    Notes: Summary Six cases of internal ophthalmoplegia due to direct head injury are presented. All six patients had a dilated, nonreactive pupil. Four had no extraocular palsies or ptosis and two had partial extraocular palsies or ptosis. Disturbance of consciousness was absent or very mild, and all patients fully recovered within 1 to 7 days after the traumatic event. No patient had a history that suggested a cause for oculomotor nerve palsy, and emergency CTscans showed no mass lesions. The internal ophthalmoplegia was recognized immediately after trauma. Although minimal oculomotor nerve palsies due to unruptured intracranial aneurysms have been described, none of our patients complained of periorbital or retroorbital pain either before or after the trauma, which rules out intracranial aneurysms as the cause of the internal ophthalmoplegia. Therefore, we concluded that the internal ophthalmoplegia was due to direct head injury. The pathophysiological mechanism of the internal ophthalmoplegia appeared to be slight injury of the pupillomotor fibres on the ventromedial surface of the third nerve at the posterior petroclinoid ligament, which acted as the fulcrum due to the downward displacement of the brainstem at the time of impact.

    Notes: Summary Fibrinopeptide A (FPA) levels as an indicator of thrombin activity in the cerebrospinal fluid (CSF) and plasma of 25 patients with subarachnoid haemorrhage (SAH) were measured serially by radioimmunoassay (RIA). FPA levels in CSF were extremely high on days 0–1 (1253±269 ng/ml, mean ± standard error) but decreased rapidly (11.3±3.9 ng/ml on days 2–4, 10.7±5.9 ng/ml on days 5–7, and 6.3±1.5 ng/ml on days 8–14). In the controls the FPA concentration in CSF was 1.2±0.9 ng/ml (mean ± standard deviation). Plasma FPA levels in patients with SAH showed no statistically significant changes with time. The bradykinin (BK) concentration in CSF and plasma in 27 patients with SAH was measured serially by RIA. The cocentrations in CSF were 122.7±22.7 pg/ml (mean ± standard error) on day 0, 38.6±6.1 pg/ml on day 1,22.7±6.3 pg/ml on day 2, and 17.1±3.0 pg/ml or less thereafter. Plasma BK levels in patients with SAH were higher than those in the control group, but there was no statistically significant change over time. From the measurement of FPA it was apparent that the coagulation system in the subarachnoid space is strongly activated in the early stage of SAH. The formation of BK in CSF after SAH is thought to be due to the contact activation of Hageman factor (intrinsic factor) in the subarachnoid space. Trabeculae as collagen bundles in the subarachnoid space were considered to have a possible role in activating the Hageman factor of the coagulation system in SAH.

    Notes: Summary We present 6 cases with dissecting aneurysm of the intracranial vertebral artery who developed subarachnoid haemorrhage (SAH). The following procedures were performed in this series; trapping of the involved artery in 2, proximal occlusion of the vertebral artery with detachable balloon in 2, and proximal clipping of the vertebral artery in 2. Proximal occlusion of the vertebral artery in 4 and trapping of the vertebral artery in one gave excellent results. We believe the treatment of choice is proximal occlusion of the vertebral artery, either by open surgery or by interventional neuroradiological procedures.

    Notes: Summary An 18-year-old man with an ulceration of his right tonsil and cervical lymphadenopathy was examined. The patient also had a papular eruption in his face, chest, abdomen, and upper extremities. There were no signs of genital involvement. Routine serological tests for syphilis were reactive and histological findings of a cervical lymph node biopsy were characteristic for syphilitic lymphadenitis. Spirochetes were also identified in the tissue sections. A diagnosis of secondary syphilis of the tonsil with cervical syphilitic lymphadenitis was made. In recent years, the number of new cases of syphilis has increased in Japan. Since the oral cavity is the most common extragenital site of syphilis, clinicians should bear in mind that oral lesions from an unknown cause might possibly be syphilitic.

