Zusammenfassung Im Allgemeinen zeichnet sich diese Kategorie von Unfällen dadurch aus, dass die Verletzungen verhältnismässig gutartig sind. Diese Feststellung erklärt wohl die Tatsache, dass diese Pathologie in der medizinischen Literatur nur wenig vertreten ist, abgesehen von klinischen Studien an hospitalisierten Patienten. Anhand einer Untersuchung, die 9261 Schulsportunfälle in Basel und Genf erfasst, werden die wichtigsten epidemiologischen Charakteristiken sowie Schweregrad und Lokalisation der den Versicherungen gemeldeten Körperschädigungen erläutert. Für die epidemiologische Beurteilung benötigen wir die Sicherheit, dass gewisse Bedingungen erfüllt sind. Diese betreffen die zu Beginn festzulegenden Definitionen betreffend das methodische Vorgehen, die Herkunft der Angaben, das Mass der Expositionsdauer in bezug auf das Unfallrisiko und die — übrigens umstrittene — individuelle Unfallprädisposition. Im Hinblick auf die Verhütung wird ein Vorgehen skizziert, das die Faktoren berücksichtigt, welche die vom Lehrer zu erteilenden Anweisungen, die besonderen Umstände und vor allem den Schüler als ein in voller Entwicklung befindliches Individuum betreffen.
Abstract:
Summary Generally speaking, this category of accidents seems to cause mostly relatively mild injuries. It could explain, at least in part, why there are so few papers on this topic in the medical literature except for clinical reports usually based on hospitalized cases. The main results of a study of the 9261 physical education accidents registered within five years in primary and secondary schools in Basle and Geneva are used to illustrate their epidemiological caracteristics, as well as the localization and the severity of the injuries. The sources of data are the files of the accident insurance companies. Initial operational definitions, data sources, measurement of exposure to risk and the (largely criticized) notion of individual accident proneness are constraints and conditions of a pertinent epidemiological approach which have to be carefully taken into consideration. This appreach should be combined with the clinical and the ergonomical approaches in order to get the best possible pictures of the causes, mechanisms and consequences not only of accidents in general but especially of those occurring in physical activities in childhood and adolescence. Some considerations on prevention are briefly outlined here. They intend to take into account the risk factors related to the tasks, the circumstances and especially to the student himself taken as an individual involved in a growth and development dynamics.

Résumé D'une manière générale, cette catégorie d'accidents semble se distinguer par le caractère relativement bénin des traumatismes qu'ils entraînent. Cette constatation explique probablement le faible intérêt qu'ils semblent avoir suscité dans la littérature médicale, en dehors des études cliniques, habituellement basées sur les cas hospitalisés. Les résultats d'une étude portant sur 9261 accidents survenus dans les écoles de Bâle et Genève servent à illustrer leurs caractéristiques épidémiologiques principales, ainsi que la gravité et la localisation des traumatismes signalés à l'assurance-accident. L'approche épidémiologique exige qu'un certain nombre de conditions soient remplies, relatives aux définitions opérationnelles initiales, aux sources de données, à la mesure de l'exposition au risque et à la notion, critiquée d'ailleurs, de prédisposition individuelle. L'approche préventive est esquissée, dans une perspective de nature à prendre en compte les facteurs liés aux consignes, aux circonstances et surtout à l'élève lui-même, en tant qu'individu en plein développement.
