Summary The metabolic disorder and the functional impairment of the kidney cause elevated serum sulphate levels in Addison's disease.Sturm andPothmann reported decreased serum sulphate levels in Addison's disease. In our patient this could not be observed because of functional renal insufficiency. Gluco- and mineralocorticoid medication caused a decrease in serum sulphate and enhanced urinary sulphate excretion. The increase in urinary sulphate excretion is due to the amelioration of renal function in the course of mineralocorticoid treatment and to the specific katabolic effect of glucocorticoids on sulphate metabolism demonstrated byMann.

Zusammenfassung Der anorganische Serumsulfatspiegel findet sich beim Morbus Addison erhöht, einmal aufgrund der für diese Erkrankung charakteristischen Stoffwechsellage und zum anderen durch die mangelhafte Ausscheidung der in ihrer Funktion beeinträchtigten Niere, hervorgerufen durch die fehlende Wirkung der Mineralocorticoide. Eine Erniedrigung des anorganischen Serumsulfatspiegels, wie sieSturm undPothmann beschreiben, war bei unserem Kranken mit ausgeprägtem extrarenalem Nierensyndrom nicht gegeben. Unter der Einwirkung der Glucocorticoide und Mineralocorticoide kommt es zu einem Abfall der Serumsulfatkonzentration und zu einer erhöhten Ausscheidung des anorganischen Sulfates im Urin. Diese erhöhte Ausscheidung erklärt sich 1. durch die Normalisierung der Nierenfunktion aufgrund der Einwirkung der Mineralocorticoide und 2. durch die vonMann gezeigte spezifische katabole Wirkung der Glucocorticoide auf den Sulfatstoffwechsel.
