Zusammenfassung Zur Charakterisierung der inotropen Wirkung von Pharmaka mit blutdrucksenkender Wirkung wurden an narkotisierten Katzen der linksventrikuläre Druck (P) und die linksventrikuläre Druckanstiegsgeschwindigkeit (dP/dt) mit Hilfe eines Digitalrechners (Dietz Mincal 621) registriert und die Ventrikeldruckkurven im isovolumetrischen Abschnitt unter Verwendung eines Off-line-Programms analysiert. Die Beurteilung von Änderungen der Kontraktilität erfolgte mit dem von der Nachlast weitgehend unabhängigen Parameter (dP/dt)/CPIP (CPIP=peak common developed isovolumetric pressure). Ferner wurde die beim instantanen Ventrikeldruck (IP) der Versuchskurve gemessene Differenz zwischen den dP/dt-Werten von Versuchs- und Kontrollkurve (Δ dP/dt) in Abhängigkeit von der IP-Senkung (Δ IP) sowie der Abnahme des diastolischen Druckes dargestellt. 10 min nach Infusion von 1,69 bzw. 2 mg/(kg×min) Lidocain bzw. Carticain wurden CPIP und die entsprechenden (dP/dt)/CPIP-Werte bestimmt. Ausgehend von den Kontrollwerten (Lidocain 90,2±8,7 s−1 (n=8) und Carticain 93,6±5,7 (n=7) verursachten die beiden Lokalanästhetika mit zunehmender Dosierung bei halblogarithmischer Darstellung eine etwa gleich starke lineare Abhahme des Quotienten (nach 10 min: Lidocain 40,3±4,1 s−1 und Carticain 46,4±3,9 s−1). Die bereits nach 1 min bei beiden Lokalanästhetika beobachtete signifikante Abnahme des Quotienten (dP/dt)/CPIP (P〈0.005) ist Ausdruck der negativ inotropen Wirkung, die eher auftritt und prozentual stärker ist als die blutdrucksenkende Wirkung. Der Blutdruck nahm nach 10minütiger Infusion von 146,5±5.0/106,7±5.0 mm Hg auf 115,6±5,0/79,8±4,3 mm Hg (Lidocain) und von 163,6±7,3/118,2±5,0 mm Hg auf 128,0±7,3/90,4±6,3 mm Hg (Carticain) ab. Nach Infusion von 13 μg/(kg×min) Nitroprussidnatrium (NPN) blieb die Kontraktilität bis zu einer Gesamtdosis von 100 nMol/kg bzw. bis zu einer diastolischen Drucksenkung um ca. 35% unbeeinflußt. Nach i.v. Injektion von Nifedipin (15 μg/kg) und Verapamil (100 μg/kg) verhielt sich die prozentuale Abnahme von IP proportional zur Abnahme der Inotropie, die mit Hilfe der beim jeweiligen Ventrikeldruck IP gemessenen dP/dt-Änderung ermittelt wurde, nach 5 mg/kg Papaverin dagegen proportional zur Zunahme der so gemessenen Inotropie.

Summary Left ventricular pressure (P) and its rate of increase (dP/dt) were recorded using a digital computer (Dietz Mincal 621) and ventricular pressure curves during isovolumetric contraction were analysed by an off-line programme in order to characterize in anaesthetized cats the inotropic action of drugs which decrease blood pressure. Changes of contractility were examined by the application of the parameter (dP/dt)/CPIP (CPIP=peak common developed isovolumetric pressure) which is nearly independent of the afterload. Furthermore, the difference between the dP/dt-values, measured at the instantaneous ventricular pressure (IP), of experimental and control curves (Δ dP/dt) was illustrated in relation to the decrease in IP (Δ IP) and diastolic pressure. CPIP and corresponding (dP/dt)/CPIP ratios were determined 10 min after infusion of 1,69 and 2 mg/(kg×min) lidocaine and carticaine, respectively. With increasing doses both local anaesthetics effected an approximately equal linear decrease of the above ratio using a half-logarithmic scale. (control: lidocaine 90.2±8.7 s−1, n=8 and carticaine 93.6±5.7, n=7 and after 10 min: lidocaine 40.3±4.1 s−1 and carticaine 46.4±3.9 s−1). The significant decrease (P〈0.005) of the ratio (dP/dt)/CPIP, which was observed as soon as 1 min after application of both local anaesthetics, demonstrates the negative inotropic action, which occurs earlier and is percentually more effective than the decrease in blood pressure. Blood pressure decreased after a 10 min infusion from 146.5±5.0/106.7±5.0 to 115.5±5.0/79.8±4.3 mm Hg (lidocaine) and from 163.6±7.3/118.2±5.0 to 128.0±7.3/90.4±6.3 mm Hg (carticaine). Contractility remained unchanged after an infusion of 13 μg/(kg × min) sodiumnitroprusside (NPN) up to 100 nM/kg or up to a decrease in diastolic pressure of about 35%. The percentual decrease in IP, after an i.v. injection of nifedipine (15 μg/kg) and verapamil (100 μg/kg), was proportional to the decrease in inotropy, measured as the change in the dP/dt-values at the respective ventricular pressure IP, whereas the effect of 5 mg/kg papaverine was proportional to the increase in inotropy.
