Notizen: 30 patients who had experienced contact urticaria after exposure to gloves, contraceptives, balloons or dental cofferdams went through a detailed examination including interviews on: exposure to rubber: local and distant symptoms; provocation test; skin prick tests with (among other substance) latex extracts from the rubber tree Hevea brasiliensis patch testing. IgE and RAST analyses, 23 patients (77%) reported additional symptoms from distant organs. 13 belonged to medical occupations: 23 were classified as atopics based on history; 21 had raised IgE-values: 25 were RAST-positive to the extracts from Hevea brasiliensis, 13 had a pre-existing hand dermatitis, of whom 6 were patch test positive to standard allergens. Skin prick tests with extracts from Hevea brasiliensis were the most sensitive diagnostic tool, while the RAST analysis was positive in 25/30 cases. These test and analyses including RAST were negative in 40 control subjects. Provocation tests are of great value, but difficult to standardize. Several patients reported severe reactions at medical and dental examination/treatment situation(surgical gloves, dental cofferdams), and the risk of life-threatening anaphylactic reactions is emphasized.

Notizen: Irritant contact dermatitis along with an increased transepidermal water loss can result from exposing the skin to solvents. A study of the interaction of various solvents with human stratum corneum was made using thin-layer chromatography. Comparison of 10 solvents (trichloroethylene, toluene, dimethylsulfoxide, n-hexane, 1-2-propanediol, carbon tetrachloride, methyl ethyl ketone, ethanol, acetone, distilled water) showed that 3 solvents (dimethylsulfoxide, 1,2 propanediol, distilled water) did not extract lipids from intact human stratum corneum. The findings are discussed in relation to the visual appearance of skin (whitening) after exposure to solvents.

Notizen: The influence of injuries on the percutaneous absorption of 3 organic solvent's (butanol, toluene, 1,1,1-trichloroethane) was studied in the guinea pig. Acute (stripping, sandpaper abrasion, needle abrasion, delipidization) and subacute (irritant and allergic contact dermatitis) injuries were induced by physical or chemical treatment of the skin. Absorption of solvents was quantified by blood analysis. Induction of acute injuries markedly altered the absorption pattern of the solvents and correlated to their water solubility, Absorption of toluene and trichloroethane was reduced and that of butanol was increased compared to normal untreated skin Subacute injures had a less pronounced effect. Irritant and allergic contact dermatitis reduced the absorption of toluene and trichloroethane, and no effect was seen for butanol. For hydrophobic solvents, it is concluded that in acute injuries strata other than stratum corneum hold the barrier properties.

Notizen: Irritancy caused by the patch test procedure itself can be evaluated by measuring the skin blood flow with a laser Doppler flowmeter before application of the patches and at intervals after their removal. To investigate this technique further. van der Bend and True test® patches with and without nickel sulfate were applied to the skin of the back and to the volar forearm in healthy subjects. Immediately after removal of the patches, the blood flow values in 26 of 32 test sites were higher than the corresponding pre-application values. In the majority of cases, blood flow had returned to pre-application values at 72 h (24 h after removal). This favours these patches compared to those used in previous studies (A1-test, Finn chambers, Silver patches), which caused a more lasting effect on skin blood flow. The common practice of relying mainly on the 72 h and 96 h readings of patch test sites was thus justified.

Notizen: In order to investigate whether abietic acid itself is the allergenic component of Colophony, 2 commercial samples were extensively purified and tested in guinea pies and in colophony-sensitive patients. In the modified FCAT method, sensitization was obtained with Portuguese colophony and when challenged with puritied abietic acid, the animals showed no reaction. In the GPMT method, the animals were exposed to purified abietic acid. Challenging with 2 different samples of it gave no significant reaction. When patch tested, patients sensitive to colophony showed no reactions to abietic acid which had been purified immediately before the test. It is concluded that abietic acid itself is not a contact allergen

Notizen: According lo some reports, mainly from Norway, visual display terminal (VDT) work has been thought lo cause rashes. In conjunction with an epidemiological study of office employees and VDT work, a dermatological examination was made. No case demonstrated the clinical picture described from Norway among VDT operators, but subjects with rosacea. seborrhoeic dermatitis and acne were over-represented in the VDT-exposed group. The object of the present study was to see whether patients with rosacea experienced aggravation of their symptoms as a result of VDT work and whether they showed a particular clinical picture. All subjects tit for work among the patients diagnosed as rosacea or perioral dermatitis during 1982 were selected for the study. The methods included a questionnaire on VDT work and suspected impairment of the skin disease, clinical examination and telephone interviews, 179 subjects were selected; 42 of them worked daily with VDT and 8 of them suspected impairment due to VDT work. The results imply that a relationship may exist between rosacea and VDT work. To what factor in the work this may he ascribed is still unclear. Those subjects whose symptoms were aggravated did not show any particular clinical picture.
