    Notes: Abstract The purposes of this study were to investigate the effect of direct application of chlorhexidine to periodontal pockets and the practicability of patient self-therapy using a technique of subgingival irrigation. Patients received no other oral hygiene instruction. After initial assessment of parameters, patients were given scaling and polishing and then instruction in the irrigation of designated pockets with chlorhexidine or a placebo using a disposable syringe and blunt needle. During the 28-day irrigation period with chlorhexidine there was a highly significant reduction in periodontal inflammation which was maintained at levels significantly below the baseline values for a further 28-day period without irrigation. There was a deterioration in the periodontal state of those patients who had used the placebo. The irrigation technique itself caused no discernible injury in this group of routine periodontal patients. Also, staining in the chlorhexidine group was minimal. It is concluded that subgingival irrigation with chlorhexidine is effective in reducing periodontal inflammation and in controlling subgingival plaque. Intermittent treatment of this kind by the patient at home might reduce to more manageable levels the frequency of hygiene visits and the need for rigorous interdental oral hygiene.

    Notes: Abstract. This study was designed to clarify the relationship between amalgam restorations with subgingival overhanging margins and gingival health. This required the presence of such a restoration defect as the sole variable parameter. This was achieved by pairing 26 defective premolar and molar amalgams with similarly defective restorations in the same mouths, test and control being allocated randomly. Test amalgams were contoured to remove overhangs. All other procedures, namely, scaling, polishing and oral hygiene instruction were applied with equal emphasis. Plaque accumulation, gingival inflammation and pocket depths were recorded at 0, 4, 8 and 12 weeks. At the baseline (week 0) there were no significant differences between test and control sides for any of the parameters. Significant differences between test and control sides for all parameters were apparent at all other times.The greatest change took place during the first 4 weeks of the study. It was concluded that the presence of a subgingival overhanging defective margin may be the only important clinically significant feature of an amalgam restoration related to the pathogenesis of chronic inflammatory periodontal disease. It was also demonstrated that correction of defective restorations should be effected during the initial phase of periodontal therapy.

    Notes: Abstract This study has confirmed the presence on the root surface of teeth affected by chronic periodontitis of a zoned organic integument analogous to that on enamel. Measurements showed a highly significant correlation between the levels of the bounding structures of this zone, that is, the apical plaque border and the coronal border of residual periodontal ligament, for both shallow and deep pockets. The middle zone between these two was on average 1 mm wide.

    Notes: Abstract Teeth destined for extraction and exhibiting periodontal pockets had reference grooves cut on the proximal surfaces at the level of the gingival margin. A disclosing agent was then applied either as a mouthrinse or by direct irrigation at the entrance of the pocket. After extraction, the extent of apical penetration of the solution was measured from the reference point. Subsequently, the tooth was stained enabling similar measurements from the reference point to the level of the apical plaque border and the coronal level of the residual periodontal ligament.Analysis of these measurements indicated that neither mouthrinsing nor direct irrigation routinely achieved penetration of the solution to the apical plaque border. While mouthrinsing failed to achieve any significant penetration of pockets, the direct irrigation technique was partially effective.It was concluded that direct irrigation offers promise as a mode of application of chemical agents to subgingival plaque.

    Notes: Abstract Eleven patients suffering acute exacerbations of chronic periodontitis were treated by the insertion into affected periodontal pockets of dialysis tubing containing 20% chlorhexidine gluconate. After 7 days, 9 of the 11 patients exhibited greatly reduced crevicular fluid flow, relief of discomfort and the cessation of bleeding on probing.

    Notes: Abstract The present investigation compared subgingival metronidazole in dialysis tubing and subgingival chlorhexidine irrigation in the control of chronic inflammatory periodontal disease. 10 patients with 4 mm or deeper periodontal pockets were divided into 2 groups. Both received baseline scaling, root planing, subcontact area cleaning and instruction in the Bass technique of tooth brushing, but not in interdental cleaning. One group with 80 pockets received 0.2% chlorhexidine subgingival irrigation for 28 days and the other with 86 pockets received 0.5 % metronidazole solution incorporated in subgingivally placed dialysis tubing. The tubings were replaced with freshly filled ones at days 7, 14 and 21. Active treatment ceased at day 28. Plaque Index, Sulcus Bleeding Index, pocket depth and gingival shrinkage were recorded at days 0, 7, 14, 21, 28, 56 and 84.Subgingival 0.5% metronidazole in dialysis tubing and 0.2% chlorhexidine irrigation were found to be equally effective in reducing chronic periodontitis. Metronidazole reduced Plaque Index less but pocket depth more than chlorhexidine. Improvements were maintained significantly below baseline levels for at least 8 weeks after the end of the 4-wcek treatment period.

    Notes: Abstract. One child and one adult with severe periodontitis were investigated for relevant systemic factors and predominant periodontal pocket bacteria. The child had a chronic neutropenia, the adult late yaws, a chronic iron deficiency and possibly rheumatoid arthritis. The predominant organisms in both pocket floras were gram-negative cocci showing occasional filament formation and resembling strains of Bacteroides asaccharolylicus and possibly Actinobacillus actinomyceiemcomitcins described by others. There were indications that the flora was determined by the host response rather than vice versa and that thorough systemic investigation may aid the efficient diagnosis and treatment of patients with severe periodontitis.

    Notes: Abstract The aim of this study was to compare the penetrability of periodontal pockets by two direct irrigation techniques. By use of a syringe and a blunt hypodermic needle, irrigation was performed with a disclosing solution either as the level of the gingival margin or 3 mm within the pocket. The latter, “deep” irrigation, was shown to be an effective method of reaching the apical plaque border in both shallow and deep pockets. It is suggested that deep irrigation may prove a suitable approach to the effective control of subgingival plaque.

    Notes: Abstract The present study is one of a series aimed at devising practical methods of control of chronic periodontitis that do not depend upon stringent interdental cleaning by the patient. The regime consists of professional and home care components thought to be realistic for general practice. Patients with chronic periodontitis, but without any complicating factor, such as systemic disease, recent antimicrobial therapy or occlusal traumatism were selected. They received an initial thorough scaling, root planing and an instruction in Bass brushing, using a multituft brush and a sodium fluoride toothpaste, but without emphasis on inter-dental cleaning. They were then assigned to one of two groups, one of which received, in addition to the above mechanical oral hygiene regime, low (0.5 %) and the other high (40 %) dose metronidazole locally in periodontal pockets once a week for 4 weeks. The low dose was supplied in the form of solution inside fine dialysis tubing. The high dose was in the form of metronidazole powder incorporated in acrylic resin. Plaque index, sulcus bleeding index, probeable pocket depth and gingival shrinkage were monitored during this period and for a further 8 weeks. Both groups showed highly significant reductions in (supragingival) plaque and periodontal inflammation, maintained at least 8 weeks after the end of the 4-week chemotherapy period. Reductions in SBI were greatest for the 40% metronidazole group. It was concluded that local metronidazole, even in the form of 0.5% metronidazole in a small portion of dialysis tubing, has a positive role in the control of chronic periodontitis.
