral clefts, there is not only a weak point in the statics of the maxilla, but also in the periodontium of the adjacent teeth in the cleft area. This causes early loss of those teeth. To maintain the permanent anterior teeth the development of the periodontium should be promoted by bone-grafting at the primary cleft palate-closure, before the eruption of the permanent anterior teeth. The periodontium is secured by the bony healing at the site of the cleft.
Abstract:
Résumé Le manque d'os causé par une fente uni- ou bilatérale au niveau du pro es alvéolaire de la mâchoire supérieure, perturbe non seulement la statique du maxillaire mais affaiblit aussi la parodonte des dents voisines ce qui peut provoquer une perte prématurée de ces dents. Pour conserver les incisives permanentes, on doit assurer le développement du parodonte au moyen d'une transplantation d'os, réalisée au cours de la première intervention chirurgicale, avant l'éruption de ces dents.

Zusammenfassung Infolge des bestehenden Knochendefektes im Alveolarfortsatz durch ein- oder beidseitige Kieferspalten werden nicht nur die Statik des Oberkiefers, sondern auch die Zahnhalteapparate der spaltbenachbarten Zähne geschwächt, wodurch es zum vorzeitigen Verlust dieser Zähne kommt. Um die bleibenden Frontzähne zu erhalten, sollte die Ausbildung des Parodontiums durch knöcherne Ausheilung der Kieferspalten vor deren Durchbruch mittels Knochentransplantat bei der primären Kiefer- und Gaumenplastik gesichert werden.
