In randomisierter Reihenfolge wurden 75 mg Hydrochlorothiazid mit und ohne vorausgegangener 6tägiger Phenytoin-Therapie (300 mg/die) oral verabreicht. Es fanden sich große intra- und interindividuelle Unterschiede in der Bioverfügbarkeit von HCT (32–87% bzw. 42–77%), Phenytoin zeigte jedoch keinen Einfluß auf die Dispositionsparameter des Diuretikums.
    Notes: Summary Plasma levels and urinary recovery of Hct were determined in seven healthy male volunteers. 75 mg Hct were administered as a tablet in a randomised fashion with or without phenytoin pretreatment (300 mg/d). Bioavailability of Hct showed considerable intra- and interindividual variation (32–87% and 42–77% respectively), but phenytoin did not influence the disposition parameters of the diuretic.

    Notes: Summary The effect-of cimetidine on hepatic and systemic haemodynamic parameters was studied in seven patients with portal hypertension due to alcohol-induced cirrhosis of the liver and in one patient with peliosis hepatis following oral contraceptive steroids. The intravenous administration of cimetidine (350 mg as bolus, followed by 2 mg/min over 60 min) reduced the hepatic extraction of continuously infused indocyanine green (ICG) by 27%; this was statistically significant (P〈0.01). Since the ICG clearance, calculated independently of hepatic perfusion, was lowered by 19%, this effect seems to be mainly due to a reduced capacity of the liver to remove the dye from the blood, rather than due to changes in perfusion. Cimetidine did not influence the elevated portal pressure in the patients with cirrhosis, or the normal pressure in the patient with peliosis hepatis. No significant effect was observed on heart rate, mean arterial pressure, pulmonary artery pressure, pulmonary capillary pressure and cardiac output. These studies indicate that the reduction of the hepatic ICG extraction following cimetidine is more the result of an inhibited capacity of the liver to remove the dye than of changes in the hepatic perfusion or in the systemic circulation.

    Notes: Summary Head velocity-related modulations in Purkinje cell activity were observed in lobules VI and VII of the cerebellar vermis. The sensitivities and phase shifts with respect to head velocity of Purkinje cells were comparable with vestibular responses observed in the fastigial nuclei. The results support the hypothesis of a vermal target velocity correlate and necessitate the inclusion of vermal lobules VI and VII among the cerebellar regions involved with vestibular information processing.

    Notes: Summary The visual properties of 77 dorsolateral pontine nucleus (DLPN) cells were studied in two alert monkeys. In 41 cells, presentation of a moving random dot background pattern, while the monkeys fixated a stationary spot, elicited modulations in discharge rate that were related either to (i) the velocity of background motion in a specific direction or to (ii) only the direction of background movement. Thirty-six DLPN cells exhibited responses to small, 0.6–1.7 deg, visual stimuli. Nine such cells exhibited non-direction selective receptive fields that were eccentric from the fovea. During fixation of a stationary bluish spot, the visual responses of 27 DLPN cells to movement of a small, white “test” spot were characterized by two components: (1) as the test spot crossed the fovea in a specific direction, transient velocity-related increases in discharge rate occurred and (2) a maintained, smaller increase in activity was observed for the duration of test spot movement in the preferred direction. This DLPN activity associated with small visual stimuli was also observed during smooth-pursuit eye movements when, due to imperfect tracking, retinal image motion of the target produced slip in the same direction. These preliminary results suggest that the DLPN could supply the smooth-pursuit system with signals concerning the direction and velocity of target image motion on the retina.

    Notes: Summary After rapid intravenous injection of furosemide 40 mg (Fu), plasma levels were determined in 7 healthy volunteers, 8 patients with liver cirrhosis with ascites and 7 patients with end-stage renal disease (ESRD). The diuretic response was evaluated by measuring the urinary excretion of sodium and potassium and the urine volume. The mean elimination half life (tF1/2+) of Fu averaged 51±7.7 (±SD) min in healthy subjects, 52±7.7 min in cirrhosis and 200±57 min in ESRD. The non-renal clearance (Clnr) in healthy subjects (56±28 ml/min) corresponds to the total plasma clearance in functionally anephric patients (54±18 ml/min). In cirrhosis there was no significant change in the disposition parameters of Fu in comparison to the healthy volunteers, but there was a significant reduction in urine sodium and volume, whereas potassium excretion remained unchanged. Fu “excretion rate — response” curves showed diminished tubular sensitivity to Fu in cirrhosis.

    Notes: Abstract: The increased levels of NAD(P)H effected by electrical depolarization are markedly augmented in the presence of cyclic AMP, isoproterenol, or RO 20–1724, agents known to elevate cyclic AMP in rat brain slices. The data presented indicate that the cyclic AMP effect on an important component of intermediate metabolism is not an enhancement of a basal response but a separate response that is activated by depolarization, is Ca2+–dependent, regulates cytochrome a-a3 independently of its effects on NAD(P)H levels, and is dependent on a substrate other than glucose.

    Notes: Summary We investigated the relation between the occurrence of fetal breathing movement and maternal blood sugar levels. Fetal breathing movements tended to be less with low maternal blood sugar levels and increased as blood sugar levels rose. However, in a significant proportion of cases, fetal breathing movements showed no relation to maternal blood sugar levels.
