    Notes: Summary Magnetic resonance scans of 74 patients with multiple sclerosis participating in a controlled trial were compared 6 months before and at the end of a 24–32 months-treatment period with either Cyclosporin A (n=31) or Azathioprine (n=43). Both qualitative rating and computation of lesion volume showed deterioration in more than 40% of the patients, while by clinical criteria only 10–30% were worse. No significant difference was noted when the two treatment groups were compared. If careful repositioning and standardized image parameters are used, MRI is an indispensable tool for the objective determination of disease progression in MS although it cannot replace clinical examination.

    Notes: Summary 1. During voluntary contraction, a muscle twitch set up by a stimulus over that muscle is followed by a long lasting inhibition of 120 ms (tibialis anterior) in normal persons. 2. To decide if this is a antidromic effect or a true autogenetic inhibition we proved the muscle twitch in patients with severe damage of muscle afferents or dorsal roots: Tabes dorsalis and Polyneuritis. The muscle twitch in these patients did not change the voluntary innervation. There is no inhibition at all. 3. With a stimulus strength sufficient to evoke a good muscle contraction a stimulus directly set up over the peripheral nerve did not show any antidromic inhibition of the voluntary innervation. 4. Without proprioceptive influence the frequence of motoneurones is still quite regular. This regularity is probably due to the intracentral feedback mechanism over the recurrent collaterals and the Renshaw-Interneurones.

    Notes: Zusammenfassung Es wird über eine Patientin mit einem Cushingschen Syndrom berichtet, die therapeutisch mittels einer auf stereotaktischem Wege vorgenommenen Hypophysenimplantation von Yttrium90 sehr gut beeinflußt wurde. Innerhalb von 4–5 Monaten nach der Implantation von zwei Yttrium-Stäbchen mit einer Gesamtaktivität von 3,4 mC bildeten sich alle Symptome des Cushingschen Syndroms zurück. Eine Beeinträchtigung der Funktion der übrigen, von der Hypophyse abhängigen endokrinen Drüsen erfolgte nicht. Es kam lediglich zum Auftreten einer leichten sekundären Nebennierenrindeninsuffizienz, die keine Substitutionstherapie erforderlich machte. Verschiedene endokrinologische Untersuchungen, wie die Vornahme des Metopiron-Testes und des ACTH-Testes, gewährten einen Einblick in die Veränderungen der Hypophysen-Nebennierenrinden-Funktion im Anschluß an die Implantation. Im Vergleich zu anderen bei der Behandlung des Cushingschen Syndroms gebräuchlichen Verfahren stellt die Implantation von Yttrium90 einen verhältnismäßig schonenden, aber therapeutisch wirksamen Eingriff dar.

    Notes: Zusammenfassung Bei einer Vertebralisarteriographie kam es durch manuelle Kompression der homolateralen Halsschlagader zu einer Teildarstellung dieses Gefäßes über die Arteria primitiva trigemini. Dieser Versuch wurde unternommen, um die Annahme zu bekräftigen, daß die Stromrichtung in der Arteria primitiva trigemini wie im Ramus communicans posterior wechselt und ihre Darstellung im wesentlichen von den hämodynamischen Verhältnissen während der Arteriographie abhängt.