    Description / Table of Contents: Zusammenfassung Es wird über einen Fall von multiplen Knochenzysten berichtet, die sowohl in langen und kurzen Röhrenknochen als auch in platten Knochen lokalisiert waren. Simultane Messungen des Zystenbinnendruckes und der Sauerstoffspannung der Zystenflüssigkeit ergaben ähnliche Werte wie bei typischen solitären Knochenzysten. Kürettage und Knochentransplantation waren bei der Kalkaneuszyste erfolgreich, während die Steroidinjektion in die Metakarpalzyste keinen Effekt hatte. Multiples Anbohren der Ulna- und Radiuszysten war in gleicher Weise effektiv. Diese Resultate lassen vermuten, daß auch in der Ätiologie der multiplen Zysten möglicherweise die venöse Abbflußbehinderung eine wichtige Rolle spielt, wie dies für typische solitäre Knochenzysten gezeigt werden konnte. Es kann jedoch die Möglichkeit nicht ausgeschlossen werden, daß einfache Knochenzysten, solitär oder multipel, so etwas wie eine sekundäre Form anderer Erkrankungen darstellen oder daß es sich bei den einfachen Knochenzysten doch um eine heterogene Gruppe von Veränderungen handelt.
    Notes: Summary We report a case of multiple, simple bone cysts in long tubular, short tubular, and flat bones. The internal pressure of the cysts and the oxygen tension of cyst fluid were measured simultaneously. These values were similar to those of typical solitary cysts in other patients. Curettage and bone graft onto the calcaneal cyst were effective, although steroid injection therapy into the metacarpal cyst had no effect. Piercing the ulnar and radial cysts a number of times was uniformly effective. These results may suggest that venous obstruction of the lesions play an important role in the etiology of multiple simple cysts, as shown in typical bone cysts. However, we could not exclude the possibility that simple bone cysts, solitary or multiple, represent some kind of secondary forms of other disorders, or that a simple bone cyst is a heterogeneous entity.

    Notes: Abstract An undamped undulation superposed on the pulse tail of the passive Q-switching is observed using HCOOH gas as a saturable absorber. The pulse shapes with the undulation are nicely reproduced through the rate-equation analysis in which the laser gain medium is described as a three-level system. Good agreements between the observation and the calculation are also obtained in the dependence of the period and the width of passive Q-switching pulse on laser parameters. The mechanism of the undulation is interpreted as the relaxation oscillation attributed to the relaxation from the lower laser level. The collisional rate constant of HCOOH molecule is also obtained.

    Notes: Abstract A small-signal gain in CO2 waveguide laser medium has been measured on rotational-vibrational transitions in the P-branch of the (0, 0, 1)-(0, 20, 0) band. It has been found that the rotational temperature is well defined in the waveguide laser system where high excitation power is injected and a large amount of energy is flowing through vibrational, rotational, and translational degrees of freedom. The rotational temperature is slightly higher than the translational temperature.

    Notes: Abstract An electron microscopic study was performed on the pigment epithelium of the iris in 13 patients with capsular glaucoma. Pseudoexfoliative material (PE) was often observed in small depressions or indentations of the cell membrane of the posterior pigment epithelium. PE was also found in the region where the cell membrane was indistinct and took a course perpendicular to the surface of the cell membrane. Serial sections revealed no PE in the cytoplasm of the posterior pigment epithelium. Furthermore, the PE found extracellularly was continuous with the cell membrane. Three stages were distinguished in the pigment epithelium as capsular glaucoma progressed. At an early stage (stage 1), the number of infoldings in the posterior pigment epithelium increased, and PE was found at that site. In a more advanced stage (stage 2), the cytplasmic processes increased in number and the basement membrane became discontinuous. In a further advanced stage (stage 3), a large amount of PE was found in the posterior chamber. The intercellular space between two adjacent pigment epithelial cells increased and the cytoplasmic processes gradually disintegrated. Only one significant correlation was found between the glaucoma stage and the stage of changes in the pigment epithelium. No associations were found between the stage of changes in the pigment epithelium and patient age or gonioscopic findings.
